Long-term Outcomes Following Endoscopic Transnasal Surgery for Optic Neuropathy Due to Craniofacial Fibrous Dysplasia.

OBJECTIVE: To investigate the feasibility of endoscopic transnasal optic canal decompression (ETOCD) guided by a navigation surgical system (NSS) for vision recovery in patients with compressive optic neuropathy (CON) caused by craniofacial fibrous dysplasia (CFD), and to explore the underlying cause of visual impairment. METHODS: All patients underwent unilateral NSS-guided ETOCD and were followed up periodically for at least six months. Paired sample t-test and Pearson correlation analyses were used to compare continuous variables of the visual outcomes at the final review. A histopathologi

Surgical Management of a Superior Orbital Fissure Cavernous Hemangioma.

BACKGROUND: Cavernous sinus hemangiomas (CSHs) are intracranial extradural benign, well-demarcated neoplasms arising within dural sinuses. The orbital apex and superior orbital fissure (SOF) are common locations for these lesions. Because of the complex anatomy of this area and potential morbidity, SOF CSH surgical management is challenging. OBJECTIVE: Describe a case of a SOF CSH and review of literature. MATERIAL AND METHODS: We present the case of a 44-year-old female with a 2-month history of right eye visual disturbances. A contrast-enhanced magnetic resonance imaging showed a right orb

Outcomes of surgical resection of sphenoid-orbital meningiomas with Sonopet ultrasonic aspirator.

Purpose: Surgical resection is the mainstay of treatment for spheno-orbital meningiomas. The Sonopet® is an ultrasonic aspirator device that provides several advantages over the traditional standard suction techniques and bone drill, including decreased collateral soft tissue damage, more precise bone removal and a clearer operative field. The purpose of the study was to examine the treatment outcomes of Sonopet®-assisted resection of spheno-orbital meningiomas. Methods: A retrospective chart review was conducted in seven patients with spheno-orbital meningioma in a single institution who under

Acute Monocular Blindness Due to Orbital Compartment Syndrome Following Pterional Craniotomy.

BACKGROUND: We present a case of orbital compartment syndrome (OCS) leading to monocular irreversible blindness following a pterional craniotomy for clipping of an anterior communicating artery aneurysm. OCS is an uncommon but vision-threatening entity requiring urgent decompression to reduce the risk of permanent visual loss. Iatrogenic orbital roof defects are a common finding following pterional craniotomies. However, complications related to these defects are rarely reported. CASE DESCRIPTION: A 65-year-old female who underwent an anterior communicating artery clipping via a pterional app

A practical 3D printed simulator for endoscopic endonasal transsphenoidal surgery to improve basic operational skills.

PURPOSE: We aimed to present a practical three-dimensional (3D) printed simulator to comprehensively and effectively accelerate the learning curve of endoscopic endonasal transsphenoidal surgery (EETS). METHODS: The 3D printed simulator consists of three parts: (1) skull frame, (2) the nasal passage and the nasal alar of the face, and (3) a modified sella turcica. We aimed to improve three basic operational skills of surgeons: drilling, curetting, and aspirating. Eighteen neurosurgeons and five post-graduates were recruited and consented for the training. RESULTS: For trainees, (1) as the tr

Plagiocephaly due to Frontosphenoidal Suture Synostosis: Report of 2 Cases and Literature Review.

BACKGROUND/AIMS: The most common cause of anterior plagiocephaly is the fusion of the unilateral coronal suture. In some rare cases, however, the fusion of the frontosphenoidal suture may lead to a resembling pattern with specific clinical and radiological features. The aim of the present study is to further enlighten this entity as it is misdiagnosed most of the time. METHODS: A report of 2 cases of plagiocephaly due to frontosphenoidal synostosis with a review of the literature. RESULTS: One female and one male baby were identified. The mean age at presentation was 11.5 months (range: 1-22

A handmade eye movement monitor using a piezoelectric device during transsphenoidal surgery.

We describe a handmade eye movement monitor featuring a piezoelectric device for use during transsphenoidal surgery (TSS). The sensor consists of a piezoelectric device, condensers, resistors, and several inexpensive parts. Eyeball movements elicited by surgical manipulations during TSS for cavernous sinus tumor are detected by small disc-shaped sensors taped to the eyelids. The responses could be monitored as sharp waves on an electroencephalograph. After we started using this monitor, both the incidence and the severity of cranial nerve injuries during TSS for cavernous sinus tumor decreased

What is the better minimally invasive surgery in pituitary surgery: endoscopic endonasal transsphenoidal approach or keyhole supraorbital approach?

Endoscopic endonasal transsphenoidal approach (EETA) is an acceptable procedure as truly minimally invasive neurosurgery in handling pituitary tumor. EETA can serve many patients in many aspects especially the hospital stay and the scarification. However EETA still has some limitations that can cause serious complications. These complications such as cerebrospinal fluid leakage and bleeding control are less likely to occur ifneurosurgeons use conventional approach named as Pterional approach which was described by Yarsargil. To gain the benefit of both pterional approach and minimally invasive

A non-midline spheno-orbital encephalocele in a newborn.

Basal encephaloceles in western countries occur in 1 of every 35 000-40 000 live births; with an incidence of less than 10% they are the least common of all encephaloceles. Certain subtypes such as transsphenoidal variants may be as rare as 1 in 700 000 live births. These rare encephaloceles are classified into five anatomic types: spheno-ethmodial, transsphenoidal, spheno-orbital, transethmoidal, and spheno-maxillary. Here we present an exceedingly rare variant of a non-midline basal encephalocele of the spheno-orbital type, which was treated by resection of the encephalocele, which contained

Surgical management of primary and secondary tumors in the pterygopalatine fossa.

OBJECTIVE: This article describes a surgical approach that has been applied to managing primary tumors in the pterygopalatine fossa and secondary tumors involving the pterygopalatine fossa. PATIENTS AND METHODS: The Barbosa approach was modified by adding a lateral incision in the mandibular gingivobuccal fold from the canine tooth to the retromolar area. This technique was applied to nine patients with primary and secondary tumors in the pterygopalatine fossa. All patients had been observed by clinical examinations, MRI, and CT examinations. RESULTS: This technique allowed a large, inferior

Complications of trans-sphenoidal surgery: the Wellington experience.

BACKGROUND: All patients who underwent trans-sphenoidal surgery between January 1984 and December 1998 were reviewed to assess morbidity resulting from this operation. METHODS: There were 185 operations on 165 patients. The operative approach was sublabial in 80 cases and transnasal in 105. One surgeon (VB) performed the vast majority of operations. RESULTS: Complications included nasal perforation (7.6%), transient diabetes insipidus (4.9%), permanent diabetes insipidus (3.8%), cerebrospinal fluid fistula (4.3%), donor site haematoma (2.2%) and residual tumour haemorrhage (1.6%) causing oph

Persistent hypophyseal (craniopharyngeal) canal.

Two cases with a broad persistent hypophyseal canal connecting pituitary fossa and nasopharynx are presented. Both had nasopharyngeal surgery in early childhood and presented later with hypopituitarism. Coronal CT demonstrated the defects with no visible pituitary tissue. The spectrum of basal skull defects is discussed. Children with midline nasal polyps should have CT or MRI of the skull base prior to surgery to prevent inadvertent hypophysectomy.

Use of the superior meatus and superior turbinate in the endoscopic approach to the sphenoid sinus.

Endoscopic surgery of the posterior ethmoid and sphenoid sinuses can present the operator with a considerable challenge. Although endoscopic training and physician experience have increased significantly in the past decade, surgical complications such as optic nerve trauma and cerebrospinal fluid leak still occur. Surgeons reporting such complications cite a lack of orientation within the dissection field as a primary cause. Because endoscopic sinus surgery is now being performed more routinely, surgical strategies designed to reduce the risk of complications are more important than ever. An a

[A comparative study of the pterygopalatine fossa by computed tomography and magnetic resonance].

The pterygopalatine fossa (PPF) is a narrow, bone lined space containing many major cranial nerves and vessels connecting it to the middle and infratemporal cranial fossa, the oral, nasal and orbital cavities and the rhinopharynx. The internal maxillary artery, the second branch of the V cranial nerve and the sphenopalatine ganglion are the main structures which are found in the PPF. Because of its strategic site, the PPF can be involved in many pathologic conditions originating from the nasal fossa, the orbit, the paranasal sinuses or the rhinopharynx roof. Severe craniofacial trauma can invo
