Controversial reports have been published about the influence of sperm source and of the underlying testicular pathology on success rates of intracytoplasmic sperm injection (ICSI). For a long period of time, azoospermia has been considered a condition with poor therapeutical possibilities. Even in successful cases the number of pregnancies has been very low. With the new techniques in artificial insemination and fertilization there is new interest in treating azoospermic patients. A complete diagnostic procedure should be performed. The use of bilateral scrotal exploration and testicular biopsy allow better chances of diagnosis and treatment. Several WB4101 (1)-related compounds were synthesized. Their alpha-adrenoreceptor blocking properties were evaluated in order to understand the kind of interaction occurring between the amine function of (1)-related compounds and the alpha-adrenoreceptor anionic site. The results suggest that a charge reinforced hydrogen bond rather than an ion pairing might play a most important role in receptor binding.

In an attempt to further improve the alpha-adrenoreceptor blocking activity of benextramine, the effect of heteroaromatic substituents on the terminal nitrogens was studied. In this communication we report on the newly synthetized N,N''-(dithio-2,1-ethanediyl)bis[N'-(pyrrol-2-ylmethyl)-1,6- hexanediamine] (pyrextramine) that displayed higher affinity (5-10-fold) and higher selectivity (about 12-fold) than benextramine. These preliminary results show that pyrextramine can replace benextramine in the elucidation and characterization of the peripheral alpha 1-adrenoreceptors.

Proper examination of the scrotum consists of inspection, palpation and, if indicated, transillumination. Inspection may reveal the hypoplastic appearance of an undescended testicle; enlargement caused by a testicular mass; the "bag of worms" signifying a varicocele; findings typical of torsion of the spermatic cord, or the "blue dot" sign, which indicates torsion of the appendix testis or appendix epididymis. During palpation, the testis must be differentiated from the epididymis.

Retention of contrast medium in the renal pelvis for several days after urography is illustrated in 2 cases of obstructing urinary stones. In a third case water-soluble contrast medium was used for retrograde pyelography before nephrectomy and was retained in the stenotic ipsilateral ureter 7 1/2 months after the operation. A further case exemplifies retention in the seminal vesicles 2 months after vasoseminal vesiculography.

Obstructive azoospermia secondary to epididymal obstruction can be corrected by microsurgical reconstruction with vasoepididymostomy (VE).
