Personal experience on the PPD-test in 154 patients with chronic liver disease is reported. The results show a frequent hyperergic response. The comparison with a similar group of patients previously studied in the same Clinic employing the old tubercolin shows the advantages of PPD in discriminating the differential degrees of reactivitty. The hyperergic response is important not only from a theoretical point of view, confirming the presence of an abnormal immunologic equilibrium in chronic liver diseases, but it may also be of therapeutic relevance particularly in the cases where the result

QuantiFERON-TB Gold (QFT) is an indirect diagnostic test for latent tuberculosis (TB) infection and active TB. This study aimed to evaluate and compare QFT and the tuberculin skin test (TST) for the diagnosis of active TB disease, not bacteriologically verified at the time of therapy initiation. Seventy-three patients with suspected active TB were examined. Further diagnostic steps confirmed the diagnosis of active TB in 53 persons. QFT was positive in 43 persons (81%), negative in seven (13%), and indeterminate results were found in three (6%). The sensitivity of the test was 86% in those wit

There was no response to tuberculin test, which is the best marker of delayed type hypersensitivity (DTH) reactions in 24% of haemophilics. In conclusion, although there was no significant change in the ratio of CD4/CD8 and lymphocyte subgroups, specific antibody responses and DTH tests we …

Isolation of mycobacterium tuberculosis is the gold standard in the diagnosis of childhood tuberculosis. However, it has inherent limitations due to paucibacillary nature of the disease in children and technical difficulties encountered in collection of appropriate sample. Thus, diagnosis is dependent on circumstantial evidence at best supported by conventional tests such as tuberculin test and chest radiograph. Several new tests are being developed but they lack ideal sensitivity and specificity. Hence, it is important to optimise use of current diagnostic tests. Clinical suspicion based on p…

Mycobacterium orygis (M. orygis), a member of the Mycobacterium tuberculosis complex, has emerged as an important cause of animal and human tuberculosis in South Asia. Despite the widespread prevalence of bovine tuberculosis (bTB) in India and the major role of over 100 million Indian water buffaloes in dairy production, the pathological and epidemiological features of M. orygis infection in buffaloes are largely unexplored. A closed herd of 279 female water buffaloes in Haryana, India, was screened for bTB by single intradermal cervical tuberculin skin (SCT) test, the comparative cervical tub

Tuberculosis vaccines (Bacillus Calmette-Guérin, BCG) were introduced 100 years ago and are still recommended by the World Health Organizati … up of 695 health care workers aged 25 years and over in Poland. All participants in the study had a tuberculin test, after which those who were negative were randomised (1:1) and received either the BCG- or placebo vaccine. From then on, these people were subjected to three months of observation for the occurrence of COVID-19 symptoms. The statistical analysis di …

BACKGROUND: The ability of Mycobacterium tuberculosis to survive intracellularly, provides a cellular adaptive immune response played by specific T cells to defend against tuberculosis. The adaptive immune response to Bacillus of Calmette and Guerin (BCG) immunization is responded to by B cells, T Follicular B helper, T regulatory, restriction CD1, CD8+, CD4+, Th1, Th2, and Th17. BCG immunization can cause a tuberculin test reaction to being positive. The tuberculin test is a method for diagnosing TB infection and for screening individuals for latent infection and assessing the rate of TB infe…

BACKGROUND: Healthcare workers in Tuberculosis (TB) and non-TB units in hospitals have a high risk of experiencing Latent Tuberculosis Infection (LTBI), because of exposure to droplets containing Mycobacterium tuberculosis. This study aims to prove LTBI incidence and risk factors to healthcare workers at the hospital in Jember City.
MATERIAL AND METHODS: a cross-sectional study, from January to March 2020 in two hospitals in Jember City. Healthcare workers in the TB care and non-TB care unit were examined using Tuberculin skin test (TST) with a cut off ≥ 10 mm for positive LTBI. Chest x-ray a

Failure to react to coccidioidin (anergy) often occurs in patients with disseminated coccidioidomycosis. One possible reason may be desensitization by excessive amounts of antigen. This was studied experimentally by injection of soluble and hyphal antigens of Coccidioides immitis into coccidioidin- and tuberculin-sensitive guinea pigs. Guinea pigs sensitized by injection of killed hyphal cells of C. immitis in complete Freund adjuvant were subsequently injected daily either with soluble coccidioidal antigen administered intraperitoneally or with hyphal antigen administered either subcutaneousl

We systematically retested children with borderline-positive quantiferon (0.35-0.99 IU/mL) during the period 2015-2019. Among 647 tests, 27 (4%) were positive (>0.35 IU/mL). Borderline-positive quantiferon accounted for 10 of 27 (37%) positive tests. When retested, 9 of 10 (90%) were negative. Children younger than 5 years had negative tuberculin skin tests. Thus, we found high test variability in the borderline range and, in some children, high suspicion of false positive tests.

Following BCG vaccination, local or systemic complications may occur. A 19-year-old female patient with negative tuberculin skin test developed a blister evolving over two weeks into an ulcer in a vaccination site on the left hip. Ulcerations following BCG vaccination are rare and show a tendency to chronicity. The treatment recommendations are controversial and range from non-specific topical treatment to systemic tuberculostatic therapy. In this case, the ulcer cleared rapidly after topical treatment with INH powder.

INTRODUCTION: The incidence of tuberculosis is increasing, particularly in the elderly, and has various clinical presentations.
EXEGESIS: We describe the case of a 78-year-old woman who presented tuberculous pleuropericarditis. This case is atypical, due to infection localisation, negativity of the tuberculin skin test, and mixed pleural effusion. Following antituberculosis antibiotic therapy and corticotherapy, the outcome was favorable.
CONCLUSION: Due to atypical and non-specific clinical presentation, diagnosis of tuberculosis may be particularly difficult in the elderly.

The incidence of genital tuberculosis has increased for the last decade 2-fold reaching 11.9% of all the extrapulmonary cases. Three clinical forms of tuberculosis of the uterine appendages are distinguished: latent or minor inflammation, marked inflammation with tubuloovarian lesions, tuberculomas. The diagnosis of the disease, especially of latent forms, is not easy and requires studying the disease history, the results of subcutaneous 50 TU tuberculin test, histerosalpingography, endoscopic (surgical laparoscopy, histeroscopy, colposcopy), histological and microbiological findings.

Despite major diagnostic and therapeutic advances, postoperative peritonitis appeared to be still associated with a severe prognosis. The failure to react to delayed hypersensitivity skin tests was recently shown to identify patients at increased risk for sepsis. In an attempt to clarify the mechanisms of this anergy, cellular and humoral immunity was studied with in vitro tests in 12 patients treated for postoperative peritonitis. Complement was decreased in 33.3% of cases and normal in the others. No significant change was found in IgG and IgM titres, but IgA concentrations were increased in

An 8-year-old girl presented with a 34-day history of cough, fatigue, and impaired exercise tolerance. She experienced cyanosis on exertion but denied fever, hemoptysis, hematuria, or seizures. Her perinatal and family histories were unremarkable, and she had no history of exposure to TB. A chest radiogram from a local clinic showed diffuse pulmonary lesions. Tuberculin skin test, interferon-γ release assay, and HIV antibody test results were all negative. Immunoglobulin levels and lymphocyte subsets were normal. The patient did not respond to IV azithromycin for 1 week for community-acquired
