[Vitamin D3 and its analogues: a new age in the treatment of psoriasis vulgaris?].

The introduction of this article is devoted to the physiological role of vitamin D3 and its synthesis. The authors refer to the accidental introduction of vitamin D3 into the psoriasis treatment, which later led to the development of a less toxic vitamin D3 analog named calcipotriol, and its administration as a topical antipsoriatic agent. Reviewing the literature the authors discuss the pharmacological and immunological effects of calcipotriol as well as the potential applications of vitamin D3.

Gangrenous dermatitis caused by Corynebacterium ulcerans in Richardson ground squirrels.

Gangrenous dermatitis caused by Corynebacterium ulcerans developed in 63 of 350 wild Richardson ground squirrels (Spermophilus richardsonii). Six squirrels died of toxemia and/or septicemia, but 57 responded to topical and parenteral administration of antibiotics. The epizo-otic was believed to be associated with fighting; infected and carrier ground squirrels most likely transmitted the C ulcerans through bite wounds. Individuals handling ground squirrels should be cautioned that C ulcerans may produce a diphtheria-like disease in human beings.

Pharmacokinetics and intraocular pressure-lowering activity of TAK-639, a novel C-type natriuretic peptide analog, in rabbit, dog, and monkey.

TAK-639 is a topical, 9-amino acid, synthetic, C-type natriuretic peptide analog in development for the treatment of primary open-angle glaucoma and ocular hypertension. This study investigated the impact of TAK-639 on intraocular pressure (IOP), the levels of TAK-639 in aqueous humor, and the pharmacokinetic/pharmacodynamic relationship of TAK-639 following topical ocular administration to normotensive female Dutch belted rabbits, beagle dogs, and cynomolgus monkeys. In the IOP studies, rabbits (n = 6/group) and dogs (n = 8/group) received a single topical ocular dose of TAK-639 0.03%, 0.1%,

[Pharmacological Glaucoma Therapy].

Pharmacological therapy remains a cornerstone in the management of glaucoma. The therapeutic spectrum has been broadened by the introduction of new active substances and drug classes, preservative-free topical formulations, fixed combinations, and innovative drug delivery systems. In addition, alternative routes of administration are being investigated, and interventional approaches are increasingly gaining importance as primary treatment options based on emerging clinical evidence. This article provides an overview of current pharmacological therapies, highlighting their clinical relevance fo

Disseminated brown macules in an infant.

Urticaria pigmentosa is the most common form of mastocytosis. Mastocytosis usually presents at birth or early childhood, and may involve only the skin or, less commonly, other internal organs. Diagnosis is clinical, but a skin biopsy may be useful. Prognosis is usually good, and treatment focuses on the avoidance of certain triggers and administration of topical and systemic medications. Appropriate counselling of parents regarding the benign nature of this disease is important as most cases resolve by adolescence.

Postvaccinal, corticosteroid-resistant bullous pemphigoid in infancy: treatment with intravenous immunoglobulin.

Bullous pemphigoid is an autoimmune subepidermal blistering disorder that typically affects elderly adults but can also occur in childhood. We report on a 3-month-old boy who developed bullous pemphigoid 1 week after the second routine administration of a hexavalent vaccine. The disease was resistant to standard therapies (including oral and topical corticosteroids) but was relieved by intravenous immunoglobulin treatment. There was no recurrence of bullous pemphigoid after the next vaccination (3 mos after discontinuation of steroids).

Treatment of advanced malignant eccrine poroma with locoregional chemotherapy.

Malignant eccrine poroma is a rare disease with approximately 200 cases reported in the literature. Regional cutaneous and systemic metastases are rarely observed and their management has been generally unsuccessful. We report on a case in which topical 5-fluorouracil application and intra-arterial chemotherapy with docetaxel resulted in a histologically confirmed complete response of multiple regional skin metastases for more than 2 years. Despite intravenous administration of docetaxel, slow progression of systemic disease was observed.

Corneal melting with intraocular lenses.

Five patients with collagen vascular disease and keratoconjunctivitis sicca underwent cataract surgery and implantation of intraocular lenses. Postoperative development of corneal melting may have been potentiated by the use of topical 0.1% dexamethasone sodium phosphate alcohol and neomycin sulfate. Permanent visual loss occurred in two patients. Implant removal was necessary in two eyes. Medical management consisted of discontinuance of administration of steroids and antibiotics, as well as the addition of tear substitutes, cycloplegics, and pressure patching.

A single-dose topical therapy cures scabies in a human-relevant porcine model.

Scabies affects ∼400 million people annually, yet current therapies lack ovicidal activity, require repeat dosing, and are increasingly compromised by treatment failure and resistance. The absence of a single-application therapy capable of eliminating both motile mites and eggs represents one important limitation to effective disease control. We developed a mechanism-informed topical therapy combining abametapir, a metalloprotease inhibitor with ovicidal activity, and flavesone, a β-triketone with rapid miticidal effects, designed to target multiple parasite life stages through distinct, non-n

Bilateral steroid-induced glaucoma following monocular steroid therapy: a case report.

BACKGROUND: Following keratoplasty, the use of steroids is typically necessary to control postoperative inflammation and prevent graft rejection. However, improper management can lead to elevated intraocular pressure (IOP) and glaucoma. In our view, the use of topical steroid eye drops in one eye is unlikely to cause bilateral steroid-induced glaucoma. CASE PRESENTATION: An 18-year-old male with a history of keratoconus in both eyes and deep anterior lamellar keratoplasty (DALK) in the right eye. Postoperatively, the patient was prescribed 0.1% fluorometholone three times daily in the right e

AI-guided CRISPR screening reveals therapeutic targets in psoriasis.

Psoriasis affects over 125 million people globally. Biologics targeting the IL-17/IL-17RA axis are effective but require systemic administration, are costly, and are unsuitable for some patients. Developing topical small-molecule alternatives requires a better understanding of how IL-17 receptor A (IL17RA) is regulated in keratinocytes, the principal effector cells of psoriatic lesions. Here, we report a genome-wide CRISPR knockout screen for regulators of surface IL17RA in primary human epidermal keratinocytes. We prioritize hits using experimental enrichment together with VirtualCRISPR, a la

Long-lasting comfort ocular surface drug delivery by in situ formation of an adhesive lubricative Janus nanocoating.

Topical drug delivery on ocular surface always suffers from frequent administration and low bioavailability due to short drug residence. Despite advances of different adhesive ophthalmic drugs in extending release, cornea and eyelid nonselective adhesion inevitably causes ocular discomfort and even damage. Here, we describe in situ formation of an adhesive lubricative Janus nanocoating (ALJN) to enable long-lasting comfort drug delivery. By iron complexation, an asymmetric ALJN is formed on ocular surface via facile sequential instillation. The adhesive polyphenol inner layer binding with ocul

Exploring the Safety and Efficacy of Glutathione Supplementation for Skin Lightening: A Narrative Review.

The rising demand for skin-lightening products has brought glutathione, a tripeptide with antioxidant properties and melanogenesis-regulating effects, into focus as a potentially safer alternative to conventional agents. This narrative review aims to evaluate the efficacy and safety of oral, topical, and intravenous glutathione in skin-lightening therapies. Oral administration shows significant but variable decreases in melanin levels with limited side effects. Topical formulations provide good-level melanin reduction and skin texture improvement but with varying sustainability. Intravenous gl

Chemical Insights into Topical Agents in Intraocular Pressure Management: From Glaucoma Etiopathology to Therapeutic Approaches.

Glaucoma encompasses a group of optic neuropathies characterized by complex and often elusive etiopathology, involvihttng neurodegeneration of the optic nerve in conjunction with abnormal intraocular pressure (IOP). Currently, there is no cure for glaucoma, and treatment strategies primarily aim to halt disease progression by managing IOP. This review delves into the etiopathology, diagnostic methods, and treatment approaches for glaucoma, with a special focus on IOP management. We discuss a range of active pharmaceutical ingredients used in glaucoma therapy, emphasizing their chemical structu

An Overview on Spray-Drying of Protein-Loaded Polymeric Nanoparticles for Dry Powder Inhalation.

The delivery of therapeutic proteins remains a challenge, despite recent technological advances. While the delivery of proteins to the lungs is the gold standard for topical and systemic therapy through the lungs, the issue still exists. While pulmonary delivery is highly attractive due to its non-invasive nature, large surface area, possibility of topical and systemic administration, and rapid absorption circumventing the first-pass effect, the absorption of therapeutic proteins is still ineffective, largely due to the immunological and physicochemical barriers of the lungs. Most studies usin

Topically Applied Chitosan-Coated Poly(isobutylcyanoacrylate) Nanoparticles Are Active Against Cutaneous Leishmaniasis by Accelerating Lesion Healing and Reducing the Parasitic Load.

Parenteral administration of amphotericin B-deoxycholate (AmB-DOC) or pentavalent antimonials to cure cutaneous leishmaniasis (CL) results in severe adverse reactions, while topically applied antileishmanial drugs are ineffective despite their good tolerance. This work is aimed to investigate whether poly(isobutylcyanoacrylate) nanoparticles coated with chitosan (Cs-NPs) could provide intrinsic antileishmanial activity after topical application. In vitro evaluations revealed that nanoparticles were active against the promastigote, axenic amastigote, and intramacrophage forms of Leishmania majo
