Risk factors and mortality in infants who received bag and mask ventilation at birth: a secondary analysis from the global network maternal newborn health registry.

OBJECTIVE: To identify risk factors for bag and mask ventilation (BMV) and compare mortality outcomes between infants who did and did not receive BMV. STUDY DESIGN: Secondary analysis of data from the seven sites of the Global Network Maternal Newborn Health registry, including fresh stillbirths and live births ≥ 1500 grams from 2017 to 2023. RESULTS: A total of 171 279 births (98.9% live births, 1.1% fresh stillbirths) were included. BMV was administered in 3.9% of cases. Maternal education, labor complications, delivery location, and prematurity were identified as major risk factors for re

Time to response and predictors of seizure response to phenobarbitone therapy among neonates admitted with hypoxic-ischemic encephalopathy at Nekemte Comprehensive Specialized Hospital, Ethiopia.

BACKGROUND: Hypoxic-ischemic encephalopathy is a brain injury that occurs in newborns when there is not enough blood flow to the brain. Recent studies have raised concerns about how well phenobarbitone works for treating seizures in newborns, as it may not effectively control seizures with the initial loading and repeated doses. OBJECTIVE: This study evaluated the time to respond and predictors of seizure response to phenobarbitone therapy among neonates admitted with hypoxic-ischemic encephalopathy. METHODS AND MATERIALS: A retrospective cohort study was conducted at Nekemte Comprehensive S

Perinatal asphyxia-prevalence and risk factors: a cohort study in a neonatal intensive care unit in northeast Brazil.

OBJECTIVE: Even with all the worldwide effort in the care of pregnant women and newborns in the last 10 years, perinatal asphyxia remains a crucial public health problem worldwide, being responsible for the high rates of morbidity and mortality in term and preterm newborns. In this perspective, research was carried out which aimed to investigate the prevalence and analyze the environmental and maternal-fetal risk factors associated with perinatal asphyxia. METHODS: This cohort study (2013-2018), corresponding to six years, was conducted in Fortaleza, Ceará, Brazil. The study involved 480 post

Admissions and outcomes after perinatal asphyxia and hypoxic-ischaemic encephalopathy before and after therapeutic hypothermia: a retrospective population-based study.

BACKGROUND: Hypoxic-ischaemic encephalopathy (HIE) is the leading cause of brain injury in term infants, and therapeutic hypothermia (TH) has been shown to improve outcomes for infants with moderate/severe HIE. We aimed to describe admission rates and outcomes after perinatal asphyxia and HIE before and after the implementation of TH in June 2007. METHODS: This single-centre, retrospective, population-based study included term/near-term infants born between January 2003 and December 2011 and admitted to a level III neonatal unit with International Statistical Classification of Diseases and Re

Feasibility Analysis of ECG-Based pH Estimation for Asphyxia Detection in Neonates.

Birth asphyxia is a potential cause of death that is also associated with acute and chronic morbidities. The traditional and immediate approach for monitoring birth asphyxia (i.e., arterial blood gas analysis) is highly invasive and intermittent. Additionally, alternative noninvasive approaches such as pulse oximeters can be problematic, due to the possibility of false and erroneous measurements. Therefore, further research is needed to explore alternative noninvasive and accurate monitoring methods for asphyxiated neonates. This study aims to investigate the prominent ECG features based on pH

Time to recovery of asphyxiated neonates and its' predictors among newborns admitted to neonatal intensive care unit at Debre Berhan Comprehensive Specialized Hospital, Ethiopia.

BACKGROUND: Neonatal asphyxia is a leading cause of early neonatal mortality, accounting for approximately 900,000 deaths each year. Assessing survival rates, recovery time and predictors of mortality among asphyxiated neonates can help policymakers design, implement, and evaluate programs to achieve the sustainable development goal of reducing neonatal mortality to 12/1,000 live births by 2030. The current study sought to ascertain the survival status, recovery time, and predictors of neonatal asphyxia. METHODS: A retrospective follow-up study conducted in Debre Berhan Comprehensive Speciali

Risk factors for hypoxic-ischemic encephalopathy in cases of severe acidosis: A case-control study.

INTRODUCTION: The aim of the study was to identify the obstetric risk factors for hypoxic-ischemic encephalopathy (HIE) in infants with asphyxia at birth. MATERIAL AND METHODS: This multicenter case-control study covered the 5-year period from 2014 through 2018 and included newborns ≥36 weeks of gestation with an umbilical pH at birth ≤7.0. Cases were newborns who developed moderate or severe HIE; they were matched with controls with pH ≤7.0 at birth over the same period without moderate or severe HIE. The factors studied were maternal, gestational, intrapartum, delivery-related, and neonatal

Early predictors of abnormal MRI patterns in asphyxiated infants: S100B protein urine levels.

OBJECTIVES: The early detection and stratification of asphyxiated infants at higher risk for impaired neurodevelopment is challenging. S100B protein is a well-established biomarker of brain damage, but lacks conclusive validation according to the "gold standard" methodology for hypoxic-ischemic encephalopathy (HIE) prognostication, i.e. brain MRI. The aim of the present study was to investigate the predictive role of urinary S100B concentrations, assessed in a cohort of HIE infants receiving therapeutic hypothermia (TH), compared to brain MRI. METHODS: Assessment of urine S100B concentrations

Troponin T correlates with MRI results in neonatal encephalopathy.

AIM: Troponin is a sensitive marker of asphyxia in term infants mirroring the myocardial injury sustained in global hypoxia-ischaemia. In addition, troponin is a sensitive marker of severity of stroke in adults and neonatal encephalopathy (NE). We aimed to examine the relationship between troponin T in infants with perinatal asphyxia and brain injury on MRI and correlate with neurodevelopmental outcome. METHODS: Serum troponin was sampled in infants requiring resuscitation at birth and/or neonatal encephalopathy in a tertiary referral neonatal centre. Birth history, clinical parameters, neuro

Fas-ligand and interleukin-6 in the cerebrospinal fluid are early predictors of hypoxic-ischemic encephalopathy and long-term outcomes after birth asphyxia in term infants.

BACKGROUND: Cerebral ischemia generates neuroinflammation that can induce neural cell death. This cohort study assessed whether Fas-ligand (FasL) and interleukin (IL)-6 levels in the cerebrospinal fluid (CSF) after hypoxic-ischemic encephalopathy (HIE) can serve as biomarkers of hypoxic brain injury in neonates. METHODS: Term infants (> 37-week gestational age) who were admitted to the neonatal intensive care unit of Karolinska University Hospital in years 2002 to 2004 with perinatal asphyxia were enrolled prospectively. Control infants without brain pathology underwent lumbar puncture for su
