"Always, always have hope:" persons with lived experience, carers and health professionals' experiences before and after discharge from hospital for an eating disorder.

BACKGROUND: Eating disorders can result in inpatient admissions. The transition of care between this tier of service and outpatient support can be challenging, particularly without intensive outpatient options. The potential benefits of providing intensive outpatient options during this transition period are significant, however there is limited research in this area. This study aimed to understand the experiences and needs of persons with lived experience, carers and health professionals before and after this transition in care for adults in Australia. METHODS: Using mixed quantitative and q

Childhood to adult transition in youth patients with lysosomal acid lipase deficiency: 43 recommendations from experts.

BACKGROUND: The process of transition from pediatric to adult care is crucial, especially in rare diseases such as lysosomal acid lipase deficiency (LAL-D). Unfortunately, this process is associated with poor outcomes, and many challenges still await to be addressed. This document provides recommendations on the pediatric to adult care transition in patients with LAL-D, based on available evidence and the experience of a panel of experts, which include specialists in the management of patients with LAL-D, and representative patients of the AELALD patient organization. Additionally, the main un

Perceived transition readiness among adolescents and young adults with neurofibromatosis type 1 and plexiform neurofibromas: a cross-sectional descriptive study.

OBJECTIVES: Neurofibromatosis type 1 (NF1) is a genetic cancer predisposition syndrome that can impact multiple organ systems and is associated with plexiform neurofibroma tumors, requiring care from birth through adulthood. Adolescents and young adults (AYAs) with NF1 face several barriers to transition from pediatric to adult care. This cross-sectional study aimed to assess transition readiness in this population and to evaluate relationships between specific NF1 symptoms and transition readiness. METHODS: AYAs (aged 16-24) enrolled in existing studies related to NF1 were eligible. AYAs and

Long-Term Adherence to a Gluten-Free Diet and Quality of Life of Celiac Patients After Transition to an Adult Referral Center.

BACKGROUND: Modalities for the transition to adult care of celiac patients diagnosed during childhood/adolescence and their impact on long-term adherence to a gluten-free diet (GFD-A), quality of life (QOL) and maintenance of follow-up in adulthood are unknown. AIMS: To evaluate whether timing of transition affects long-term GFD-A, QOL, and continuity of follow-up in adulthood and to identify predictors of long-term GFD-A. METHODS: Clinical and demographic data about pediatric care and adult follow-up at our center were retrospectively collected from clinical notes of celiac patients diagnos

Electroconvulsive Therapy Considerations for Transgendered Patients.

As the transgender patient population continues to grow, health care providers will need to become aware of elements unique to the transgender community in order to provide the highest quality of care. Neuromuscular blockade with succinylcholine is routinely administered to patients undergoing electroconvulsive therapy (ECT). Decreased amounts or activity of pseudocholinesterase in serum can lead to prolonged duration of muscle paralysis. Causes of reduced action by pseudocholinesterase include genetically abnormal enzymes, reduced hepatic production, pregnancy, and various drug interactions.

[Reflections on the clinical diagnosis and management of Turner syndrome].

There is a delay in the clinical diagnosis of Turner syndrome (TS), particularly for patients with mosaic karyotypes. This diagnostic delay can hinder age-appropriate treatments and lead to adverse health outcomes. Therefore, it is necessary to explore improvement measures for early diagnosis and treatment plans based on the current clinical situation. Furthermore, as research progresses, the focus of clinical diagnosis and treatment of TS is gradually expanding to multiple aspects, including cardiovascular health, fertility, and transitional care, in order to improve the prognosis and quality…

Case Report of Myroides odoratimimus Cellulitis in Chronic Venous Stasis Dermatitis With Literature Review.

Myroides spp.-induced cutaneous infections are rare, with only 17 reported cases in the literature. Myroides spp. behave like low-grade opportunistic pathogens, with symptomatic infections observed typically in severely immunocompromised patients and seldom in immunocompetent patients. In this paper, we present an immunocompetent 61-year old male with a past medical history of hypertension, hyperlipidemia, morbid obesity, and patient-reported peripheral neuropathy who presented to the transitional care clinic with bilateral lower extremity swelling and hemosiderin-pigmented dry wounds consiste…

Evaluation and Long-term Management of Neurogenic Bladder in Spinal Dysraphism.

Spinal dysraphism, which includes conditions such as myelomeningocele and sacral agenesis, is one of the most common causes of congenital lo … nd therapeutic modalities. The result is a real, and unfortunately often unmet, need for successful transitional care in this complex patient population. Clinicians must be able to identify the unique challenges encountered by patients with neurogenic bladder as they shift through different stages of their life.

Low cardiac output secondary to a malpositioned umbilical venous catheter: value of targeted neonatal echocardiography.

Systemic hypotension is common in very low birthweight preterm infants but the nature of the precipitating cause may be unclear. Targeted neonatal echocardiography (TnEcho) is being increasingly used to support hemodynamic decisions in the neonatal intensive care unit (NICU), including identifying impairments in the transitional circulation of preterm infants, providing timely re-evaluation after institution of therapies and evaluating the placement of indwelling catheters. We present a case of a preterm infant with systemic hypotension and low cardiac output secondary to a large transatrial s

"It's more than just being in a place, it's more like a peace of mind": Ontological security among residents of a transitional housing program implemented during the COVID-19 pandemic.

BACKGROUND: Homelessness is a visible and persistent manifestation of social and structural inequity in the United States. When individuals experience homelessness, their ontological security, which refers to the sense of constancy, safety, and trust in daily life, is often deeply eroded. We explored the connections between embodied experiences and ontological security among residents of a transitional housing program implemented during the COVID-19 pandemic. METHODS: We conducted in-depth interviews between November 2022 and October 2023 with 24 former residents of Operation Safer Ground (OS

Real-world management and outcomes of myocardial infarction in a transitional country: insights from an 11-year observational study of 13,099 patients.

BACKGROUND AND AIM: Acute myocardial infarction (AMI) remains a leading cause of death globally, despite substantial advances in prevention, diagnosis and reperfusion therapy. In transitional health systems, in-hospital mortality is higher due to limited reperfusion capacity and infrastructure limitations. Kosovo introduced primary percutaneous coronary intervention (PPCI) in 2014, achieving nationwide 24/7 coverage in 2018. We evaluated 11-year trends in AMI management in Kosovo and identified independent predictors of in-hospital mortality. METHODS: We analyzed 13,099 consecutive patients h
