Prostatitis--an increasing clinical problem for diagnosis and management.

Prostatitis remains a challenging condition. The clinical features are often nonspecific while the aetiology and pathogenesis can be diverse and includes inflammatory, obstructive, and/or chemical causes and may also be related to calculi. Four categories are recognized: acute bacterial prostatitis, chronic bacterial prostatitis, non-bacterial prostatitis and prostatodynia. The diagnosis of prostatitis was advanced substantially by the introduction of sequential sampling of urine aliquots following prostatic massage. Bacterial prostatitis is largely associated with the Enterobacteriaceae altho

Biphasic bactericidal activity of nitroxoline against Acinetobacter baumannii assessed by Raman-DIP.

The increasing antimicrobial resistance (AMR) of Acinetobacter baumannii presents a challenge to clinical management and underscores its role as a critical pathogen in refractory urinary tract infections (UTIs). Nitroxoline has gained renewed interest as a potential therapeutic option. Raman deuterium stable isotope probing (Raman-DIP), which allows for the analysis of bacterial metabolic activity, has shown promise as a tool for assessing antimicrobial efficacy. The aim of this study was to evaluate the bactericidal activity of nitroxoline against A. baumannii and the potential of Raman-DIP t

Biofilm-forming ability, antibiotic resistance and phylogeny of Escherichia coli isolated from extra intestinal infections of humans, dogs, and chickens.

Escherichia coli (E. coli) causes various infections in humans and animals. The biofilm-forming ability of E. coli has increased antimicrobial resistance and capacity to cause recurrent and chronic infections. This study determined the biofilm-forming ability of E. coli isolated from extraintestinal infections of humans, chickens, and dogs in relation to the phylogroup, type of infection, and antibiotic resistance. Isolates from chickens showed significantly higher biofilm-forming ability compared to those causing urinary tract infections in humans (p = 0.0001). Further, isolates belonging to

Oral Fosfomycin Treatment for Enterococcal Urinary Tract Infections in a Dynamic In Vitro Model.

There are limited treatment options for enterococcal urinary tract infections, especially vancomycin-resistant Enterococcus (VRE). Oral fosfomycin is a potential option, although limited data are available guiding dosing and susceptibility. We undertook pharmacodynamic profiling of fosfomycin against E. faecalis and E. faecium isolates using a dynamic in vitro bladder infection model. Eighty-four isolates underwent fosfomycin agar dilution susceptibility testing (E. faecalis MIC50/90 32/64 μg/ml; E. faecium MIC50/90 64/128 μg/ml). Sixteen isolates (including E. faecalis ATCC 29212 and E. faeci

[Distribution of phylogenetic groups, virulence factors and antimicrobial susceptibility in strains of uropathogenic Escherichia coli].

BACKGROUND: The difference between the pathogenic isolates and commensals of Escherichia coli is based on their phylogenetic antecedents. In Venezuela there are few studies that describe the pathogenic potential of phylogenetic groups in E. coli. AIMS: Relate antimicrobial susceptibility, distribution of phylogenetic groups and virulence genes in strains of uropathogenic E. coli (ECUP) isolated from patients with UTI. METHODS: We studied 17 ECUP strains, isolated from adult patients hospitalized in two health institutions. The susceptibility to 8 antibiotics was determined by the broth micro

Urinary tract infection: recent insight into the evolutionary arms race between uropathogenic Escherichia coli and our immune system.

Urinary tract infections (UTIs) are among the most common bacterial infections worldwide. Humans evolved various immune-dependent and independent defense mechanisms, while pathogens evolved multiple virulence factors to fight back. This article summarizes recent findings regarding the arms race between hosts and pathogens in UTIs. It was recently reported that macrophage subsets regulate neutrophil-mediated defense in primary UTIs but seem to subvert adaptive immunity upon re-infection. Moreover, some bacterial strains can survive inside macrophages, leading to recurrent infections. Inflammaso

Intermittent catheterization in neurologic patients: Update on genitourinary tract infection and urethral trauma.

Intermittent catheterization is considered the standard of care in most neurologic patients with lower urinary tract disorders. However, in this context, genitourinary tract infection and urethral trauma represent specific challenges. Such conditions have been found to significantly deteriorate quality of life and complicate subsequent treatments. Only optimal prevention associated with appropriate treatment allows for the long-term continuation of such bladder management. Here, we discuss the diagnosis and therapeutic and preventive approaches associated with genitourinary tract infection and

In vivo evaluation of FimH antagonists - a novel class of antimicrobials for the treatment of urinary tract infection.

The discovery of antimicrobials as β-lactam antibiotics or aminoglycosides revolutionized the treatment of infectious diseases. However, the extensive use rapidly created the problem of resistant pathogens, which are increasingly difficult to treat. FimH antagonists are a new class of antimicrobials, which target the bacterial adhesion to urothelial cells, a crucial first step in the establishment of urinary tract infections. Because of their different mode of action, FimH antagonists neither kill nor inhibit the growth of bacteria, they should have a reduced potential to generate resistant st
