Clinical, behavioral, and physiological characteristics of isolated rapid eye movement sleep behavior disorder patients with ambulatory dream enactment behaviors.

STUDY OBJECTIVES: Isolated rapid eye movement (REM) sleep behavior disorder (iRBD) involves dream enactment behaviors (DEBs) during REM sleep, accompanied by REM sleep without atonia (RWA). While most DEBs are bed-confined, a minority engage in ambulatory behaviors. This study investigated clinical, behavioral, and physiological characteristics of iRBD patients exhibiting ambulatory behaviors. METHODS: We retrospectively reviewed electronic medical records of 361 patients with video-polysomnography (v-PSG)-confirmed iRBD (mean age: 66.9 years; 59.0% males). Patients reporting at least one DEB

The effects of RBD medications on dream content: A critical review of evidence.

REM sleep behavior disorder, often an early marker of α-synucleinopathies, is characterized by the loss of REM-related muscle atonia and the occurrence of dream enactment behaviors. Dream reports in REM sleep behavior disorder are frequently described as more vivid, violent, and negative than typical dreams. However, evidence for this remains inconsistent, likely due to methodological variability and unaddressed confounds - most notably, the influence of pharmacological treatments. The extent to which medications affect REM sleep behavior disorder dream content remains poorly understood. This

Video-Polysomnography in Peripheral Nerve Hyperexcitability: Clues to Morvan Syndrome in Two Patients and Literature Review.

Peripheral nerve hyperexcitability syndromes are characterized by spontaneous motor discharges, fasciculations, and cramps. Morvan syndrome differs from Isaacs syndrome by its central nervous system involvement, autonomic dysfunction, and profound sleep disruption, often presenting as agrypnia excitata. We report two seropositive patients with cramps, fasciculations, insomnia, and autonomic dysfunction but without neuropsychiatric features. Case 1, a 49-year-old man, had LGI1 antibodies and presented with insomnia and autonomic symptoms. Video-polysomnography revealed severe insomnia with abse

Rapid eye movement sleep contributes to the formation of new axonal varicosities in mouse cerebellar parallel fibers after motor training.

Synaptic structural plasticity is essential for the development, learning and memory. It is well established that sleep plays important roles in synaptic plasticity after motor learning. In cerebellar cortex, parallel fibers of granule cells make excitatory synapses to the dendrites of Purkinje cells. However, the synaptic structural dynamics between parallel and Purkinje cells after motor training and the function of sleep in cerebellar synaptic plasticity remain unclear. Here, we used two-photon microscopy to examine presynaptic axonal structural dynamics at parallel fiber-Purkinje cell syna

Deciphering Insomnia: Benchmarking Automated Sleep Staging Algorithms for Complex Sleep Disorders.

Polysomnography (PSG) is essential for diagnosing sleep disorders, but its manual interpretation is labor-intensive. Automated sleep staging algorithms are promising, yet their utility in complex sleep disorders such as insomnia remains uncertain. This study evaluates five of the most recognised sleep staging classifiers-U-Sleep, STAGES, GSSC, Luna and YASA-on PSG data from 904 patients with chronic insomnia. Performance was assessed using F1 scores, confusion matrices and predicted sleep metrics. The effect of demographics, sleepiness and PSG metrics on each classifier's performance was asses

[Modulation by the hypocretinergic/orexinergic neurotransmission system in sleep-wakefulness cycle states].

INTRODUCTION: The hypocretins/orexins are neuropeptides synthesized by a small neuronal cell group located in the posterior and lateral hypothalamus. These peptides have been considered modulators of the sleep-wakefulness cycle since their discovery in 1998; the hypocretinergic/orexinergic system is very active during wakefulness. In addition, the absence of either these peptides or their receptors is associated to narcolepsy-cataplexy, a disease in which the sleep-wakefulness cycle is completely disorganized. DEVELOPMENT: Hypocretinergic/orexinergic neurons directly activate the cerebral cor

Influence of apnea type and sleep stage on nocturnal postapneic desaturation.

We examined the influence of apnea type and sleep stage on the severity of apnea-induced desaturations in 32 patients with a sleep apnea syndrome. The individual postapneic desaturations were evaluated by a desaturation curve; this curve was built by plotting the fall in SaO2 after an apnea against this apnea's length for each apneic event during the whole night recording. We considered only apneas where the preapneic SaO2 was greater than 90% and the lowest SaO2 value after the apnea was equal to or greater than 60% (limit of linearity of our oximeter). From the desaturation curve, we determi

The effects of a dual orexin receptor antagonist on fear extinction memory and sleep in mice: Implications for exposure therapy.

Extinction of conditioned fear is considered a fundamental process in the recovery from posttraumatic stress disorder and anxiety disorders. Sleep, especially rapid-eye-movement (REM) sleep, has been implicated in promoting extinction memory. The orexin system contributes to the regulation of sleep and wakefulness and emotional behaviors. In rodents, administrations of an orexin receptor antagonist following fear extinction training enhanced consolidation of extinction memory. Although orexin antagonists increase sleep, including REM sleep, the possible contribution of sleep to the effects of

Extract of Ganoderma lucidum potentiates pentobarbital-induced sleep via a GABAergic mechanism.

Ganoderma lucidum has been used for the treatment of a variety of diseases. For the first time here we report a detailed study on the mechanisms and effects of G. lucidum aqueous extract (GLE) on sleep and its sedative activity. GLE showed no effects on sleep architecture in normal rats at doses of 80 and 120 mg/kg. However, GLE significantly decreased sleep latency, increased sleeping time, non-REM sleep time and light sleep time in pentobarbital-treated rats. Suppression of locomotor activity in normal mice induced by GLE was also observed. Flumazenil, a benzodiazepine receptor antagonist, a

Assessment of glymphatic function in narcolepsy using DTI-ALPS index.

INTRODUCTION: Sleep is a modulator of glymphatic activity which is altered in various sleep disorders. Narcolepsy is a sleep disorder characterized by excessive daytime sleepiness (EDS), rapid onset of rapid eye movement (REM) sleep, cataplexy, disturbed night sleep with fragmentation. It is categorized into two types, type 1 (NT1) and type 2 (NT2) depending on the presence of cataplexy and/or absence of orexin. We sought for alterations in glymphatic activity in narcoleptic patients using diffusion tensor imaging (DTI) along perivascular space (ALPS) index on magnetic resonance imaging (MRI).

A non-neurodegenerative REM parasomnia with immersive dreaming and dream-reality confusion: a case report.

BACKGROUND: REM sleep behavior disorder (RBD) is characterized by loss of normal muscle atonia during REM sleep, often associated with dream enactment behaviors, and is typically a prodromal neurodegenerative condition in middle-aged and older adults. However, emerging case reports and case series suggest that not all RBD presentations follow this trajectory, particularly in younger individuals. CASE PRESENTATION: A case of 7-year history of vivid, immersive dreaming perceived as continuous with waking life, accompanied by persistent dream-reality confusion, is described. The patient frequent

Sleep in the dromedary camel: features of the 'first night effect'.

The 'first night effect' (FNE) is a well-known phenomenon in polysomnographic (PSG) sleep studies, resulting in significant variations in the macrostructure of wakefulness and sleep states, particularly between the initial and subsequent sleep recording sessions. The FNE phenomenon during sleep has been studied in various species, revealing complex variations between several sessions of sleep recording. The present study used a non-invasive PSG method to examine differences between various vigilance states in four adult female dromedary camels during 4 consecutive nights and days of sleep reco
