Dissociative disorders among alcohol-dependent inpatients.

The aim of this study was to determine the prevalence of dissociative disorders among inpatients with alcohol dependency. The Dissociative Experiences Scale was used to screen 111 alcohol-dependent patients consecutively admitted to the inpatient unit of a dependency treatment center. Subgroups of 29 patients who scored 30.0 or above and 25 patients who scored below 10.0 were then evaluated with the Dissociative Disorders Interview Schedule and the Structured Interview for DSM-IV Dissociative Disorders. The interviewers were blind to the Dissociative Experiences Scale scores. Of the 54 patient

Psychological determinants of fibromyalgia and related syndromes.

Fibromyalgia and other chronic pain and fatigue syndromes constitute an increasingly greater societal burden that currently is not being approached effectively by traditional Western medicine. Although the hallmarks of fibromyalgia--chronic widespread pain, fatigue, and multiple other somatic symptoms--have neurophysiologic and endocrinologic underpinnings, these biological aspects derive primarily from psychological variables. Female gender, adverse experiences during childhood, psychological vulnerability to stress, and a stressful, often frightening environment and culture are important ant

Dizziness and panic disorder: a review of the association between vestibular dysfunction and anxiety.

Dizziness is a common and costly condition that causes significant distress and impairment yet often confounds appropriate diagnosis and treatment. Among patients presenting for evaluation and treatment of dizziness, rates of panic disorder are elevated to 5 to 15 times the general population rates. In addition, the limited studies to date of dizziness in patients with panic disorder suggest that panic patients frequently experience significant dizziness and often demonstrate evidence of vestibular dysfunction. In this paper we review studies investigating the relationship between panic disord

[Psychological classification of functional voice disorders].

In an explorative study the classification of a collective of patients with different voice disorders by discriminant and cluster analysis was tried. 21 variables, obtained from 128 patients with various diagnoses of voice disorders, were used. A first discriminant analysis on the basis of diagnoses-groups permitted no differentiation. A subsequent hierarchical cluster analysis indicated a four-cluster-solution. The clusters showed only little association with the phoniatric diagnoses. Cluster 1 is characterized by patients with non-organic voice disorders. Cluster 2 is marked by emotional uns

Somatization in frequent attenders of general practice.

The DSM-III-(R) definition of somatization disorder is too restrictive for use in general practice. A more comprehensive definition, the "somatic symptom index" (SSI) has shown good validity in open populations. However, a definition has to differentiate validly within a population of frequent attenders to be a useful diagnostic instrument in general practice. We studied a threshold of five complaints (nearly identical to the SSI) in 80 Dutch general practice patients. Patients were selected on age (20-44 years), history of back, neck or abdominal complaints, and on frequency of consultation-

Briquet's syndrome in association with depression and panic: a reconceptualization of Briquet's syndrome.

In a sample of 188 consecutive female psychiatric patients, Briquet's syndrome was found to be associated more closely with the aggregation of both major depression and either panic disorder or agoraphobia than with major depression or panic disorder-agoraphobia alone. These results are consistent with the hypothesis of a shared diathesis underlying some cases of major depression, panic disorder, agoraphobia, and Briquet's syndrome. The author proposes that Briquet's syndrome may represent the most extreme expression of a tendency for a number of physical and psychological syndromes to aggrega

Somatic symptoms and related disorders in a large cohort of people with epilepsy: A cohort study.

OBJECTIVE: This study was undertaken to characterize somatic symptoms and related disorders (SSD) in epilepsy. METHODS: Adults with epilepsy under active follow-up at a tertiary epilepsy center were consecutively enrolled. The diagnosis of SSD was performed by an experienced psychologist based on the structured clinical interview for Statistical Manual of Mental Disorders, 5th edition. Detailed social/demographic data, epilepsy features, psychiatric features, life quality, disability, and economic burden were collected and compared between people with SSD and those without. Stigma perceived by patients with functional somatic syndromes and its effect on health outcomes - A systematic review.

BACKGROUND: Patients with functional somatic syndromes (FSS) experience stigma which arguably affects their health. AIM: To determine the presence of perceived stigma and its effects on physical and mental health in patients with FSS compared to patients with comparable explained conditions. METHODS: A comprehensive search of PubMed, Embase, PsycINFO, CINAHL and Cochrane Library was performed to select studies focusing on stigma perceived by patients with irritable bowel syndrome (IBS), fibromyalgia (FM) or chronic fatigue syndrome (CFS), comparing these patients to patients with comparable

Frequency-specific alterations of the frontal-cerebellar circuit in first-episode, drug-naive somatization disorder.

BACKGROUND: With the development of imaging techniques, evidence of abnormal neural activity has been implicated in patients with somatization disorder (SD). It remains unclear whether abnormal spontaneous neural activities are related to specific frequency bands. In this study, resting-state functional magnetic resonance imaging (fMRI) using the frequency-specific amplitude of low frequency fluctuation (ALFF) approach was applied to investigate changes in spontaneous neural activity in different frequency bands in patients with SD. METHODS: Twenty-five first-episode, medication-naive patient

Genesis and dissemination of a controversial disease: Chronic Lyme.

OBJECTIVES: To describe the role of the internet and cognitive biases in the controversy surrounding chronic Lyme disease. The attribution of chronic but aspecific symptoms to Lyme borreliosis is a source of worldwide controversy. PATIENTS AND METHODS: Some patients attribute their aspecific symptoms to Lyme borreliosis even though, in fact, they have a functional somatic syndrome. RESULTS: These patients' diagnostic and therapeutic wandering contributes to the above attribution and to their suffering. The Internet has deregulated the information market. Cognitive confirmation bias contribut

Somatic symptoms in children who have a parent with cancer: A systematic review.

BACKGROUND: This systematic review explored the occurrence and types of somatic symptoms in children (0-20 years) who have experienced parental cancer. METHODS: We complemented a systematic literature search from PubMed and PsycInfo with a reference search. We identified 1,694 articles, which were independently screened by two authors; they further evaluated potentially relevant papers for quality and extracted the data. We found nine relevant studies (10 publications) on altogether 672 children with a parent who had cancer; four studies included a control group. RESULTS: The studies typical

The Specificity of Health-Related Autobiographical Memories in Patients With Somatic Symptom Disorder.

OBJECTIVE: Patients with somatic symptom disorder (SSD) have persistent distressing somatic symptoms that are associated with excessive thoughts, feelings, and behaviors. Reduced autobiographical memory specificity (rAMS) is related to a range of emotional disorders and is considered a vulnerability factor for an unfavorable course of pathology. The present study investigated whether the specificity of health-related autobiographical memories is reduced in patients with SSD with medically unexplained dyspnea complaints, compared with healthy controls. METHODS: Female patients with SSD (n = 30

Anxiety, depression, and pain in burning mouth syndrome: first chicken or egg?

BACKGROUND: Burning mouth syndrome (BMS) is an idiopathic and chronic pain condition for which patients may experience high levels of pain, anxiety, and depression. So far, it has not yet been well investigated whether specific psychiatric features (anxious traits, personality disorder, or somatization) may play a role in the BMS pathogenesis or whether some BMS symptoms, or BMS itself, may cause secondary psychiatric symptoms. OBJECTIVE: The aim of this study was to evaluate the relationship between pain, depression, and anxiety in BMS and healthy patients in order to hypothesize a possible

Serum cholesterol concentration and distress in the initial days after childbirth.

The link between mood disorders and cholesterol is characterized by some contradictory data. In particular, it is not clear whether health factors are responsible for lowered cholesterol levels and mood swings. The present study tests the association between serum cholesterol level and psychological distress in women in two different post-delivery hospital settings: rooming-in (RI) and no rooming-in (no-RI). On day 3 after childbirth, 147 RI and 209 no-RI women completed the Kellner Symptom Questionnaire (SQ), which evaluates anxiety, depression, somatic symptoms and hostility. Plasma choleste

Symptom overlap and comorbidity of irritable bowel syndrome with other conditions.

Irritable bowel syndrome (IBS) is one of several highly prevalent, multi-symptom gastrointestinal motility disorders that have a wide clinical spectrum and are associated with symptoms of gastrointestinal dysmotility and visceral hypersensitivity. Symptom overlap and comorbidity between IBS and other gastrointestinal motility disorders (eg, chronic constipation, functional dyspepsia, gastroesophageal reflux disease), with gastrointestinal disorders that are not related to motility (eg, celiac disease, lactose intolerance), and with somatic conditions (eg, fibromyalgia, chronic fatigue syndrome

Somatization disorder and the chronic candidiasis syndrome: a possible overlap.

The close similarity between the complex of symptoms found in the recently described Chronic Candidiasis Syndrome and the Somatization Disorder is pointed out. Some evidence in support of the validity of the Chronic Candidiasis Syndrome is presented including some studies from the past and more recent medical literature regarding symptoms, physiological effects, and immunological findings accompanying Candida infections. Logical possibilities for the relationship of each disorder with the other is discussed. It is concluded that there is a possible overlap of the two disorders and that some pa

Birth order of patients with DSM-III-R somatization disorder.

A sample of 143 patients, 76 meeting strict DSM-III-R criteria for somatization disorder and 67 somatizers, was studied using Slater's index to measure birth order position. The results showed that birth order position was not associated with the diagnosis of somatization disorder and that sibship position in both groups occurred by chance. Thus, although environmental factors may influence the development of somatization disorder, the expression of environmental influence through birth order is unlikely.
