Clinical and pathologic studies of twenty-six patients with penetrating foreign body injury to the joints, bursae, and tendon sheaths.

Foreign body synovitis has been neglected in the rheumatology literature. We describe 26 patients in whom arthritis, bursitis, or tenosynovitis appeared within 1 day to 7 years after an initial injury by a penetrating foreign body. Twenty-two patients presented with acute synovitis, which was followed by chronic or recurrent inflammation mimicking septic arthritis, osteomyelitis, monarticular juvenile rheumatoid arthritis, bone tumor, or apatite deposition disease. Foreign bodies were not seen in 5 inflammatory synovial fluids studied, but were seen in the synovium or periarticular tissues of …

Complications Following Endoscopic and Open Trigger Finger Release: A Retrospective Comparative Study.

BACKGROUND: Open trigger finger release (OTFR) and endoscopic trigger finger release (ETFR) are effective methods in treating stenosing tenosynovitis. However, a paucity of literature exists comparing the techniques. This study describes and compares postoperative complications following OTFR and ETFR at a single institution. METHODS: Patients undergoing trigger finger release between 2018 and 2020 within a single institution were identified. Electronic medical records were reviewed for patient demographics, surgical history, surgical characteristics, and clinical outcomes. Major and minor po…

MRI and ultrasound of the hands and wrists in rheumatoid arthritis. I. Imaging findings.

The management of patients with rheumatoid arthritis (RA) has rapidly evolved with the development of newer disease-modifying drugs and the recognition that long-term damage can be mitigated by an earlier and more-informed use of these medications. Historically, radiographs were the mainstay of imaging in RA patients, but radiographic joint narrowing and erosions are late and insensitive findings in the disease. MRI (with intravenous contrast agent) and ultrasound (with power Doppler interrogation) of the hands and wrists are able to demonstrate erosions earlier and with greater sensitivity th

[Rheumatoid arthritis and hand surgery].

Rheumatoid arthritis results in characteristic deformities of the hand. Medical treatment has undergone a remarkable development. However, not all patients achieve remission or tolerate the treatment. Patients who suffer from deformities and persistent synovitis may be candidates for hand surgery, for which the main goals are pain relief and improved function. Surgical interventions can be divided into prophylactic and therapeutic procedures. The treatment strategy is individual and depends on close collaboration between rheumatologists, hand surgeons and patients.

Clinical Sign for Missed Decompression of a Separate Extensor Pollicis Brevis Compartment in de Quervain's Disease.

Persistent pain despite previous surgery for de Quervain's disease might be due to an overlooked septum between the abductor pollicis longus tendon slips and the extensor pollicis brevis tendon, or an overlooked completely separate compartment for the extensor pollicis brevis tendon alone. In both of these instances, extension of the MP joint of the thumb against resistance elicits pain at the distal level of the first extensor compartment of the wrist. When this sign is positive, revisional surgery and decompression of the remaining septum or separate compartment is indicated.

Right Ring Finger Volar Mass in a 14-Year-Old Boy.

A trigger digit is relatively uncommon in adolescents and often has a different etiology in that age group vs adults. In the pediatric popul … ons has been described, which is what occurred in the current case. This report describes a case of tenosynovitis with psammomatous calcification treated with excision of the mass from the flexor digitorum superficialis tendon and release of both the A1 and palmar aponeurosis pulleys in an adolescent patient. [Orthopedics. 2017; 40(5):e918-e920.].

Ultrasound and magnetic resonance imaging in the evaluation of psoriatic dactylitis: status and perspectives.

OBJECTIVE: Dactylitis, a characteristic feature of the spondyloarthropathies, occurs in up to 48% of patients with psoriatic arthritis (PsA) … dies were assessed. RESULTS: The most commonly described features of dactylitis were flexor tendon tenosynovitis and joint synovitis (90%). Extratendinous soft tissue thickening and extensor tendonitis were described nearly equally as being present and absent. Discrepancy exists as to whether entheses proper contribute to the etiology of dactylitis. An increas …

Histological appearances in post-partum de Quervain's disease.

Six patients who developed de Quervain's disease during pregnancy or within 12 months of childbirth were treated surgically. Histopathological examination of the tendon sheaths using alcian blue stain showed that myxoid degeneration was responsible for the thickening observed in the sheath. Characteristic intramural deposits of mucopolysaccharides were present, predominantly in the subsynovial region. Acute or chronic inflammatory changes were not seen. These findings were the same as those described in patients with de Quervain's disease unrelated to pregnancy or childbirth.

Percutaneous A1 pulley release: a cadaveric study.

Percutaneous A1 pulley release was performed on 17 fresh-frozen cadaveric hands with a 14-gauge angiocath needle. Each hand was then explored to assess the adequacy of release and the degree of injury to adjacent structures. Complete release of the A1 pulley was obtained in 45 of the 66 fingers and in 10 of the 17 thumbs. Significant injury to the flexor tendons was observed in two digits. All tendon injuries occurred along the line of the fibers. There were no digital nerve injuries. The release was within 2 mm of a thumb digital nerve in seven hands and the little finger ulnar digital nerve

Results of surgery for trigger finger.

Forty-three patients were reviewed between 17 and 113 months (mean of 47 months) following surgery for acquired stenosing tenosynovitis of the fingers or thumbs. 32 (60.4%) of the 53 operations were completely successful. Of the remaining 21 operations, 26% either failed to relieve all symptoms, or symptoms had recurred at review. 15 operations led to complications that bothered the patient to some extent. Three procedures resulted in significant functional deficit of the hand, two because of nerve damage and one because of stiffness following infection. The three major complications all follo…

Bowstringing as a complication of trigger finger release.

Bowstringing is a rare complication of trigger finger release caused by excessive loss of the proximal pulleys. As the flexor tendon moves away from the center of rotation of the metacarpophalangeal joint, the flexion moment arm is increased. Thus the flexors gain an increased mechanical advantage over the extensors resulting in limited digital extension. We describe a case of severe bowstringing progressing over 9 years resulting in pain, altered digital function, and loss of full extension.

[Carpal tunnel syndrome in tuberculous tendosynovitis of the hand].

A case of tuberculous tendosynovitis of the hand is reported. Since this is a very rare disease nowadays, which if untreated results in caseoid necrosis and destruction of the flexor tendon apparatus, we thought it interesting to draw attention to this observation. The unspecific symptoms and diagnostic signs are discussed. Therapy consists in an early combination of surgery and tuberculostatic therapy. Without treatment, caseoid necrosis and destruction of the flexor tendons ensue.

Trigger thumb in adults after hyperextension injury.

Three cases of trigger thumb in adults developing after hyperextension injury are described. The similarity of the mode of injury in the cases and its possible role in the production of initial stenosis in the tendon sheath is discussed. Trauma alone may not cause triggering in the fingers, for there are other important factors such as rheumatoid disease, which with trauma may play a part. Trauma may however be the sole initiating factor in other cases of adult trigger thumbs.

Pathogenesis of some traumatic and degenerative disorders of soft tissue.

1. Trauma to soft tissues is associated with tissue rupture and formation of a haematoma. 2. Such lesions cannot be expected to resolve without complications in the absence of appropriate treatment. 3. Myositis ossificans should be anticipated in lesions of muscles adjacent to their attachments. 4. Chronic tendonitis and enthesopathies not associated with inflammatory connective tissue disease are primarily degenerative in nature and are probably the result of comparative ischaemia. Mesenchymoid transformation is a feature of both.
