Association of palatine tonsil size and obstructive sleep apnea in adults.

OBJECTIVE: The relationship between palatine tonsil (PT) size and obstructive sleep apnea (OSA) has not been well established in adults. The purpose of this study was to test the association between PT grade, PT volume, and OSA severity in U.S. adult patients. STUDY DESIGN: Cross-sectional study of all patients (age ≥ 18 years) who underwent pharyngeal surgery for OSA that included palatine tonsillectomy with tonsil volume measurement from January 2011 to June 2016. METHODS: Medical records were reviewed for PT grade (measured on clinical exam by the Brodsky tonsil grading scale), PT volume

Subtypes of Maxillomandibular Advancement Surgery for Patients With Obstructive Sleep Apnea.

Maxillomandibular advancement (MMA) surgery, which is the most effective treatment modality for patients with moderate-to-severe obstructive sleep apnea with apparent skeletal discrepancies, has been modified in conjunction with segmental osteotomies, counterclockwise rotation of maxillomandibular complex, and other adjunctive procedures. However, any single type of MMA could not treat or cure all the patients with obstructive sleep apnea showing different dentofacial and pharyngeal patterns. We aimed to suggest critical decision factors for the selective application of MMA subtypes, categoriz

Obstructive sleep apnea syndrome and hypothyroidism - merely concurrence or causal association?

The prevalence of obstructive sleep apnea-hypopnea syndrome (OSAHS) ranges from 4 to 7% in men and from 2 to 5% in women. Its deleterious consequences such as traffic accidents, cardiovascular complications increasing morbidity and mortality, make it a major health problem. Apart from obesity (a major risk factor for OSAHS), hypothyroid patients are prone to reveal this phenotype. Although hypothyroidism seems an acknowledged risk factor for OSAHS, some authors report the lack of clinically relevant association. The argument partly depends on the increased prevalence of hypothyroidism in OSAHS

Risk factors for residual obstructive sleep apnea after adenotonsillectomy in children.

OBJECTIVES/HYPOTHESIS: To determine the prevalence of residual obstructive sleep apnea (OSA) in children who had adenotonsillectomy (AT) and to identify the risk factors for residual OSA after AT. STUDY DESIGN: Retrospective chart review. METHODS: Children with OSA who had AT at a tertiary care children's hospital were reviewed. Data pertaining to demographics, past medical history, body mass index, tonsil and adenoid size, and polysomnography were obtained. Residual OSA was defined as apnea hypopnea index (AHI) greater than 2. The rate of residual OSA and risk factors for residual OSA were

Obstructive Sleep Apnea With Objective Daytime Sleepiness Is Associated With Hypertension.

Subjective daytime sleepiness is considered a significant risk factor of hypertension in patients with obstructive sleep apnea (OSA). In this study, our goal was to examine the joint effect on hypertension of OSA and objective daytime sleepiness measured by the multiple sleep latency test (MSLT). A total of 1338 Chinese patients with OSA and 484 primary snorers were included in the study. All subjects underwent 1 night polysomnography followed by MSLT. The MSLT values were classified into 3 categories: >8 minutes, 5 to 8 minutes, and <5 minutes. Hypertension was defined based either on direct

Relationship between C-reactive protein levels and obstructive sleep apnea syndrome.

This study aims to determine the relationship between C-reactive protein levels and obstructive sleep apnea syndrome (OSAS). We recruited 30 OSAS patients into the observation group (OSAS group), and subdivided them into mild, moderate and severe groups according to the apnea hypopnea index. In addition, 20 normal individuals were included in the control group. Plasma CRP levels of two groups were measured. As compared with the control group, the CRP levels in the OSAS group were significantly increased (P < 0.05). ANOVA showed that CRP levels in the three subgroups differ; statistically signi

Predicting Obstructive Sleep Apnea with Periodic Snoring Sound Recorded at Home.

STUDY OBJECTIVES: The cost-effectiveness of diagnosing obstructive sleep apnea (OSA) could be improved by using a preliminary screening method among subjects with no suspicion of other sleep disorders. We aimed to evaluate the diagnostic value of periodic snoring sound recorded at home. METHODS: We included 211 subjects, aged 18-83 (130 men), who were referred to our laboratory for suspicion of OSA, and had a technically successful overnight polygraphy, measured with the Nox T3 Sleep Monitor (Nox Medical, Iceland) with a built-in microphone. We analyzed the percentage of periodic snoring duri

Obstructive sleep apnea promotes cancer development and progression: a concise review.

BACKGROUND: Obstructive sleep apnoea-hypopnoea (OSA) is an increasingly common sleep disorder which is widely accepted to be associated with high rates of morbidity and mortality. OSA is an independent risk factor for cardiovascular diseases, cerebrovascular disease, and metabolic disease. Recently, several studies have demonstrated that patients with OSA have a higher prevalence of cancer and cancer-related mortality. The epidemiological surveys suggest that patients with OSA had a higher incidence of cancer and cancer-related mortality than patients without OSA. Animal studies indicate that

Erythrocyte sedimentation rate may help predict severity of obstructive sleep apnea.

PURPOSE: Previous studies have shown a relationship between obstructive sleep apnea (OSA) and erythrocyte sedimentation rate (ESR), a representative marker for inflammation. We aimed to elucidate the possible association between elevated ESR and OSA severity. METHODS: A total of 341 patients who visited a sleep center were retrospectively enrolled. Subjects underwent physical examination, overnight polysomnography (PSG), and blood sampling for ESR and high-sensitivity C-reactive protein (hs-CRP). We compared the ESR and hs-CRP level to OSA severity and measured their correlation with other PS

Understanding the anatomic basis for obstructive sleep apnea syndrome in adolescents.

RATIONALE: Structural risk factors for obstructive sleep apnea syndrome (OSAS) in adolescents have not been well characterized. Because many adolescents with OSAS are obese, we hypothesized that the anatomic OSAS risk factors would be more similar to those in adults than those in children. OBJECTIVES: To investigate the anatomic risk factors in adolescents with OSAS compared with obese and lean control subjects using magnetic resonance imaging (MRI). METHODS: Three groups of adolescents (age range: 12-16 yr) underwent MRI: obese individuals with OSAS (n = 49), obese control subjects (n = 38)

Functional disconnection of the right anterior insula in obstructive sleep apnea.

BACKGROUND: The right anterior insula (AIns) is an important node of the salience network and serves to switch between two major cognitive-related functional brain networks, ie, the central executive network (CEN) and the default mode network (DMN), both of which show functional deficits in obstructive sleep apnea (OSA) patients. However, the effect of OSA on functional connectivity of the right AIns remains uncertain. OBJECTIVE: To determine whether the resting-state functional connectivity (rsFC) between the right AIns and the CEN and DMN is disrupted in OSA patients, which may be associate

Detection of pediatric obstructive sleep apnea syndrome: history or anatomical findings?

OBJECTIVE: To assess how history and/or anatomical findings differ in diagnosing pediatric obstructive sleep apnea (OSA). METHODS: Children aged 2-18 years were recruited and assessed for anatomical (ie, tonsil size, adenoid size, and obesity) and historical findings (ie, symptoms) using a standard sheet. History and anatomical findings, as well as those measures significantly correlated with OSA, were identified to establish the historical, anatomical, and the combined model. OSA was diagnosed by polysomnography. The effectiveness of those models in detecting OSA was analyzed by model fit, d

Normal-tension glaucoma and obstructive sleep apnea syndrome: a prospective study.

BACKGROUND: Today, identified risk factors for normal-tension glaucoma (NTG) include abnormal ocular blood flow, abnormal blood coagulation, systemic hypotension, ischemic vascular disorders, and autoimmune diseases. However, pathogenesis of the condition remains unclear. On the other hand, there are also a few studies suggesting that the obstructive sleep apnea syndrome (OSAS) may compromise optic nerve head perfusion and cause glaucomatous optic neuropathy by creating transient hypoxemia and increasing vascular resistance. In this study, we evaluated the possible association between OSAS and

Does neck-to-waist ratio predict obstructive sleep apnea in children?

OBJECTIVES: Central adiposity and large neck circumference are associated with obstructive sleep apnea (OSA) in adults but have not been evaluated in children as predictors of OSA. Study objectives were to determine whether (1) anthropometric measures including neck-to-waist ratio are associated with OSA in older children; (2) body fat distribution, measured by neck-to-waist ratio, is predictive of OSA in overweight/obese children. METHODS: Cross-sectional study involving children 7-18 years scheduled to undergo polysomnography at a tertiary care children's hospital. OSA was defined as total
