Root reshaping: an integral component of periodontal surgery.

It is the aim of this article to present a surgical option to the traditional method of returning lost biologic width where invasion of the junctional and/or connective tissue attachment associated with a tooth has occurred. The alternative to conventional osseous resection involves reshaping the existing tooth surface in combination with conservative removal of the supporting alveolar bone to create the width needed for the restoration to be biologically acceptable. This procedure accomplishes several goals: (1) minimum supporting bone is removed; (2) deleterious root surface anatomy, such as

Caries Direction Antimicrobial Activity of Zinc Oxide Nanoparticles Containing Cement for the Management of Orthodontic Teeth against Streptococcus mutans and Lactobacillus casei.

Streptococcus mutans and Lactobacillus casei are the two bacterial species that cause tooth decay and affect orthodontically treated teeth. In orthodontic treatment, zinc oxide (ZnO) is incorporated into the cement, providing antibacterial properties. The objective of the study was to evaluate the antimicrobial activity of zinc oxide nanoparticles containing cement for the management of orthodontic teeth. ZnO nanoparticles were prepared using ZnCl2 and then incorporated into the Orthodontic Portland Cement (OPC) with a grade of 43. It was evaluated for physicochemical parameters like colour, o

Scalable Fabrication of Polymeric Composite Microspheres to Inhibit Oral Pathogens and Promote Osteogenic Differentiation of Periodontal Membrane Stem Cells.

Periodontitis is a worldwide bacterial infectious disease, resulting in the resorption of tooth-supporting structures. Biodegradable polymeric microspheres are emerging as an appealing local therapy candidate for periodontal defect regeneration but suffer from tedious procedures and low yields. Herein, we developed a facile yet scalable approach to prepare polylactide composite microspheres with outstanding drug-loading capability. It was realized by blending equimolar polylactide enantiomers at the temperature between the melting point of homocrystallites and stereocomplex (sc) crystallites,

Management of bisphosphonate preparation-treated children in the field of pediatric dentistry.

Since most of the reports of BRONJ onset are adults, in order to clarify the current situation of BRONJ onset in children, it is necessary to search for articles and report on the current status and actual conditions of surgical treatment of children with BP preparations who are being followed up in our clinic. In previous reports both inside and outside Japan, there was no mention of jaw bone necrosis during tooth extraction or surgery in children who were receiving or had a history of BP administration. There were 15 children with a history of BP administration who manage the oral cavity in

Stepwise removal compared to complete removal for deep carious lesions.

INTRODUCTION: Carious lesions have traditionally been treated by non-selective (complete or total) removal of carious tissue. However, due to its risks and adverse effects, carious tissue removal techniques have been developed, based on the preservation of tooth tissue, including the stepwise removal. The objective of this summary is to clarify the uncertainty about the effectiveness and safety of the stepwise removal technique compared to complete removal. METHODS: To answer this question we used Epistemonikos, the largest database of systematic reviews in health, which is maintained by sear

Dominant Charcot-Marie-Tooth syndrome and cognate disorders.

Charcot-Marie-Tooth neuropathy (CMT) is a group of genetically heterogeneous disorders sharing a similar phenotype, characterized by wasting and weakness mainly involving the distal muscles of lower and upper limbs, variably associated with distal sensory loss and skeletal deformities. This chapter deals with dominantly transmitted CMT and related disorders, namely hereditary neuropathy with liability to pressure palsies (HNPP) and hereditary neuralgic amyotrophy (HNA). During the last 20 years, several genes have been uncovered associated with CMT and our understanding of the underlying molec

Mineral trioxide aggregate apical plug method for the treatment of nonvital immature permanent maxillary incisors: Three case reports.

Treatment of nonvital immature permanent teeth with calcium-hydroxide is associated with some difficulties such as weakened tooth fracture, root canal reinfection and long treatment time. Mineral trioxide aggregate (MTA) apical plug method is an alternative treatment option for open apices, and has gained popularity in the recent times. In this case report, we have attempted to present successful treatment of three maxillary incisors with open apices and periapical lesions with MTA. After preparing the access cavity, the working length was determined. The root canals were irrigated with 2.5% S

Film and digital radiography for detection of simulated root resorption cavities.

OBJECTIVES: The purpose of this study was to evaluate observers' ability to detect simulated root resorption cavities in conventional radiographs and digital radiographs from a charge-coupled device system and a storage phosphor system and to determine whether the detectability was influenced by resorption size and exposure. STUDY DESIGN: In teeth from dry mandibular jaw specimens, resorptive defects 1.2 mm in diameter and two different depths (0.6 and 0.9 mm) were prepared in the buccal root surface. Each tooth was radiographed at a number of different exposure settings. Observers were asked

Micro-PIXE (proton-induced X-ray emission) study of the effects of fluoride on mineral distribution patterns in enamel and dentin in the developing hamster tooth germ.

Micro-PIXE (proton-induced X-ray emission) analysis was performed on unfixed and anhydrously prepared sections from developing enamel and dentin from hamsters injected with a single dose of 20 mg NaF/kg body weight. Fluoride, apart from inducing the formation of the characteristic paired response in the enamel (i.e., a hyper- followed by a hypomineralized band in the secretory enamel), also induces the formation of sub-ameloblastic cystic lesions under the transitional and early secretory enamel accompanied by relatively intense hypermineralization of the underlying cystic enamel surface. Thes

Scanning electron microscopic characterization of sensitive vs. insensitive human radicular dentin.

Our aim was to investigate the structural changes of dentinal tubules in specimens obtained from both hypersensitive and naturally desensitized areas in wedge-shaped defects on the same exposed cervical dentin surface of a hypersensitive tooth. A new biopsy technique that makes use of a hollow, cylindrical diamond bur was designed so that specimens from exposed root dentin of vital teeth could be obtained. Twenty-two dentin biopsy pairs were divided into two groups; one was prepared for scanning electron microscopy (SEM) and the other for microradiography (MR). Small hypersensitive areas were

Influence of Access Cavities on Maxillary Central Incisor Fracture Resistance: Finite Element Study.

INTRODUCTION AND AIMS: Altering the position and orientation of the root canal access cavity passway, or modifying the reduction of dentin volume, can influence the strength of dentition. This study aimed to compare the effects of different access cavities on the biomechanical performances of maxillary central incisors with a finite element analysis. METHODS: Based on the micro-computed tomography (CT) scan of a maxillary central incisor, the finite element models of the intact tooth and teeth with 4 access cavity designs: conservative incisal access cavity, incisal access cavity, conservativ

Maxillary molars with morphologic variations of the palatal root canals: a report of four cases.

INTRODUCTION: The purpose of this article was to show the importance of the knowledge of the anatomy of root canals. Unusual root and root canal morphologies associated with both buccal roots of upper molars have been recorded in several studies in the literature. However, scientific information focusing on variations of the palatal root is rare. METHODS: In this report, four cases are presented involving the root canal treatment of maxillary first and second molars with unusual morphologic configurations of the palatal root canals. RESULTS: During root canal treatment, type IV and V configu

Root canal treatment in mandibular canines with two roots: a report of two cases.

AIM: To highlight anatomical variation in the mandibular canine. SUMMARY: Two clinical case reports are presented to illustrate anatomical variation in the human mandibular canine. Endodontic treatment may sometimes fail because morphological features of the tooth adversely affect the treatment procedures. Many investigators have reported the anatomical variations associated with mandibular canines. Mandibular canines are recognized as usually having one root and one root canal in most cases, although approximately 15% may have two canals or sometimes two roots. This paper describes two clini

The double-barrier technique using platelet-rich fibrin for closure of oroantral fistulas.

An oroantral fistula (OAF) or oroantral communication (OAC) is an opening between the oral cavity and the maxillary sinus. If left untreated, these openings may cause chronic maxillary sinusitis. Although small defects (diameter <5 mm) may close spontaneously, larger communications require surgical intervention. Various studies have been conducted on OAC closure using a platelet-rich fibrin (PRF) membrane; most of these prior studies have involved simple direct application of PRF clots. This study introduces a new "double-barrier technique" using PRF for closure of an OAF involving sinus mucos

Computer-guided implant placement in contact with root remnants in the esthetic area.

The extraction of a tooth with root resorption can be a complicated procedure and can result in the removal of a significant amount of alveolar bone. In the anterior maxilla, esthetic rehabilitation of damaged sites with implant-supported crown could require multiple surgeries. Patients often ask for an alternative, and minimally invasive treatments are generally preferred. In this paper, a clinical case of implant-supported crown in a site affected by root resorption is described. Immediate implant was placed leaving some root fragments in the osteotomy site, thus avoiding invasive extractive

Influence of alveolar bone level on the pull-out bond strength of fiber-reinforced composite posts to root dentin.

This in vitro study evaluated the influence of alveolar bone level and type of cement on pull-out bond strength between different fiber-reinforced composite (FRC) posts and root dentin. Sixty bovine teeth were sectioned, and their root canals were prepared. The specimens were divided into 6 groups (n = 10) according to 2 factors: alveolar bone level and type of cement. Each root was embedded in acrylic resin to the depth of 7, 10, or 14 mm, simulating different alveolar bone levels. After the FRC posts were treated with 37% phosphoric acid and silane was applied, they were cemented with either

Canine-lateral incisor transposition: Controlling root resorption with a bone-anchored T-loop retraction.

A 12-year-old girl presented with a Class II Division 1 malocclusion, complicated by a complete transposition of the maxillary left canine into the position normally occupied by the left lateral incisor. Dental and medical histories were noncontributory. Brackets were bonded on all maxillary teeth, from first molar to first molar, except for the left lateral incisor. Because the lateral incisor was not engaged on the archwire, the tooth was free to physiologically move out of the path of canine root movement. To prepare the site for canine retraction, a coil spring was used to open space betwe
