After reviewing the definition, incidence and main etiologic factors of cystoid macular edema (CME) the authors analyze the structural and physiologic characteristics of the macular area, including the blood-retinal barriers, and the basic concepts of retinal edema. Personal observations on three different groups of diseases associated with CME, retinitis pigmentosa, pars planitis and aphakia, are presented. 64 cases (66 eyes) of retinal detachment in which no breaks can be found were treated with pars plana vitrectomy, scleral buckling or encircling procedures. The selection of operation depended on the shape and size of retinal detachment, the position of peripheral retinal lattice degeneration, phakia or aphakia, traumatic situation and the appearance of proliferative vitreoretinopathy. Four children ranging from 4 to 6 years of age with unilateral aphakia and corneal scarring secondary to penetrating injuries were treated with epikeratophakia. One child had undergone a rotational penetrating keratoplasty to displace the corneal scar from the visual axis prior to the epikeratophakia procedure. Fungal growth was detected in two aphakic hydrophilic contact lenses. Aspergillus species was cultured from the extended-wear contact lens and Dermatophilus congolensis was identified by specific antibody staining of the daily-wear contact lens.
