Left atrial volume index as a predictor for persistent left ventricular dysfunction after aortic valve surgery in patients with chronic aortic regurgitation: the role of early postoperative echocardiography.

BACKGROUND: This study aimed to explore whether echocardiographic measurements during the early postoperative period can predict persistent left ventricular systolic dysfunction (LVSD) after aortic valve surgery in patients with chronic aortic regurgitation (AR). METHODS: We prospectively recruited 54 patients (59 ± 12 years) with isolated chronic severe AR who subsequently underwent aortic valve surgery. Standard transthoracic echocardiography was performed before the operation, during the early postoperative period (≤2 weeks), and then 1 year after the surgery. RESULTS: Twelve patients wit

External validation of artificial intelligence-guided electrocardiogram (AI-ECG) in detecting asymptomatic left ventricular dysfunction: A systematic review and meta-analysis.

AIM: Asymptomatic left ventricular dysfunction (LVD) represents a key and frequently under-recognized stage in the heart failure (HF) continuum. Early identification enables targeted prevention and timely initiation of evidence-based therapies. Artificial intelligence-enabled electrocardiography (AI-ECG) may provide a scalable alternative to imaging for LVD detection. This systematic review synthesized evidence from exclusively externally validated AI-ECG studies evaluating the detection of asymptomatic LVD, with symptomatic HF cohorts included to contextualize the evidence base across the HF

Determinants of Left Ventricular Diastolic Dysfunction in Normotensive Patients With Type 2 Diabetes Mellitus: A Tertiary Hospital-Based Study in Ghana.

Background Diabetes mellitus is a growing global health challenge, with significant cardiovascular complications among affected populations. Left ventricular diastolic dysfunction (LVDD) represents an early manifestation of diabetic cardiomyopathy and predicts adverse outcomes. Data on LVDD in normotensive patients with type 2 diabetes mellitus (T2DM) in Ghana are limited. This study aimed to estimate the prevalence and identify determinants of LVDD among normotensive T2DM patients presenting to the Korle-Bu Teaching Hospital in Ghana. Methodology This was a descriptive cross-sectional study o

Comparative predictive value of three non-insulin-based insulin resistance indices for subclinical left ventricular dysfunction in type 2 diabetes mellitus.

BACKGROUND: Current research indicates that novel non-insulin-based insulin resistance indicators—including the triglyceride-glucose (TyG) index, triglyceride/high-density lipoprotein cholesterol ratio (TG/HDL-C), and Metabolic Score for Insulin Resistance (METS-IR)—are closely associated with cardiovascular disease risk in patients with type 2 diabetes mellitus (T2DM). Notably, the TyG index demonstrates significant correlation with subclinical left ventricular dysfunction (SLVD), making it a potential biomarker for early detection of SLVD in T2DM patients. However, the predictive value of TG

Prolonged ST-Segment Elevation and Persistent Left Ventricular Dysfunction Assessed by Dual-Isotope Myocardial Scintigraphy in Takotsubo Syndrome: A Case Report.

Takotsubo syndrome is characterized by transient left ventricular (LV) systolic dysfunction in the absence of obstructive coronary artery disease. ST-segment elevation is frequently observed in the acute phase and typically resolves within several days, evolving into T-wave inversion. Prolonged ST-segment elevation is uncommon, and its clinical significance remains incompletely understood. Here, we report the case of an elderly woman with takotsubo syndrome likely triggered by a urinary tract infection. The initial electrocardiogram showed marked ST-segment elevation with terminal T-wave inver

Delayed bioprosthetic valve thrombosis after transcatheter aortic valve implantation in a patient with severe left ventricular diastolic dysfunction: A case report.

A 78-year-old female developed symptomatic bioprosthetic valve thrombosis 32 months post-transapical TAVI, despite 6-month warfarin therapy. Echocardiography showed severe hemodynamic compromise with left ventricular remodeling and diastolic dysfunction. CT-confirmed thrombosis resolved after warfarin intensification and cardiac optimization, achieving mean gradient reduction to 17 mmHg within 8 weeks. This highlights the association between delayed thrombosis, ventricular remodeling, and subtherapeutic anticoagulation.

Establishment and internal-external validation of a 28-day mortality prediction model for septic shock patients with left ventricular systolic dysfunction.

BACKGROUND: Septic shock remains a major cause of death in the ICU with high 28-day mortality, and concomitant left ventricular systolic dysfunction (LVSD) further worsens patient outcomes, while existing prediction models lack adequate specificity. This study aimed to establish and perform internal-external validation of a 28-day mortality prediction model specifically for septic shock patients with LVSD as a key risk factor. METHODS: This retrospective multicenter cohort study enrolled 226 septic shock patients from September 2021 to October 2024 as the training cohort and 132 patients from

Prediction of Left Ventricular Systolic Dysfunction Using an Artificial Intelligence-Based Electrocardiogram Analysis Model in Patients Presenting to the Emergency Department.

Background: Left ventricular systolic dysfunction (LVSD) is a precursor to heart failure arising from diverse cardiac conditions. Although echocardiography remains the reference standard for LVSD diagnosis, its routine use in the emergency department (ED) may be constrained by cost, time, equipment availability, and the need for specialized expertise. We evaluated the diagnostic performance of an artificial intelligence-based electrocardiogram analysis model (AI-ECG model) for detecting LVSD in patients presenting to the ED. Methods: This retrospective observational study included patients tre

Association Between Left Ventricular Diastolic Dysfunction and Subsequent Detection of Sick Sinus Syndrome and Atrial Fibrillation in Patients With Unexplained Syncope.

BACKGROUND: Atrial fibrillation (AF) and sick sinus syndrome (SSS) frequently coexist and may share common atrial structural and electrophysiological remodeling substrates. Left ventricular (LV) diastolic dysfunction frequently coexists with AF; however, its association with subsequent detection of SSS and AF remains incompletely understood. This study investigated whether LV diastolic dysfunction is associated with the detection of SSS and AF. METHODS: This study included patients who underwent insertable cardiac monitor implantation for recurrent unexplained syncope. Patients with prior AF,

Predictors and Prognostic Impact of Perioperative Hypotension During Transcatheter Aortic Valve Implantation: The Role of Diabetes Mellitus and Left Ventricular Dysfunction.

BACKGROUND: Perioperative hypotension is a frequent but underrecognized complication during transcatheter aortic valve implantation (TAVI). Although reduced left ventricular ejection fraction (EF) and low baseline blood pressure have been linked to hemodynamic instability, the role of metabolic comorbidities and procedural factors remains less well established. METHODS: We retrospectively analyzed 123 patients who underwent transfemoral TAVI between June 2016 and June 2022. Perioperative hypotension was defined as a sustained systolic blood pressure < 90 mmHg or ≥30% reduction from baseline f

Treat-and-repair strategy for atrial septal defect with left ventricular systolic dysfunction: a case report of cardiac resynchronization therapy responder.

BACKGROUND: The treatment strategy for closing atrial septal defect (ASD) in patients with left ventricular (LV) dysfunction remains to be elucidated. Current guidelines recommend a balloon occlusion test to determine whether the ASD should be closed, fenestrated, or not. CASE SUMMARY: A 56-year-old man was referred to our hospital for secundum ASD with LV dysfunction. He was diagnosed with non-ischaemic cardiomyopathy with LV ejection fraction of 24%. A secundum ASD with a diameter of 18 mm also existed, with a pulmonary blood flow to systemic blood flow ratio over 2.0. Initially, ASD closur

Asymptomatic left ventricular dysfunction 48 years after the onset in an adult patient with three-vessel giant aneurysms caused by Kawasaki disease.

UNLABELLED: Although giant coronary aneurysms caused by Kawasaki disease (KD) often lead to symptomatic or asymptomatic occlusion within the first year of the acute KD, the long-term fate of giant aneurysms remains unknown. A 51-year-old man with giant coronary aneurysms in three major coronary arteries developed asymptomatic left ventricular dysfunction with complete left bundle branch block (CLBBB) 48 years after the onset of acute KD. He had been treated with antiplatelet therapy. The 13N-ammonia positron emission tomography findings revealed significant ischemia with a reduced left ventric

Prognostic impact of diagnosis-to-ablation time on outcomes following catheter ablation in persistent atrial fibrillation and left ventricular systolic dysfunction.

BACKGROUND: The optimal timing of catheter ablation in individuals with atrial fibrillation (AF) and left ventricular systolic dysfunction (LVSD) remains uncertain. OBJECTIVE: We examined whether AF diagnosis to ablation time (DAT) influences outcomes following catheter ablation (CA) in patients with persistent AF (PsAF) and LVSD from the CAMERA-MRI and CAPLA randomized studies. METHODS: We evaluated clinical outcomes according to DAT < 1 year ("shorter DAT") and ≥1 year ("longer DAT"), comparing AF recurrence, AF burden, left ventricular ejection fraction (LVEF), and LV recovery (LVEF ≥ 50%

Investigation of Left Ventricular Strain and Its Morphological Basis During Different Stages of Diastolic and Systolic Dysfunction in Spontaneously Hypertensive Rat.

BACKGROUND: Myocardial fibrosis plays an important role in the pathogenesis of hypertensive cardiac dysfunction, and myocardial strain could detect early systolic abnormalities when left ventricular ejection fraction (LVEF) is preserved. The aim of this study was to investigate the characteristics of left ventricular multidirectional strain during different stages of heart dysfunction and the possible morphological basis in spontaneously hypertensive rats (SHRs). METHODS: SHRs and Wistar-Kyoto (WKY) rats were randomly divided into cages and observed for 3-25 months. Echocardiographic measurem

Electrocardiogram abnormalities in chronic Chagas cardiomyopathy correlate with scar mass and left ventricular dysfunction as assessed by cardiac magnetic resonance imaging.

UNLABELLED: Electrocardiographic (ECG) abnormalities are frequently identified in Chronic Chagas cardiomyopathy (CCC) patients and advanced abnormalities are related to a worse prognosis. Cardiac Magnetic Resonance (CMR) can precisely assess ventricular systolic dysfunction and quantify myocardial fibrosis (MF), both identified as prognostic factors. We sought to investigate if ECG abnormalities in CCC patients were associated with more severe myocardial involvement as evaluated by CMR. METHODS: CCC patients with 12‑lead ECG and CMR closely obtained were included. ECG analysis evaluated rhyth
