America's Original Immunization Controversy: The Tercentenary of the Boston Smallpox Epidemic of 1721.

The CoVID-19 pandemic marks the 300th anniversary of the Boston smallpox epidemic of 1721, America's first immunization controversy. Puritan minister Cotton Mather learned of inoculation for smallpox from Onesimus, a man enslaved to him. When the disease broke out in May 1721, Mather urged Boston's physicians to inoculate all those vulnerable to the disease. Zabdiel Boylston, alone among his colleagues, decided to proceed with the procedure, igniting a heated debate that occasionally grew violent. The division between the advocates and detractors of inoculation were as deep as religion and pol

Impact of climate change on West Nile virus distribution in South America.

BACKGROUND: West Nile virus (WNV) is a vector-borne pathogen of global relevance and is currently the most widely distributed flavivirus causing encephalitis worldwide. Climate conditions have direct and indirect impacts on vector abundance and virus dynamics within the mosquito. The significance of environmental variables as drivers in WNV epidemiology is increasing under the current climate change scenario. In this study we used a machine learning algorithm to model WNV distributions in South America. METHODS: Our model evaluated eight environmental variables for their contribution to the o

Infection of Mammary Glands of Small Mammals in Eastern North America by Helminths.

To determine whether small mammals living in natural settings harbor helminth infections in their mammary glands, we conducted a survey of helminths infecting rodents and soricimorphs in three widespread locations in the eastern United States: states of New York, Tennessee, and Georgia. We examined all the primary organs in all hosts, and identified all helminths. We also excised the complete mammary glands within their subcutaneous fat pads, then stained and mounted each whole mammary gland set for microscopical examination. A total of 53 individual hosts were examined, including 32 Peromyscu

Endoscopic Submucosal Dissection in North America: A Large Prospective Multicenter Study.

BACKGROUND AND AIMS: Endoscopic submucosal dissection (ESD) in Asia has been shown to be superior to endoscopic mucosal resection (EMR) and surgery for the management of selected early gastrointestinal cancers. We aimed to evaluate technical outcomes of ESD in North America. METHODS: We conducted a multicenter prospective study on ESD across 10 centers in the United States and Canada between April 2016 and April 2020. End points included rates of en bloc resection, R0 resection, curative resection, adverse events, factors associated with failed resection, and recurrence post-R0 resection. RE

Patterns and processes of pathogen exposure in gray wolves across North America.

The presence of many pathogens varies in a predictable manner with latitude, with infections decreasing from the equator towards the poles. We investigated the geographic trends of pathogens infecting a widely distributed carnivore: the gray wolf (Canis lupus). Specifically, we investigated which variables best explain and predict geographic trends in seroprevalence across North American wolf populations and the implications of the underlying mechanisms. We compiled a large serological dataset of nearly 2000 wolves from 17 study areas, spanning 80° longitude and 50° latitude. Generalized linea

First Confirmed Diagnosis of Nocardioform Placentitis (Amycolatopsis lexingtonensis) in South America.

Nocardioform placentitis is a pathologically unique form of placental disease first diagnosed in central Kentucky in the mid-80s. Since then, the occurrence of nocardioform placentitis in the region has varied over the years, from sporadic cases to outbreaks. The disease has been sporadically detected in other countries and has not been confirmed in South America. A 13-year-old multiparous Mangalarga delivered a healthy filly at 340d gestation. The mare passed the fetal membranes 33 minute after foaling. Gross examination of the fetal membranes identified two focal lesions on the chorionic sur

Ticks infesting humans in Central America: A review of their relevance in public health.

Ticks are blood-sucking arthropods that parasitize most groups of terrestrial or semiaquatic vertebrates. Humans are accidental hosts to the ticks; however, in humans the ticks can cause damages varying from simple irritation to severe allergies, toxicosis, paralysis, and the transmission of pathogens, some of which can be fatal. Central America represents a narrow isthmus between North and South America and is considered a biodiversity hotspot. The importance of tick-borne diseases in this region is manifested by fatal outbreaks caused by Rickettsia rickettsii, severe and mild cases of other

A Content Analysis of Prison Websites: Exploring Approaches to Rehabilitation in Latin America.

Across much of Latin America, prison systems are characterized as having significantly high incarceration and overcrowding rates. In recent years, some of these systems have sought to address such problems by implementing rehabilitation and early release programs. However, the extent to which rehabilitation has been embraced and the nature of said efforts is not known. Using content analysis methods, this exploratory study examines the existence of language supporting rehabilitative practices and goals on official prison system websites across the region. While our findings suggest that prison

This Is America: Systemic Racism and Health Inequities Amidst the COVID-19 Pandemic.

The Coronavirus 2019 (COVID-19) continues to devastate the world and the United States remains number one of reported COVID-19 cases and deaths. Research demonstrates that Blacks and Hispanics in the United States are disproportionately impacted by COVID-19, especially among highly marginalized people at the intersection of immigration and incarceration. Social distancing is a privilege and contact tracing is a deterrent for historically oppressed populations. Public health professionals have attempted a multicausal approach to prevent the spread of infectious diseases, but they have been unsu

"The Talk" and Parenting While Black in America: Centering Race, Resistance, and Refuge.

"The Talk" refers to a specific type of racial socialization message that many Black parents have with their children about how to safely conduct themselves when interacting with police officers and other individuals in positions of power. With the recent increased exposure of racialized violence against Black people at the hands of police and vigilantes in the United States, many parents of young Black children now feel especially compelled to initiate these conversations to equip their children with the necessary knowledge to protect themselves when interacting with police officers. Ramaria flavo-brunnescens mushroom poisoning in South America: A comprehensive review.

Ramaria flavo-brunnescens is a mushroom that grows in eucalyptus forests causing poisoning in cattle, sheep and, with less frequency, in horses and pigs. It causes ulcerative and gangrenous lesions in the skin, tongue and esophagus, and loss of hairs of the tip of the tail. Nervous signs are occasionally observed in sheep. This review aims to update the knowledge about poisoning by R. flavo-brunnescens in cattle and sheep in South America. Articles published as of 1950 were retrieved, and the epidemiological data, clinical signs, pathologies, and data on the pathogenesis of intoxication were r

Chromoblastomycosis in Latin America and the Caribbean: Epidemiology over the past 50 years.

UNLABELLED: Chromoblastomycosis is a chronic disease caused by melanized fungi that mainly affect individuals performing soil-related labor. The objective of this study was to analyze the epidemiological and clinical characteristics of chromoblastomycosis in Latin America and the Caribbean by an extensive literature review. An integrative review was performed of English, French, Portuguese, and Spanish publications in LILACS, SciELO, PubMed, SCOPUS and Web of Science databases covering the period 1969-2019. A total of 1211 articles were identified, of which 132 were included in the review, cov

Seasonal and inter-annual drivers of yellow fever transmission in South America.

In the last 20 years yellow fever (YF) has seen dramatic changes to its incidence and geographic extent, with the largest outbreaks in South America since 1940 occurring in the previously unaffected South-East Atlantic coast of Brazil in 2016-2019. While habitat fragmentation and land-cover have previously been implicated in zoonotic disease, their role in YF has not yet been examined. We examined the extent to which vegetation, land-cover, climate and host population predicted the numbers of months a location reported YF per year and by each month over the time-period. Two sets of models were

Predicting eastern equine encephalitis spread in North America: An ecological study.

Eastern equine encephalitis (EEE) is a rare but lethal mosquito-borne zoonotic disease. Recent years have seen incursion into new areas of the USA, and in 2019 the highest number of human cases in decades. Due to the low detection rate of EEE, previous studies were unable to quantify large-scale and recent EEE ecological dynamics. We used Bayesian spatial generalized-linear mixed model to quantify the spatiotemporal dynamics of human EEE incidence in the northeastern USA. In addition, we assessed whether equine EEE incidence has predictive power for human cases, independently from other enviro

[Neurotropic bovine astrovirus-associated encephalitis: An underdiagnosed disease in South America?].

We describe a case of neurotropic bovine astrovirus-associated encephalitis in a Jersey dairy cow from the department of San José, Uruguay. This represents the second case of this condition reported in the Southern Hemisphere. The cow was the only one affected in a herd of 70 cows, showing neurological signs with a 2-day clinical course, before dying spontaneously. Histopathological examination revealed lymphocytic, histiocytic, and plasmacytic meningoencephalitis with neuronal necrosis, without detectable inclusion bodies. Other infectious agents, including Rabies virus(Lyssavirus), Bovine al

Current Knowledge of Small Flukes (Digenea: Heterophyidae) from South America.

Fish-borne heterophyid trematodes are known to have a zoonotic potential, since at least 30 species are able to infect humans worldwide, with a global infection of around 7 million people. In this paper, a 'state-of-the-art' review of the South American heterophyid species is provided, including classical and molecular taxonomy, parasite ecology, host-parasite interaction studies and a list of species and their hosts. There is still a lack of information on human infections in South America with undetected or unreported infections probably due to the information shortage and little attention b

Aedes aegypti spreading in South America: new coldest and southernmost records.

The geographic distribution of Aedes (Stegomyia) aegypti (L.) in South America has been expanding during the last decades. Herein we present two new distribution records that extend its southern limits towards localities with extremer environmental conditions than reported to date. San Antonio Oeste constitutes the southernmost finding for the continent (40º44'S), whereas Tandil is the infested locality with the coldest mean annual temperature in Argentina (14.17ºC). The projection of a previous distribution model for Ae. aegypti predicts these two cities as positive and suggests several other

Neonatal Screening for MPS Disorders in Latin America: A Survey of Pilot Initiatives.

Newborn screening enables the diagnosis of treatable disorders at the early stages, and because of its countless benefits, conditions have been continuously added to screening panels, allowing early intervention, aiming for the prevention of irreversible manifestations and even premature death. Mucopolysaccharidoses (MPS) are lysosomal storage disorders than can benefit from an early diagnosis, and thus are being recommended for newborn screening. They are multisystemic progressive disorders, with treatment options already available for several MPS types. MPS I was the first MPS disorder enrol

Immunological Memory to Zika Virus in a University Community in Colombia, South America.

Zika virus appeared in South America in 2015, generating alarm worldwide as it causes microcephaly and autoimmunity. This study aims to determine the serological footprint of the incoming epidemic in a student community and to characterize the memory functional cell response during post convalescence. In a cross-sectional study, Zika-specific IgG using LIA immunoassay was found in 328 university students (CI=95%), while in the second phase, the functional cellular memory response for IFN-γ and IL-2 was quantified using post-stimulus ELISpot with inactivated virus, starting with individuals ser

Dementia in Latin America: Paving the way toward a regional action plan.

Across Latin American and Caribbean countries (LACs), the fight against dementia faces pressing challenges, such as heterogeneity, diversity, political instability, and socioeconomic disparities. These can be addressed more effectively in a collaborative setting that fosters open exchange of knowledge. In this work, the Latin American and Caribbean Consortium on Dementia (LAC-CD) proposes an agenda for integration to deliver a Knowledge to Action Framework (KtAF). First, we summarize evidence-based strategies (epidemiology, genetics, biomarkers, clinical trials, nonpharmacological intervention
