Progressive sensorineural hearing loss and a widened vestibular aqueduct in Pendred syndrome.

Pendred syndrome is an autosomal recessive inherited disorder. Obligatory features are profound deafness in childhood and defective organic binding of iodine in the thyroid gland. Therefore, goiter is a common symptom. Hypoplasia of the cochlea is another feature. Recently, the gene for Pendred syndrome was identified. We describe a boy whose sensorineural hearing loss in both ears progressed rapidly from about 50 to 60 dB at the age of 3 years and 3 months to more than 100 dB at the age of 4 years and 4 months. This loss was preceded by a medical history of a progressive hearing loss. The pro

Vestibular functions in patients with tinnitus only.

OBJECTIVE: Tinnitus is the perception of sound in the ears or head without any external or internal acoustic stimulation, and it is usually associated with hearing loss. In addition, it has been reported that there is a relationship between vestibular problems and sensorineural hearing loss. The aim of this study was to evaluate the vestibular function in patients with tinnitus without hearing loss. METHODS: A total of 32 patients who complained only of tinnitus without hearing loss and 30 control subjects without tinnitus or hearing loss were included in this study. Oculomotor and caloric te

Semicircular canal plane head impulses detect absent function of individual semicircular canals.

We studied the human vestibulo-ocular reflex (VOR) in response to head 'impulses': brief, unpredictable, passive, high-acceleration (up to 4000 degrees/s2), low-amplitude (20-30 degrees) head rotations. We delivered the head impulses approximately in the plane of the semicircular canal (SCC) being tested. To test the anterior and posterior SCCs, the head impulses were delivered in a diagonal plane, midway between the frontal (roll) and sagittal (pitch) planes. We recorded head and eye position in three dimensions with scleral search coils in nine normal subjects, seven patients following unila

Self-Reported Dizziness Handicap is Related to Functional Capacity in People with Vestibular Hypofunction.

BACKGROUND AND PURPOSE: Individuals with vestibular hypofunction (VH) report low levels of physical activity (PA), corresponding with restrictions when performing activities of daily living. However, a clear association between physical function and self-reported dizziness handicap and measures of vestibular function has not been established. Our purpose is to investigate whether functional aerobic capacity and PA are related to dizziness handicap and objective assessments of the vestibulo-ocular reflex in participants with VH. METHODS: A total of 36 participants (14 males, 22 females; 70.11

Preoperative vestibular evoked myogenic potentials (VEMPs), caloric test, and pure tone audiometry to identify the vestibular nerve branch of schwannoma origin: preliminary results in a series of 26 cases.

Vestibular schwannoma (VS) is a benign tumor which develops in the internal auditory canal and the cerebellopontine angle, potentially diminishing hearing or balance. Most VS tumors arise from one of two vestibular branches: the superior or inferior vestibular nerve. Determining the specific nerve of origin could improve patient management in terms of preoperative counseling, treatment selection, and surgical decision-making and planning. The aim of this study was to introduce a preoperative testing protocol with high accuracy to determine the nerve branch of origin. The nerve of origin was pr

Clinical Findings in a Pati ent with Large B-Cell Lymphoma andNeuroniti s: A Case Study.

BACKGROUND: Given the multifaceted nature of the balance system, an interdisciplinary approach to patient care is imperative. For surgical patients with complex medical histories, comprehensive vestibular evaluations provide information on baseline vestibular function and identify possible surgical risks and outcomes. This case study follows a patient diagnosed with probable right vestibular neuronitis immediately preceding surgical resection of a left brainstem lesion, later diagnosed as a B cell lymphoma (BCL) and provides details regarding pre- and postsurgical outcomes. PURPOSE: This case

Consequences of vestibular hypofunction in children with ADHD/DCD.

BACKGROUND: Children with Attention Deficit Hyperactivity Disorder (ADHD) demonstrate a heterogeneous sensorimotor, emotional, and cognitive profile. Comorbid sensorimotor imbalance, anxiety, and spatial disorientation are particularly prevalent among their non-core symptoms. Studies in other populations presented these three comorbid dysfunctions in the context of vestibular hypofunction. OBJECTIVE: To test whether there is a subgroup of children with ADHD who have vestibular hypofunction presenting with concomitant imbalance, anxiety, and spatial disorientation. METHODS: Children with ADHD

Probabilities of Isolated and Co-Occurring Vestibular Disorder Symptom Clusters Identified Using the Dizziness Symptom Profile.

OBJECTIVES: Dizziness is among the most common reasons people seek medical care. There are data indicating patients with dizziness, unsteadiness, or vertigo may have multiple underlying vestibular disorders simultaneously contributing to the overall symptoms. Greater awareness of the probability that a patient will present with symptoms of co-occurring vestibular disorders has the potential to improve assessment and management, which could reduce healthcare costs and improve patient quality of life. The purpose of the current investigation was to determine the probabilities that a patient pres

Relationship between video head impulse test (vHIT) and caloric test in patients with vestibular neuritis.

INTRODUCTION AND OBJECTIVES: The caloric test is the gold standard for the loss of vestibular function diagnosis. The Video Head Impulse Test (vHIT) assesses the same reflex by using a video- assisted examination of the impulsive maneuver. We intend to compare the variation of results of the vHIT and the caloric test in patients with vestibular neuritis with respect to their initial condition at two different moments of their evolution and to check the level of correlation between them and with that of the DHI test METHODS: We explored 20 patients with neuritis by using both vHIT and the calo

Development of vestibular afferent projections into the hindbrain and their central targets.

In contrast to most other sensory systems, hardly anything is known about the neuroanatomical development of central projections of primary vestibular neurons and how their second order target neurons develop. Recent data suggest that afferent projections may develop not unlike other sensory systems, forming first the overall projection by molecular means followed by an as yet unspecified phase of activity mediated refinement. The latter aspect has not been tested critically and most molecules that guide the initial projection are unknown. The molecular and topological origin of the vestibular

Cisplatin Differentially Damages Mammalian Vestibular End Organs.

OBJECTIVES: Cisplatin is an effective anticancer drug that can induce cochleotoxicity in up to 50 to 60% of treated patients. However, the vestibulotoxic potential of cisplatin remains unclear. Our lab previously developed a multicycle model of cisplatin administration in mice that results in hearing loss similar to that observed clinically. The purpose of this study was to comprehensively characterize the functional and morphological consequences of cisplatin on the vestibular periphery in this mouse model. DESIGN: Twenty-nine adult CBA/CaJ mice were assigned to control or cisplatin-treated

Preserved vestibular evoked myogenic potentials (VEMP) in some patients with walking-induced oscillopsia due to bilateral vestibulopathy.

Bilateral vestibulopathy, i.e. decreased peripheral vestibular function affecting both ears, is characterized by unsteadiness of gait, particularly in darkness and by motion-induced oscillopsia. We have recently seen a few patients with severely impaired semicircular canal function albeit with rather normal vestibular evoked myogenic potentials (VEMP) suggesting normal saccular function. The five young patients, mean age 27 years (range 15-45), 4 males and 1 female, had severely impaired balance in darkness and they all reported walking-induced vertical oscillopsia. Hence, these patients with

Defining diagnostic thresholds for dissociation between caloric test and vHIT in Ménière's disease.

OBJECTIVE: Ménière's disease (MD) presents with episodic vertigo, hearing loss, and tinnitus; however, its diagnosis remains challenging owing to symptom overlap with other vestibular disorders. We evaluated the diagnostic value of dissociation between caloric test and video head impulse test (vHIT) results in MD compared to vestibular schwannoma (VS) and benign paroxysmal positional vertigo (BPPV). METHODS: A retrospective analysis included 195 patients with MD (n = 51), VS (n = 112), or BPPV (n = 32). Vestibular function was assessed using caloric tests and vHIT. Dissociation was defined as

Effect of methimazole, an FMO substrate and competitive inhibitor, on the neurotoxicity of 3,3'-iminodipropionitrile in male rats.

This study was designed to examine the role of flavin-containing monooxygenase (FMO) on the auditory and vestibular neurotoxicity of 3,3'-iminodipropionitrile (IDPN) using the FMO substrate and competitive inhibitor methimazole (MMI). Specifically, the purpose was to block the FMO-mediated conversion of IDPN to the putative neurotoxic metabolite N-hydroxy3,3'-iminodipropionitrile (HOIDPN). In three separate experiments, adult male Long-Evans hooded rats were administered (ip) saline (vehicle), MMI, IDPN, or HOIDPN individually, or a combination of IDPN and MMI or HOIDPN and MMI.
