MR imaging has evolved as the primary modality for imaging spinal neoplasms; however, neoplasms must be distinguished from unusual appearances of degenerative lesions. This article reviews MR imaging findings of a variety of neoplastic lesions of the lumbar spine. For example, extradural, metastatic tumors are the most common; nerve sheath tumors are the most common intradural extramedullary tumors; and ependymoma accounts for most of the tumors arising from the filum terminale.

OBJECTIVE: To determine the incidence of back pain in children admitted with sickle cell disease so as to increase awareness of this disease … MEASURES: Clinical manifestations of spinal involvement by sickle cell disease.
MAIN RESULTS: The spine represented the second most common area of bone involvement (26%) exceeded only by the knee (35%). The vertebral level affected was lumbosacral in 66% of cases, followed by thoracic in 22% and cervical in only 12%. Eighty-six percent of the children …

Significant cervical spine injury is very unlikely in a case of trauma if the patient has normal mental status (including no drug or alcohol use) and no neck pain, no tenderness on neck palpation, no neurologic signs or symptoms referable to the neck (such as numbness or weakness in the extremities), no other distracting injury and no history of loss of consciousness. Views required to radiographically exclude a cervical spine fracture include a posteroanterior view, a lateral view and an odontoid view. The lateral view must include all seven cervical vertebrae as well as the C7-T1 interspace,…

MR imaging is the imaging modality of choice for evaluation of the traumatized lumbar spine, providing critical information for determination of appropriate therapy. It is superior to other modalities for evaluating the supporting ligaments, disc, spinal cord, and the cauda equina. Major fracture patterns are readily discernable. Canal compromise caused by osseous fragments, epidural hemorrhage, or disc fragments is well visualized.

OBJECTIVE: Experimental investigations analyzing the biomechanics of the cervical spine are less common than similar studies of other regions of the spine. There are no reports on cervical intradiscal pressure (PID) measurements in vitro. We therefore wanted to establish normal values for PID under physiological conditions by simultaneous muscle force simulation. Moreover, the impact of ventral cervical fusion should be elucidated, because in clinical studies, it is a well-known phenomenon that the adjacent segments often show increased degenerative changes. We present a pilot study.

STUDY DESIGN: This study introduces a useful plating technique for complex reconstructions of the anterior cervical spine.
OBJECTIVE: To provide a short-segment-buttressing technique for the stabilization of long anterior fusion constructs in the cervical spine while avoiding the potential morbidity and risks associated with long-segment anterior cervical plating.
SUMMARY OF BACKGROUND DATA: Anterior fibular or iliac crest strut grafts are at risk of dislodging when used after multisegment (> 3 vertebrae) corpectomy in various spinal disorders. Long-segment anterior cervical plates have been…

The evaluation of children with cervical spine disorders requires an understanding of the anatomic and developmental features that are particular to the pediatric spine. In this article, cervical spine developmental anatomy is briefly reviewed, along with common radiographic features of the pediatric cervical spine. The epidemiology, clinical presentation, and management of congenital cervical anomalies are considered. The evaluation and management of pediatric cervical trauma are also reviewed. Other disorders with common cervical spine involvement, such as skeletal dysplasias, connective tis…

The neutral position of the spine is the posture most commonly sustained throughout daily activities. Previous investigations of the spine focused mainly on maximal exertions in various symmetric and asymmetric postures. This report proposes a new synergetic approach for analysis of the spine in neutral postures and evaluates its performance. The model consists of passive components, the osteoligamentous spine, and active components, the spinal muscles. The muscle architecture includes 60 muscles inserting onto both the rib cage and lumbar vertebral bodies. The passive spine is simulated by a …

STUDY DESIGN: Electrical and mechanical stimulation of the lumbar supraspinous ligament of three patients with L4-L5 spinal deficits and of the feline model, respectively, was applied while recording electromyography on the multifidus muscles.
OBJECTIVES: To determine if mechanoreceptors in the human spine can reflexively recruit muscle force to stabilize the lumbar spine, and to demonstrate, in the feline model, that such ligamento-muscular synergy is elicited by mechanical deformation of the lumbar supraspinous ligament (and possibly of other spinal ligaments), the facet joint capsule, and …

Degenerative changes of the spinal column have long been and continue to be confused with the presence of spinal distress and pain. All parts of the spine undergo degenerative changes as we age. The purpose of this chapter is to describe the degenerative process and its clinical consequences. The disc degenerative process will be discussed; its consequences on the facet joint and osteophyte formation are considered. The prevalence of disc degeneration, the role of physically demanding work and leisure and the interference of spinal deformity is clarified. A section particularly important for t…

Headache related to the cervical spine is often misdiagnosed and treated inadequately because of confusing and varying terminology. Primary headaches such as tension-type headache and migraine are incorrectly categorized as "cervicogenic" merely because of their occipital localization. Cervicogenic headache as described by Sjaastad presents as a unilateral headache of fluctuating intensity increased by movement of the head and typically radiates from occipital to frontal regions. Definition, pathophysiology; differential diagnoses and therapy of cervicogenic headache are demonstrated. Ipsilate…

Unstable fractures of the upper thoracic spine are a therapeutic challenge because of the high rate of associated injuries and frequent occurrence of spinal cord lesions. This study focuses on the operative treatment of these injuries. We present nine patients with unstable injuries of the upper thoracic spine. According to the classification of Magerl et al., there are seven type C and two type B fractures. Eight patients had complete transverse lesions of the cord and one had no evidence of spinal cord injury. All patients presented associated injuries: nine patients had fractured ribs, thre…

Twenty patients with a diagnosis of Saethre-Chotzen syndrome had their cervical spine radiographs reviewed. Radiologic abnormalities including vertebral fusion were present in 9 of the 20 patients. Fusion of both the vertebral bodies and the posterior elements was noted, although the latter site was more common. C2-3 was the level most commonly involved, although other levels were recorded. Analysis of sequential radiographs in nine patients revealed evidence of progression in seven patients. In those studies in children aged under 2 years, only 1 of 18 films showed evidence of fusion, while i…

The deformity in severe spondylolisthesis consists of two components: the parallel anterocaudad slip of the spondylolisthetic vertebra, and its tilt into kyphotic malposition. The influence of the two components is very different: the anterocaudad slippage has not much impact on the sagittal profile of the spine and is easily compensated for by a slight increase in lumbar lordosis. The kyphotic deformity has a high impact on trunk imbalance and the sagittal profile. There are two compensation mechanisms: hyperlordosis of the lumbar spine to its anatomical extremes and - if that is not sufficie…

Fifteen patients with ankylosing spondylitis who had developed a severe flexion deformity of the cervical spine which restricted their field of vision to their feet, were treated by an extension osteotomy at the C7/T1 level. The operation was performed under general anaesthesia with the patient in the prone position and wearing a halo-jacket. Three had internal fixation using a Luque rectangle and wiring. Their mean age was 48 years. Before operation the mean cervical kyphosis was 23 degrees; this was corrected to a mean of 31 degrees of lordosis, a mean correction of 54 degrees. All the patie…

Recently, magnetic resonance (MR) imaging has become the preferred method of evaluation for spinal disorders. The vertebrae, intervertebral disks, ligaments, spinal canal, and neural foramen may all be evaluated using current MR imaging techniques. MR imaging with paramagnetic contrast has developed into a valuable technique for diagnosing a tumor, an infection, or a degenerative disease. Computed tomography remains the procedure of choice for examining fine cortical bone detail including evaluation of spine fractures and assessing neural foraminal size, but it is not sensitive for detecting m…

A prospective study was performed in obese and nonobese patients undergoing lumbar spine surgery to report perioperative data and surgical outcomes. One hundred fifty-nine consecutive patients who underwent lumbar spine surgery by a single surgeon entered the study. Among 159 consecutive patients, 55 met the criteria for obesity (> 20% ideal body weight). The average weight was 226 lb in the obese group. There were 28 men and 27 women with ages ranging from 25 to 81 years, with a mean of 47.9 years. The diagnoses and procedures included 31% herniated nucleus pulposus with laminotomy and discec…
