[Volar locking plate fixation of unstable distal radius fractures].

OBJECTIVES: We evaluated radiographic and functional results of volar locking plate fixation of unstable distal radius fractures. METHODS: The study included 27 patients (15 males, 12 females; mean age 45 years; range 18 to 77 years) who were treated with volar locking plate fixation for unstable distal radius fractures. Twenty-two patients (81.5%) had AO type C and five (18.5%) had type B fractures. Two patients (7.4%) had type I open fractures. Dorsal and volar angulation were present in 21 (77.8%) and six (22.2%) fractures, respectively. Four patients (14.8%) also had elbow dislocation and

Complications and results of long adult deformity fusions down to l4, l5, and the sacrum.

STUDY DESIGN: This is a consecutive study of patients having undergone surgical treatment of adult lumbar scoliosis. Follow-up ranged from 2 to 13 years (average 5 years). OBJECTIVES: To assess the complications and outcomes of patients with long fusions to L4 (n=23), L5 (n=21), or the sacrum (n=15) and determine if a "deeply seated" L5 segment is protective. SUMMARY OF BACKGROUND DATA: Few studies assess outcomes and complications in adults fused from the thoracic spine to L4, L5, or the sacrum with minimum 2-year follow-up. METHODS: Fifty-eight patients (59 cases; average age 43 years; ra

Acute slipped capital femoral epiphysis.

Of 12 cases of acute slipping of the capital femoral epiphysis (representing 11 patients), results were satisfactory in nine cases and poor in three cases at follow-up (in two of the three cases with poor results, reduction was lost postoperatively). Five of nine patients with prodromal symptoms were seen by a physician but were undiagnosed or untreated before the acute slipping. An additional patient had roentgenographic evidence of an early (asymptomatic) slipping capital femoral epiphysis one year before the acute slipping. Preferred treatment is gentle manipulation and fixation with multip

Closed Intramedullary Pinning of Displaced Radial Neck Fracture (Metaizeau Technique).

BACKGROUND: Radial neck fractures account for 1% of all pediatric fractures and 5% to 10% of pediatric elbow fractures. The mechanism of injury is typically a fall with the elbow in hyperextension and the forearm in supination. A valgus force compresses the radial head against the capitellum, causing a radial neck fracture. Displaced radial neck fractures are difficult to treat and account for a disproportionate number of bad outcomes, including malunion, nonunion, and osteonecrosis. The preferred treatment is closed reduction and fixation, as open reduction is associated with an inordinately

A modified technique for esophagojejunostomy or esophagogastrostomy after laparoscopic gastrectomy.

OBJECTIVE: Reconstruction of the digestive tract involving esophageal anastomosis after laparoscopic gastrectomy is a surgically difficult procedure. In this study, a newly developed transoral pretilted circular anvil, a "the oral to the abdomen" method, was proven to be effective. METHOD: A total of 34 consecutive patients underwent esophageal anastomosis using the OrVil in our hospital from July 2009 to February 2011. The esophagus was transected and a small hole was then made in the esophageal stump through which the nasogastric tube of the OrVil was passed to insert the anvil into the abd

Integration of the Zeiss QEVO® Inspection Tool in a Commercial Retractor System for Bimanual Angled View Tumor Resections: Technical Note With Case Illustration.

BACKGROUND AND OBJECTIVES: The QEVO® microinspection tool integrated into the ZEISS KINEVO® platform provides high-quality angled visualization and is increasingly used to inspect microscopic blind spots during neurosurgical tumor resections. However, effective integration of the QEVO® tool into the standard neurosurgical bimanual workflow remains challenging in delicate environments, as no commercially available device can securely position the tool to enable undisturbed bimanual dissection under angulated endoscopic vision. This report describes a practical and reproducible technique for sta

Comparison Between the Bone Lid Technique and the Traditional Technique in Surgical Treatment of the Posterior Mandibular Lesions: A Randomized Controlled Trial.

Introduction Cystic lesions of the jaws and the impacted teeth are two of the most common cases that require surgical intervention in oral and maxillofacial surgery; however, surgeons also frequently use a traditional technique that involves the removal of the buccal bone plate. This study was conducted to compare the clinical and radiologic outcomes of the bone lid technique and the traditional technique. Methods This randomized controlled trial included 20 patients who were randomly divided into two groups (n = 10 each): the T group, in which the lesions were accessed using the traditional t

Endoscopic-assisted ACDF for C2-C3 disc herniation : a case report of successful decompression.

The C2-C3 segment presents unique surgical challenges due to its anatomical complexity. Traditional anterior cervical discectomy and fusion (ACDF) under microscopy often yields suboptimal visualization. We report the first successful application of full endoscopic-assisted ACDF for C2-C3 disc herniation with spinal cord compression. A 46-year-old male with refractory neck pain (VAS 8/10) underwent preoperative MRI/CT confirming severe C2-C3 herniation. Endoscopic-assisted ACDF via anterior approach achieved complete discectomy, and PEEK cage placement with fixation. Endoscopic visualization en

Injury to the anterior tibial system during percutaneous plating of a proximal tibial fracture.

Minimally invasive osteosynthesis of proximal tibial fractures has grown in popularity in recent years. This article describes a patient with a Schatzker type VI proximal tibial fracture (AO/OTA type 41.C3) and previous compartment syndrome treated with definitive fixation 8 weeks after initial injury with a precontoured proximal tibial plate and a distal targeting device. Brisk bleeding occurred during percutaneous insertion of a cortical screw at the midshaft of the tibia. Surgical exploration revealed sidewall tearing of the anterior tibial artery and vein, which were clipped at the screw i

Internal Distraction Resulted in Improved Patient-Reported Outcomes for Midface Hypoplasia.

BACKGROUND: Both internal and external distraction devices have been used successfully in correcting midface hypoplasia. Although the indication for surgery and the osteotomy techniques may be similar, deciding when to use internal versus external devices has not been studied. The authors studied patient-reported outcomes with FACE-Q and functional surveys for internal and external devices for midface distraction patients. METHODS: Patients who underwent distraction advancement after Le Fort I and Le Fort III were surveyed using the FACE-Q survey and a functional survey. Equal groups of inter

Stress distribution on the thorax after the Nuss procedure for pectus excavatum results in different patterns between adult and child patients.

OBJECTIVE: In the Nuss procedure, in which the deformed thorax is forcibly corrected by insertion of correction bars, considerable stresses occur on the patient's thorax. We performed the present study to elucidate how stress patterns on the thorax after this procedure differ between child and adult patients. METHODS: Eighteen patients with pectus excavatum, constituting a child group (n = 10) and an adult group (n = 8), were included in the study. After a 3-dimensional computer-assisted design model was produced with computed tomographic data from each patient, simulation of the Nuss procedu
