Sleep Disorders in Cancer-A Systematic Review.

INTRODUCTION: Sleep disorders, especially insomnia, are very common in different kinds of cancers, but their prevalence and incidence are not well-known. Disturbed sleep in cancer is caused by different reasons and usually appears as a comorbid disorder to different somatic and psychiatric diagnoses, psychological disturbances and treatment methods. There can be many different predictors for sleep disturbances in these vulnerable groups, such as pre-existing sleep disorders, caused by the mental status in cancer or as side effect of the cancer treatment. METHODS: A systematic literature revie

[The tired adolescent: Evaluation and management in the family practice].

The narrative review is used as a methodological tool for the presentation of this article. A search was made in the PubMed, Elsevier, Ebsco, and Medline databases using temporal limits (publications from 2005 to 2016), in the Spanish, English, and Portuguese language, and by the type of persons analysed in the study (adolescents), along with the keywords in English: Adolescent/teen, tiredness, fatigue, sleepiness, drowsiness. A total of 33 articles of relevance were obtained for the diagnostic and therapeutic approach of the tired adolescent in Primary Care. It was found that the main cause o

Recent lifestyle change impacts sleep and circadian rhythms among the Indigenous peoples of Peninsular Malaysia.

Sleep disorders are rising globally, but their lifestyle causes remain unclear. We recorded sleep-wake patterns via actigraphy from 1039 Orang Asli adults in Peninsular Malaysia experiencing varying levels of market integration, including access to permanent electrical and housing infrastructure, use of digital technologies, and participation in wage labour. We evaluated associations between these lifestyle variables and sleep timing, continuity and quantity, while accounting for relevant demographic and environmental covariates. Access to powerline electricity was associated with delayed and

Narcolepsy and psychosis: A systematic review.

OBJECTIVE: Narcolepsy is a rare sleep disorder in which psychotic-like symptoms can present diagnostic and therapeutic challenges. We aimed to review the association between, and medical management of, narcolepsy and psychosis in children and adults. METHODS: We reviewed the full text of 100 papers from 187 identified by a PubMed search on narcolepsy plus any of these keywords: psychosis, schizophrenia, delusion, side effects, safety, and bipolar disorder. RESULTS: Three relevant groups are described. (i) In typical narcolepsy, psychotic-like symptoms include predominantly visual hallucinati

Relationship among chrononutrition, sleep, and glycemic control in women with gestational diabetes mellitus: a randomized controlled trial.

BACKGROUND: Gestational diabetes mellitus is associated with an increased risk of maternal, fetal, and neonatal morbidities. Chronobiological disorders have recently been identified as risk factors for those morbidities. The disorders include chrononutritional disorders related to meal frequency and content according to the sleep-wake cycle, sleep disorders related to sleep quality, and chrono-obesity disorders, such as abnormal weight gain because of sleep deprivation and time of eating. OBJECTIVE: This study aimed to assess whether a chrononutritional and sleep hygiene intervention can impr

Pharmacologic Management of Excessive Daytime Sleepiness.

Excessive daytime sleepiness (EDS) is related to medical and social problems, including mental disorders, physical diseases, poor quality of life, and so forth. According to the International Classification of Sleep Disorders, Third Edition, diseases that result from EDS are narcolepsy type 1, narcolepsy type 2, idiopathic hypersomnia, hypersomnia due to a medical disorder, and others. EDS is usually treated using amphetamine-like central nervous system stimulants or modafinil and its R-enantiomer, armodafinil, wake-promoting compounds unrelated to amphetamines; a variety of new drugs are unde

Selective targets for arousal-modifying drugs: implications for the treatment of sleep disorders.

The level of arousal reflects the interaction between wakefulness-promoting and sleep-promoting nuclei located in the hypothalamus and brainstem. The nuclei and their connections constitute the sleep-arousal network. Mapping out this network, together with the neurotransmitters involved, has created a unique opportunity for the design of drugs for sleep disorders-it has become possible to target specific sites within the network with predictable effects on the level of arousal. Recent examples of this approach are orexin receptor and 5HT2A serotonin receptor antagonists and melatonin receptor

Suvorexant: a novel therapy for the treatment of insomnia.

Suvorexant is a pharmacologically novel dual antagonist of orexin receptors OX1R and OX2R, which has an effect that promotes sleep by reducing arousal and wakefulness. Its approval for the treatment of insomnia was based on three clinical trials that found it to be efficacious and relatively well tolerated. Somnolence, headache, and dry mouth are the most common side effects. Because suvorexant has unique effects on arousal systems and rapid eye movement (REM) sleep, it is contraindicated in patients with narcolepsy, and its use should be avoided or closely monitored in patients at risk for RE

The relation between cigarette smoking and sleep disturbance.

BACKGROUND: There is little epidemiological or clinical information on the relation between smoking and sleep disturbance, despite evidence suggestive of a relationship. The present study tested the hypothesis that cigarette smoking is associated with sleep disturbance. METHODS: Survey data from 3,516 adults were collected as part of a longitudinal, epidemiologic study of sleep-disordered breathing. Symptoms of insomnia, hypersomnia, and parasomnia were assessed using diagnostic criteria from the Diagnostic and Statistical Manual of Mental Disorders (3rd ed., revised). RESULTS: Among both ma

Rheumatic manifestations of sleep disorders.

PURPOSE OF REVIEW: We have long assumed that rheumatic pain causes sleep problems, fatigue, and functional disability. This paper reviews the accumulating evidence from human and animal experimental research studies that show a bidirectional relationship of disordered sleep to pain and fatigue. RECENT FINDINGS: The studies demonstrate that both disturbances of sleep and sleep restriction result in increased sensitivity to noxious stimuli and musculoskeletal pain symptoms. The notion of central nervous system hypersensitivity affecting widespread pain in patients with fibromyalgia syndrome is

Nocturnal dipping of heart rate is impaired in children with Down syndrome and sleep disordered breathing.

BACKGROUND: Children with Down syndrome (DS) are at increased risk for sleep disordered breathing (SDB), which can have adverse effects on the cardiovascular system. In adults with SDB, nocturnal dipping of heart rate (HR) and blood pressure (BP) is reduced, and this is associated with an increased risk of future cardiovascular events. We aimed to compare nocturnal dipping of HR and pulse transit time (PTT) (a surrogate inverse measure of BP change) in children with DS and SDB to those of typically developing (TD) children with and without SDB. METHODS: 19 children with DS (3-18 years) were a

Sleep-induced periodic breathing and apnea: a theoretical study.

To elucidate the mechanisms that lead to sleep-disordered breathing, we have developed a mathematical model that allows for dynamic interactions among the chemical control of respiration, changes in sleep-waking state, and changes in upper airway patency. The increase in steady-state arterial PCO2 accompanying sleep is shown to be inversely related to the ventilatory response to CO2. Chemical control of respiration becomes less stable during the light stage of sleep, despite a reduction in chemoresponsiveness, due to a concomitant increase in "plant gain" (i.e., responsiveness of blood gases t

[Insomnia in the elderly: cognitive involvement and therapeutic attitudes].

INTRODUCTION: Sleep is an important biological function, which greatly affects the quality of life at all ages. Sleep influences the alertness and stress levels, psychosomatic diseases and health in general. DEVELOPMENT: The predisposition to insomnia increases with age, illnesses such as heart disease, breathing disorders, cancer, strokes, pain and polyuria often disturb sleep patterns. In addition, the aging process, which directly alters sleep-wake cycles and other cerebral functions, affects the elderly. All the above points can lead to an increase in sleep latency, less total sleep time,

Recent advances in the treatment and management of excessive daytime sleepiness.

Excessive daytime sleepiness (EDS) is a prevalent complaint among patients in psychiatric care. Patients with conditions of EDS have often been misdiagnosed with depression due to their complaints of lack of energy, poor concentration, memory disturbance, and a reduced interest in life. Impaired alertness associated with EDS can be detrimental to a person's quality of life by causing decreased work performance, self-consciousness, low self esteem, and social isolation. Excessive sleepiness is also associated with various health problems, comorbid medical and psychiatric conditions, and fatal a

Paroxysmal nocturnal dystonia presenting as excessive daytime somnolence.

We describe the clinical and electrophysiologic findings in seven patients referred for evaluation of excessive daytime somnolence. These patients had none of the usual causes of excessive daytime somnolence but during sleep exhibited stereotypic body movements, tachycardia, respiratory disturbances, somniloquy, and transient arousals in a repetitive fashion. These episodes induced fragmentation of sleep. The polysomnograms revealed an increase in wakefulness and stage I decreased rapid eye movement during sleep in addition to the episodes of abnormal body movements. No epileptiform features w

Heart rate variability analysis in comorbid insomnia and sleep apnea (COMISA).

Obstructive sleep apnea (OSA) and insomnia are the two most prevalent sleep disorders, often co-occurring in a condition termed comorbid insomnia and sleep apnea (COMISA). While autonomic nervous system (ANS) dysfunction resulting from each of these disorders has been separately established through heart rate variability (HRV) analysis, the specific overnight ANS alterations due to COMISA have not been explored. This study aims to characterize nocturnal ANS alterations attributable to COMISA through time and frequency HRV analysis, distinguishing them from those of isolated insomnia or OSA. A

Pathophysiology and pharmacological management for sleep problems in Parkinson's disease.

Sleep problems are among the most frequently reported non-motor symptoms of Parkinson's disease (PD), with a broad range of disorders: insomnia, REM sleep behavior disorder, restless legs syndrome, excessive daytime sleepiness, and sleep-related breathing disorders. These disturbances evolve in complexity across PD severity stages, significantly impact the patients' quality of life and may exacerbate motor and other non-motor symptoms. Neurodegenerative processes, impaired function of neurotransmitters, medication side effects, circadian rhythm dysfunction are among the most proposed mechanism
