Intraocular lenses in children.

The intraocular lens offers an experimental alternative to be considered for the visual rehabilitation of children with traumatic or unilateral infantile cataracts. The disadvantages of aphakic spectacles or contact lenses and amblyopia have committed many of these eyes to the status of visual cripples. In this series of 41 patients with a maximum of 24 months follow-up, it has been concluded that an IOL may be successfully implanted. I wish to reemphasize that vitreous involvement does not preclude IOL implantation providing all the vitreous is removed from the anterior chamber by the conclus

Accuracy of intraocular lens calculation formulas in aphakic eyes undergoing simultaneous silicone oil removal and intraocular lens implantation.

PURPOSE: To evaluate the accuracy of intraocular lens (IOL) power calculation formulas in aphakic eyes undergoing simultaneous silicone oil removal (SOR) and IOL implantation, including in-the-bag implantation and ciliary sulcus implantation, and assess the effect of 'Rule of 9s' adjustment for ciliary sulcus implantation. METHODS: The performance of six formulas (BUII, EVO, LSF, Hoffer Q, Holladay 1, SRK/T) was evaluated. Outcomes were assessed using mean prediction error (ME) for trueness, standard deviation (SD) for precision, and median absolute error (MedAE) for accuracy. Ciliary sulcus

Effects of Age at Surgery and Laterality of Cataract on Visual Acuity 5 Years after Surgery in Infants Left Aphakic.

PURPOSE: To evaluate 5-year visual acuity (VA) outcomes by age at surgery and laterality among infants left aphakic at initial lensectomy. DESIGN: Prospective Pediatric Eye Disease Investigator Group cataract registry. PARTICIPANTS: A total of 149 infants (203 eyes; 123 with bilateral surgery) underwent surgery before 12 months of age (median, 1.8; range, 0.6-11.6 months) for nontraumatic cataract without preexisting glaucoma or anterior/posterior segment anomalies who were left aphakic. METHODS: Records were reviewed annually for 5 years after surgery. Children were grouped by age at first

Predictors of early secondary IOL implantation after pediatric cataract surgery.

BACKGROUND: Pediatric aphakia may be treated conservatively with aphakic contacts or spectacles. Many families and surgeons opt for a secondary intraocular lens (IOL) when the child is older. In certain situations, pediatric aphakic patients must undergo implantation earlier than planned. The purpose of this study was to investigate how often and why early implantation occurs. METHODS: We retrospectively reviewed the medical records of consecutive patients who were left aphakic after cataract surgery in infancy and were seen at our institute at ≥4 years of age. Early implantation was defined

ONE-YEAR OUTCOMES OF A NOVEL SURGICAL TECHNIQUE FOR SUTURELESS INTRASCLERAL FIXATION OF A THREE-PIECE INTRAOCULAR LENS USING A 30-GAUGE NEEDLE.

PURPOSE: To present the 1-year results of a surgical technique for the sutureless intrascleral fixation of a 3-piece intraocular lens using a 30-gauge needle. METHODS: A retrospective chart review of a consecutive series of 9 eyes of 8 patients who underwent sutureless intrascleral fixation of posterior chamber intraocular lens using a 30-gauge needle was performed. Patients were required to have at least 1 year of follow-up to be included in the analysis. Short-term data were collected 1 month after surgery, and long-term data were collected 1 year after surgery. Data collected included visu

Surgical outcomes, complications and learning curve of glued intraocular lens of a vitreo retinal fellow in training.

PURPOSE: To evaluate surgical outcomes, complications and learning curve of glued intraocular lens surgery by a vitreoretinal (VR) fellow in training. METHODS: Analysis of 50 eyes requiring glued intraocular lens (GIOL) surgery for various indications was done. Both the consultant VR surgeon (Group 1) and VR fellow in training (Group 2) operated 25 eyes each. The primary outcome measures were visual acuity at 3 months, and time taken for completion of surgery. Secondary outcome measures were refractive correction, intraocular pressure and intraoperative or postoperative complications. RESULT

Long-term results of secondary intraocular lens implantation in children under 30 months of age.

PURPOSE: To report the long-term outcome of early secondary intraocular lens (IOL) implantation following congenital cataract extraction in a large number of eyes. METHODS: Data of aphakic children under 30 months of age who underwent secondary IOL implantation and had at least one year of follow-up after the surgery was reviewed. In all of the patients, a foldable three-piece acrylic IOL was implanted in the ciliary sulcus by the same surgeon using the same technique. The database studied included refractive and visual acuity (VA) outcomes and complications. RESULTS: Fifty patients (75 eyes

Femtosecond laser-assisted pediatric cataract surgery: Bochum formula.

PURPOSE: To assess capsulotomy enlargement after femtosecond laser treatment in pediatric cataract surgery and to assess the possible relationship to the patient's age. SETTING: Ruhr University Eye Hospital, Bochum, Germany. DESIGN: Prospective case series. METHODS: Anterior and posterior laser capsulotomies were performed in cases of pediatric cataract. The Pearson correlation coefficient (r) was used to evaluate the relationship between the programmed capsulotomy diameter and the achieved capsulotomy diameter. Linear bivalent regression analysis was used to describe the extent to which th

Optical correction of aphakia following unilateral infantile cataract removal.

PURPOSE: To compare primary intra-ocular lens (IOL) implantation with use of press-on spherical lens for aphakia correction following unilateral infantile cataract removal. METHODS: Sixty infants with a unilateral cataract underwent cataract surgery and were randomly assigned to the IOL (A group) or no IOL group (B group). Residual refractive error was corrected with spectacles in the A group and a press-on spherical lens was used to treat aphakia in the B group. Grating visual acuity (VA) was measured and patients were followed for up to 1 year. RESULTS: LogMAR VA in the operated eyes of bo

Central corneal thickness in children and adolescents with pediatric glaucoma and eye disorders at risk of developing glaucoma.

BACKGROUND: To investigate central corneal thickness (CCT) in children with glaucoma and at risk for glaucoma. METHODS: The study included 139 children with glaucoma: 66 at risk for glaucoma (ie, aphakia, aniridia, or uveitis) and 66 normal children. CCT was measured by ultrasound pachymetry and intraocular pressure (IOP) by applanation. Analysis of variance was used to compare CCT between groups. Correlation analysis assessed associations between CCT and ocular factors including spherical equivalent, cup-to-disc ratio, glaucoma medications, and number of intraocular surgeries. RESULTS: CCT

Human lens effect on in vivo scanning laser polarimetric measurements of retinal nerve fiber layer thickness.

OBJECTIVE: To determine the effect of the human lens on retinal nerve fiber layer measurements by scanning laser polarimetry. This technique uses the form birefringence of the retinal nerve fiber layer to estimate its thickness. The GDx Nerve Fiber Analyzer contains an optical retarder element to compensate for the birefringence of the human cornea. PATIENTS AND METHODS: Fourteen nonglaucomatous pseudophakic or aphakic eyes were matched by age and race to 14 nonglaucomatous phakic eyes. Intraocular pressure, optic nerve head evaluation by slit lamp biomicroscopy and visual fields were perform
