Bone-sparing chemoradiotherapy for anal cancer - Results of a phase II trial by the Danish Anal Cancer Group - The DACG II (NCT05385250).

INTRODUCTION: Radiotherapy-induced bone damage and pelvic insufficiency fractures (PIFs) are known complications from pelvic radiotherapy. In anal cancer (AC), PIFs have been reported in up to 50% of patients with associated pain. This multicenter phase II trial aimed to reduce PIF rates from 50% to < 35% using bone-specific MRIs and associated symptoms in AC. METHODS: Patients with localized AC eligible for curatively intended chemoradiotherapy (CRT) were prospectively included. Detailed bone substructure delineation was performed using a standardized atlas. For each patient, a standard plan

From advanced anal squamous cell carcinoma to complete remission: A case report with clinical, radiological, and patient-narrative perspectives.

This revised case report presents the clinical course of a 42-year-old woman diagnosed with locally advanced squamous cell carcinoma of the anal canal (T4N2M1), with radiological evidence of extensive locoregional disease and suspected metastatic involvement, including para-aortic and hepatic lesions. Initially managed with palliative intent, the patient demonstrated an exceptional response to systemic chemotherapy followed by definitive chemoradiotherapy, resulting in complete radiological and clinical remission sustained over two years of follow-up. In addition to clinical data, this report

Anal symptoms and high-grade squamous intraepithelial lesions in patients from the Anal Neoplasia Clinic of Puerto Rico.

BACKGROUND: Anal cancer can cause several symptoms, such as anal bleeding, pain or lumps. Although high-grade squamous intraepithelial lesions (HSIL) are precursors of anal cancer, scarce information exists regarding its association with anal symptomatology. This study aims to assess anal symptoms and their association between anal squamous intraepithelial lesions and HSIL in a Hispanic high-risk population, as well as predictors of anal symptoms. METHODS: Data from the baseline visit of patients attending the Anal Neoplasia Clinic of the University of Puerto Rico Comprehensive Cancer Center

Local excision for anal squamous cell carcinoma: challenges in patient selection and diagnosis.

BACKGROUND: Local excision (LE) has a limited role in the management of anal squamous cell carcinoma (SCC), typically restricted to carefully selected early-stage tumors. However, in clinical practice, LE is often performed in broader contexts, particularly in patients with large perianal lesions without prior confirmation of invasive carcinoma, in whom diagnostic uncertainty may influence treatment decisions. AIMS: To evaluate oncologic outcomes after LE for anal SCC, with particular emphasis on the role of lesion characteristics, diagnostic uncertainty, and treatment patterns in real-world

Anal Intraepithelial Neoplasia Detection on Routine Colonoscopy: Review of 3550 Patients.

BACKGROUND: Human papillomavirus (HPV) is prevalent in the general population with lifetime risk of about 80% whom didn't receive HPV vaccine. AIN publications have been focusing on high risk patients for developing AIN. It is usually asymptomatic especially during the early phases; 60-70% of anal squamous cell carcinomas (ASCC) are seen in an advance stage. MATERIALS AND METHODS: We performed a retrospective review of 3550 colonoscopies done at a single institution in Henry Ford Providence Hospital. All patients underwent routine colonoscopy which had the anorectum and transition zone were i

Anal intraepithelial neoplasia in Ireland: practice patterns and the case for regionalisation.

BACKGROUND: Anal intraepithelial neoplasia (AIN) is a precursor to anal squamous cell carcinoma (ASCC). Although rare in the general population, the incidence of anal cancer is increasing particularly in high risk groups. While emerging evidence based management strategies are evolving, there is increasing recognition of the need for specialized, regionalized services to improve patient outcomes. This nationwide survey was conducted to determine the current AIN care landscape and to gauge opinion on development of centralised regional pathways for its management. METHODS: A structured survey

Primary Neuroendocrine Carcinoma of the Anal Canal with Cancer Genome Profiling.

Primary neuroendocrine carcinoma (NEC) of the anal canal is a rare, highly malignant tumor with a poor prognosis. Despite the standard first-line treatment with etoposide or irinotecan combined with cisplatin, effective second-line therapies are lacking. In 2019, Japan approved cancer genome profiling (CGP) tests for solid tumors to enhance genomic understanding. We present the case of a 79-year-old woman with NEC of the anal canal, treated with etoposide, carboplatin, and amrubicin. As Post-standard therapy, CGP suggested pemigatinib, a tyrosine kinase inhibitor; however, the patient died bef

Trends in incidence and survival from anal cancer and incidence of high-grade anal intraepithelial neoplasia in Denmark.

OBJECTIVE: The aim of the current study was to assess temporal trends in incidence of anal squamous cell carcinomas (SCC) and high-grade anal intraepithelial lesions (AIN2/3), and estimate survival from anal cancer and factors related to 5-year mortality in Denmark. METHODS: We analyzed anal SCC and AIN2/3 cases in the period of 1998-2018 from the Danish Cancer Register and the Danish Registry of Pathology, respectively. Overall, period, gender, and histology specific age-standardized incidence rates, average annual percentage change (AAPC), and 5-year relative survival were estimated. Cox pr

[Laparoscopic Abdominoperineal Resection for Anal Canal Cancer with Pagetoid Spread].

A woman in her 60s had been aware of perianal erosions for 1 month. Skin biopsy showed Paget's cells in the stratified squamous epithelium, and immunohistochemistry showed negative staining for CK7 and positive staining for CK20. Colonoscopy revealed well-differentiated adenocarcinoma in the anal canal. Computed tomography showed neither regional lymph node metastasis nor distant metastasis. Hence, she was diagnosed with anal canal carcinoma with pagetoid spread. We determined the extent of resection of the skin lesion preoperatively by mapping biopsy, and performed laparoscopic abdominoperine

Excess cancer prevalence in men with HIV: A nationwide analysis of Medicaid data.

BACKGROUND: Cancer is one of the most common comorbidities in men living with HIV (MLWH). However, little is known about the MLWH subgroups with the highest cancer burden to which cancer prevention efforts should be targeted. Because Medicaid is the most important source of insurance for MLWH, we evaluated the excess cancer prevalence in MLWH on Medicaid relative to their non-HIV counterparts. METHODS: In this cross-sectional study using 2012 Medicaid Analytic eXtract data nationwide, we flagged the presence of HIV, 13 types of cancer, symptomatic HIV, and viral coinfections using codes from

Metformin use and the risk of anal intraepithelial neoplasia in type II diabetic patients.

AIM: Emerging evidence has suggested that metformin may be protective against the development of human-papillomavirus-related cancers. Anal intraepithelial neoplasia (AIN) is highly associated with human papillomavirus infection and a precancerous status of anal cancer. The aim of this study was to investigate the relationship between metformin usage and the development of AIN in a large national sample. METHODOLOGY: The IBM MarketScan dataset was used to design a nested case-control study from 2010 to 2017. Patients aged 18-65 years with type 2 diabetes mellitus (DM) were evaluated, and case

Screening for intra-anal squamous intra-epithelial lesions in women with a history of human papillomavirus-related vulvar or perianal disease: results of a screening protocol.

AIM: Women with a history of human papillomavirus (HPV)-related cervical, vaginal or vulvar high-grade squamous intra-epithelial lesions (HSILs) or cancer are at increased risk of developing anal squamous intra-epithelial lesions (SILs) or a squamous cell carcinoma of the anus (SCCA). Screening for intra-anal SILs with high-resolution anoscopy (HRA) in high-risk populations is a subject of debate. In this study we aimed to answer the following question: what is the prevalence of intra-anal (H)SIL in women with HPV-related vulvar and/or perianal disease using HRA for screening? METHOD: A retro

Reconstruction of large perineal defects after advanced malignant tumour resection: a simple gluteal thigh flap modification.

Reconstruction of large defects in the perineal region, after resection of advanced malignant tumours is challenging because both coverage of the perineal skin defect and adequate filling of the pelvic space must be achieved simultaneously. This manuscript introduces a simple gluteal posterior thigh flap modification for the reconstruction of such defects. A trilobed flap, in the gluteal and posterior thigh regions, was designed to be of an adequate size to repair the dermal and pelvic floor defects. After the flap was elevated and rotated 90°, the posterior thigh portion was denuded. The late

Anal sphincter dysfunction in patients treated with primary radiotherapy for anal cancer: a study with the functional lumen imaging probe.

BACKGROUND: Sphincter-sparing radiotherapy or chemoradiation are standard treatments for patients with anal cancer. The ultimate treatment goal is full recovery from anal cancer with preserved anorectal function. Unfortunately, long-term survivors often suffer from severe anorectal symptoms. The aim of the present study was to characterize changes in anorectal physiology after radiotherapy for anal cancer. METHOD: We included 13 patients (10 women, age 63.4 ± 1.9) treated with radiotherapy or chemoradiation for anal cancer and 14 healthy volunteers (9 women, age 61.4 ± 1.5). Symptoms were ass

A qualitative investigation among men who have sex with men on the acceptability of performing a self- or partner anal exam to screen for anal cancer.

PURPOSE: Persistent infection with oncogenic human papillomavirus (HPV) is the primary cause of anal cancer, a disease that disproportionately affects men who have sex with men (MSM); however, there is no uniform screening protocol to detect anal cancer. This qualitative study explores whether a self-anal exam (SAE) or partner anal exam (PAE), that includes self-palpation or palpation of a partner's anal canal, is an acceptable and self-efficacious screening test, which will cue appropriate follow-up care in MSM. METHODS: Twenty-four MSM living in Houston took part in four focus group session

[Multidisciplinary Treatment for Squamous Cell Carcinoma of the Anal Canal].

Chemoradiotherapy(CRT)has been recognized as a standard treatment for locoregional squamous cell carcinoma of the anal canal in Western countries. However, surgery had historically been considered as a standard treatment and there are only a few reports on CRT for anal canal cancer in Japan. In this study, we analyzed medical records of 5 anal canal cancer patients treated with CRT in our hospital between 2005 and 2015. Patients' characteristics were as follows: median age, 70 years (range 42-80 years); male/female, 1/4; and clinical Stage I / III a/ III b, 3/1/1. The regimens of chemotherapy
