Diabetes Technology Use Among Pregnant and Nonpregnant Women with T1D in the T1D Exchange.

BACKGROUND: Gestational tight glycemic control is critical for women with type 1 diabetes (T1D). Limited data exist on the adoption and retention of diabetes technologies among women in different parity strata. METHODS: We compared T1D management between T1D Exchange clinic registry participants (mean age 28 ± 9 years, 84% white non-Hispanic, and median T1D duration 13 years) who were pregnant at enrollment or year 1 follow-up ("recently pregnant" between 2010 and 2013, n = 214), ever (but not recently) pregnant (n = 1540), and never pregnant (n = 2586). We examined self-reported maternal and

Perinatal mortality in high risk pregnancy: a prospective study of preventable factors.

The present study was undertaken to identify the preventable factors operative in high fetal and neonatal losses. Over a period of one year, of total of 1,600 consecutive deliveries, 1,107 were considered to be at-risk: there were 33 fetal and 31 early neonatal deaths with an overall perinatal mortality rate of 40/1,000 births. Perinatal mortality was higher in mothers who had received inadequate antenatal care and/or with bad obstetric history. Major maternal and obstetric factors associated with a high PMR were: advancing maternal age and parity, antepartum hemorrhage, diabetes, anemia, inst

Egg donation and gestational surrogacy: Pregnancy is riskier with an unrelated embryo.

Modern medicine has revolutionized family planning. Remarkably, women1 can carry to term embryos with whom they share no genetic connection, … likely to develop preeclampsia and other diseases above and beyond the known risks associated with advanced maternal age, IVF, multiple gestation, and subfertility. Such "allogeneic pregnancies" are riskier even in fertile, healthy, commercial surrogates and when the egg is donated by a young, healthy donor. We propose that unrelated embryos present a special immune c …

Prenatal diagnosis of maternal uniparental disomy 5 by amniocentesis associated with confined placental mosaicism for trisomy 5 and fetal trisomy 21 in a pregnancy.

OBJECTIVE: We present prenatal diagnosis of maternal uniparental disomy (UPD) 5 by amniocentesis associated with confined placental mosaicism (CPM) for trisomy 5 and fetal trisomy 21 in a pregnancy. CASE REPORT: A 45-year-old woman underwent chorionic villus sampling (CVS) at 11 weeks of gestation because of maternal advanced age and an increased nuchal translucency of 4.0 mm in the first-trimester screening. CVS revealed a karyotype of 47,XY,+21[98]/48,XY,+5,+21[25]. Array comparative genomic hybridization (aCGH) analysis on the DNA extracted from chorionic villi revealed arr (5) × 3, arr (2

Maternal dietary docosahexaenoic acid supplementation attenuates fetal growth restriction and enhances pulmonary function in a newborn mouse model of perinatal inflammation.

The preterm infant is often exposed to maternal and neonatal inflammatory stimuli and is born with immature lungs, resulting in a need for oxygen therapy. Nutritional intervention with docosahexaenoic acid (DHA; 6.3 g/kg of diet) has been shown to attenuate inflammation in various human diseases. Previous studies demonstrated that maternal DHA supplementation during late gestation and lactation attenuated hyperoxic lung injury in newborn mouse pups. In the present studies, we tested the hypothesis that DHA supplementation to the dam would reduce hyperoxic lung injury and growth deficits in a m

Looking beyond prevention of parent to child transmission: Impact of maternal factors on growth of HIV-exposed uninfected infant.

BACKGROUND: Compared to HIV-infected children, relatively little has been described regarding the health status, particularly growth of HIV-exposed but uninfected children in resource-limited settings. This is particularly relevant with widespread implementation of the prevention of parent to child transmission program. METHODS: At a tertiary care health institute in India, a cohort of 44 HIV-exposed but uninfected children were followed through 6 months of age. The anthropometric parameters weight, length, and head circumference were investigated at birth, 3 weeks, 6 weeks, 3 months, and 6 m

Spatially resolved fetal and maternal cell contributions to severe preeclampsia across gestation.

The molecular and cellular pathophysiology of preeclampsia remains poorly understood, but it is increasingly clear that in addition to the placenta, other tissues of the fetal-maternal interface advance the disease. Here, we distinguish fetal and maternal contributions to the early and late presentation of severe preeclampsia by interrogating, across time and space, tissues and cell types relevant to the disease. Accounting for gestational age in a third trimester preterm cohort, we find single-cell and spatial molecular signatures of concerted hypoxia, angiogenic imbalance, fibrosis, and aber

Innovations in assisted reproductive technologies: evaluating efficacy, safety, and long-term outcomes in female infertility.

This narrative review evaluates innovations in assisted reproductive technology (ART) for female infertility, focusing on efficacy, safety, and long-term outcomes. The key objectives include synthesizing advancements such as artificial intelligence (AI)-driven embryo selection, preimplantation genetic testing (PGT), vitrification, and time-lapse imaging, along with their clinical applications. Evidence indicates that PGT can improve implantation and live birth rates per transfer in selected patient groups, that is, those with recurrent pregnancy loss or advanced maternal age. Evidence of AI-as…

Predicting under-five child mortality in Zambia: Machine learning versus parametric survival models.

BACKGROUND: In sub-Saharan Africa, child mortality remains a critical public health challenge, with Zambia exhibiting some of the highest rates in the region. While survival analysis has identified key risk factors, these methods often fail to capture complex non-linear interactions and the dynamic, time-varying nature of longitudinal markers like birth intervals. This study aimed to develop and compare predictive survival models to improve risk stratification for under-five mortality using the 2024 Zambia Demographic and Health Survey (ZDHS). METHODS: A cohort of 35,610 live births was initi

Influencing factors and predictive model of live birth involving low-grade blastocyst frozen-thawed transfer: a retrospective study.

BACKGROUND: Whether only low-grade blastocysts should undergo freeze-thaw transfer during the in vitro fertilization/intracytoplasmic sperm injection cycle remains controversial; however, high-quality embryos cannot be obtained from some patients. Therefore, we aimed to identify factors that may affect the live birth. METHODS: A total of 662 couples with only low-grade blastocysts who voluntarily accepted freeze-thaw blastocyst transfer at a single reproductive center over a 7-year period were followed-up. According to the outcome after transfer, they were divided into live birth group and fa
