Surgical Site Infiltration for Abdominal Surgery: A Novel Neuroanatomical-based Approach.

BACKGROUND: Provision of optimal postoperative analgesia should facilitate postoperative ambulation and rehabilitation. An optimal multimodal analgesia technique would include the use of nonopioid analgesics, including local/regional analgesic techniques such as surgical site local anesthetic infiltration. This article presents a novel approach to surgical site infiltration techniques for abdominal surgery based upon neuroanatomy. METHODS: Literature searches were conducted for studies reporting the neuroanatomical sources of pain after abdominal surgery. Also, studies identified by preceding

Hypertensive crisis after a successful placement of an epidural anesthetic in a hypertensive parturient. Case report.

BACKGROUND AND OBJECTIVES: The use of epinephrine-containing local anesthetic solutions in pre-eclampsia is controversial. While epinephrine in local anesthetic solutions has been used without complications in this setting, a hypertensive reaction remains a concern among many clinicians. METHODS: An epidural anesthetic containing epinephrine was administered to a hypertensive parturient for cesarean delivery. When a hypertensive crisis developed, the radial artery was cannulated and administration of sodium nitroprusside was initiated. Arterial blood samples were evaluated for plasma norepine

Wide Awake Local Anesthesia No Tourniquet Endoscopic Cubital Tunnel Release.

As demands for faster return to function, improvement in surgical scarring, and minimal disruption to personal schedule have increased, so also have the demands increased for minimally invasive procedures under the wide awake, local anesthetic, no tourniquet method. However, owing to the subcutaneous yet constrained position of the ulnar nerve at the elbow, wide-awake endoscopic cubital tunnel release has remained a technical challenge. We describe a 2-stage local anesthetic injection method that safely introduces local anesthetic within the cubital tunnel and simultaneously achieves comfort,

When pharmacologic anesthesia is precluded: the value of hypnosis as a sole anesthetic agent in dentistry.

Occasionally, a dental patient presents his/her dentist with a history of hypersensitivity to local anesthetic agents. The symptoms may include immediate reactions to the injection procedure (dizziness, shortness of breath, tachycardia, etc), or delayed reactions to the anesthetic (swelling, urticaria, etc). Although the true incidence of local anesthetic allergy is low, such a history often involves the patient's anxiety regarding the use of the drug in question, and the dentist's apprehension to treat the "problematic" patient. In such cases, hypnosis can play a major role in controlling pai

Aspiration and injection-based technique for incision and drainage of a sacrococcygeal pilonidal abscess.

The incision and drainage of a sacrococcygeal abscess is a common procedure in the Emergency Department (ED) both to decrease a patient's pain and to improve the local wound conditions for subsequent definitive surgical therapy. However, the local infiltration of anesthetics is often problematic due to the unacceptable and unavoidable pain resulting from the injection itself, as well as the inability to achieve a complete anesthetic response. Therefore, standard textbooks generally recommend the concomitant use of local and systemic analgesics in the treatment of sacrococcygeal abscesses. We d

[Case report: bilateral block of the brachial plexus--approaches, dosage and effectiveness].

A young man squeezed both hands in a bending machine with the result of complex hand injuries. Because of a high risk for general anesthesia, operation as well as postoperative analgesia was performed under bilateral brachial plexus block with the axillary approach. We discuss the considerations concerning the possible approaches with bilateral anesthesia of the brachial plexus under consideration of side effects and effectiveness. The needed dosage of the local anesthetics will be discussed.

[Pharmacotherapeutic use of antipruritic drugs in skin diseases].

Pruritus is a predominant and serious phenomenon of various skin diseases. As mediators of pruritus, histamine, serotonin (hydroxytryptamine), eicosanoids, neuropeptides, cytokines and proteases are known. The symptomatic treatment of pruritus is variable. Glucocorticoids are well known antipruritic drugs. H1-antihistaminics are used frequently. But the efficacy of these drugs is questionable in dogs. Additionally, mast cell stabilisators, unsaturated fatty acids (i. e. primrose oil, fish oil) and other antipruritic agents (local anesthetics) are used.

[Follow-up of patients with atypical facial pain].

100 patients with abnormal pain in the facial area have been examined and questioned about their symptoms and the preceding therapy. At the time of this study the patients already had a history of pain of about five years. Since no cause could be discovered for the pain, treatment was limited to symptomatic relief using injections of long-acting local anesthetics or carbamacepine. Generally, both methods resulted in a relief of the symptoms.

Neurological Complications of Local Anesthesia in Dentistry: A Review.

Local anesthesia is a technique that temporarily desensitizes a specific body area, typically for a surgical procedure, dental work, or pain management. It is described as a sensation loss in a specific area of the body due to depression of excitation in the nerve endings or due to the inhibition of the conduction process within the peripheral nerves. It allows for safer and more comfortable medical procedures, reducing the need for general anesthesia and facilitating faster recovery. Local anesthesia is generally safe, but like any medical intervention, it carries potential risks and side eff

A nomogram for calculating the maximum dose of local anaesthetic.

Toxic dose limits (mg.kg(-1)) for local anaesthetics based on body weight are well-established, but calculation of the maximum safe volume (ml) of a given agent and formulation is complex, and frequently results in errors. We therefore developed a nomogram to perform this calculation. We compared the performance of the nomogram with a spreadsheet and a general purpose calculator using simulated clinical data. Bland-Altman analysis showed close agreement between the nomogram and spreadsheet, with bias of -0.07 ml and limits of agreement of -0.38 to +0.24 ml (correlation coefficient r(2) = 0.998

The Holy Grail: long-acting local anaesthetics and liposomes.

The ability to provide an extended duration of analgesia of days following a single injection without the need for catheters, pumps and infusion systems would be a great benefit in acute and chronic pain. Exciting progress is being made in our ability to combine local anaesthetics with liposomes and polymer microspheres. These interesting formulations in animal models have allowed up to 4 days of analgesia. Their use clinically will be a great advance which could possibly occur in the near future.

Spinal anesthesia in children: pro.

Spinal anaesthesia, first introduced in children at beginning of 20th century, offers adequate intraoperative anaesthetic conditions (analgesia and muscle relaxation). The technique is easy to perform even if lumbar puncture is more difficult when the children is smaller. It has not been observed haemodynamic modifications especially in patients under 5 year of age; duration of spinal anaesthesia is shorter if compared to adults and post dural puncture headache (PDPH) after spinal anaesthesia is as common in children as it is in adults, but was usually mild and very short lasting. PDPH has not

Adverse effects of regional anaesthesia in children.

True complications of regional block procedures pertain to the performance of the block technique and the local anaesthetic. Such complications include lesions caused by the device used, and many of these complications can be avoided by using specifically designed devices.Complications related to the local anaesthetic solution mainly consist of local and systemic complications. Local toxicity has mainly been reported in adults following spinal administration of 5% lidocaine (lignocaine), a drug that is not usually used in children. Systemic toxicity consists of CNS and cardiovascular complicat

[Possibilities of neural therapy in internal medicine].

Neural therapy is the diagnostic-therapeutic application of local anaesthetics, in order to be able to treat functional, painful, reversible diseases, disturbed regulations including inflammations. The results of the cure are to be explained by complex neurophysiologic, electrophysiologic, molecularbiologic, immunologic, humoral and thus cybernetic courses of reaction. Neural therapy is to be used under two complexes. Firstly, symptomatically in the segment as weal, in trigger points, on and in joint structures, in vessels, muscular and tendon attachments, sympathetic-parasympathetic ganglia i
