BACKGROUND: Closure of the left atrial appendage in patients with atrial fibrillation significantly decreases the rate of future complications. Exclusion success is typically evaluated via transesophageal echocardiography, an invasive procedure typically requiring patient sedation. We investigated left atrial appendage (LAA) exclusion rates postoperatively utilizing cardiac computed tomography (CT).
METHODS: We conducted a retrospective review of patients who underwent surgical LAA closure. Closure techniques were compared utilizing postoperative cardiac CT. Successful closure was defined by …

BACKGROUND: Left atrial appendage (LAA) clip placement is increasingly used for stroke prophylaxis in atrial fibrillation during cardiac surgery. Rare mechanical interactions with adjacent grafts may cause graft failure and type 5 myocardial infarction.
CASE SUMMARY: A 75-year-old man underwent coronary artery bypass grafting with left internal mammary artery to left anterior descending artery and saphenous vein grafts to posterior descending artery and second obtuse marginal branch, with LAA clip placement. On postoperative day 2, he suffered pulseless electrical activity arrest and cardioge…

Periprocedural complications of left atrial appendage occlusion (LAAC) are varied and potentially life-threatening. We report a rare case of an iatrogenic esophageal-pleural fistula following a TEE-guided LAAC procedure. A 77-year-old woman with atrial fibrillation and a contraindication to long-term anticoagulation underwent successful LAAC. Her post-operative course was complicated by a pneumothorax and persistent pleural effusion. The diagnosis of an esophageal-pleural fistula was established after analysis of the chest tube drainage revealed food particles. Given her age and active infecti…

BACKGROUND: Peri-device leak after left atrial appendage closure has emerged as a clinically relevant finding, which has been associated with residual thromboembolic risk. Strategies to achieve complete seal after left atrial appendage closure (LAAC), including coil- and plug-based approaches, are increasingly being used in selected anatomies.
CASE SUMMARY: An 82-year-old woman with paroxysmal atrial fibrillation underwent prior LAAC. Follow-up cardiac computed tomography and transesophageal echocardiography demonstrated a skirt leak caused by incomplete sealing of the inferior aspect of the …

Growing evidence has demonstrated the clinical benefit of percutaneous left atrial appendage closure (LAAC) in patients with atrial fibrillation. Although procedural complication rates have declined with increasing experience, post-procedural device-related complications persist, impacting prognosis and reducing the long-term benefits of the procedure. Given the potential impact of these complications, surveillance imaging after LAAC is mandatory. Currently, different imaging modalities offer unique advantages to manage these complications which warrant a combined approach to optimize both sho…

BACKGROUND: Atrial fibrillation is a major stroke risk, with >90% of thrombi originating from the left atrial appendage (LAA). Oral anticoagulation is effective, but many patients cannot use it long term. Current percutaneous LAA occlusion devices reduce stroke risk but are limited by device-related thrombus (DRT) and peridevice leak.
OBJECTIVES: The aim of this study was to assess the safety and performance of the novel Zenith LAA occlusion system in both preclinical canine models and a prospective first-in-human (FIH) clinical study.
METHODS: A multistage evaluation included 3 preclinical …

BACKGROUND: Percutaneous left atrial appendage (LAA) closure (LAAC) is a proven stroke prevention strategy for patients with atrial fibrillation (AF). However, incomplete sealing in complex LAA anatomies may compromise efficacy.
OBJECTIVES: This study evaluates the safety, feasibility, and efficacy of concomitant dual-device LAAC in multilobed anatomies, representing the largest cohort examined to date.
METHODS: We reviewed all LAAC procedures performed at the University Hospital of Bern between 2009 and 2025. Baseline characteristics, procedural details, and follow-up data were analyzed for…

Transcatheter left atrial appendage occlusion (LAAO) is an alternative to systemic anticoagulation in patients with non-valvular atrial fibrillation with increased risk for thromboembolic events. Pre- and post-procedural imaging is essential for technical success, allowing practitioners to identify contraindications, select appropriate devices, and recognize procedural complications. Although transesophageal echocardiography has traditionally served as the preeminent imaging modality in LAAO, cardiac computed tomography imaging has emerged as a noninvasive surrogate given its excellent isotrop…

Transesophageal echocardiography (TEE) has been the preferred imaging modality to help guide left atrial appendage closure. Newer technologies such as the Nuvision 4D Intracardiac echocardiography (ICE) catheter allow for real-time 3D imaging of cardiac anatomy. There are no direct comparison studies for procedural imaging between TEE and 4D ICE. To evaluate the performance and safety of left atrial appendage (LAA) closure procedures with the Watchman FLX and Amulet, guided by the Nuvision 4D ICE Catheter. This retrospective observational analysis was conducted on institutional LAAO National C…

BACKGROUND: Transcatheter left atrial appendage occlusion (LAAO) has emerged as an alternative treatment for stroke prevention in patients with atrial fibrillation (AF) at high risk of thromboembolism, who cannot tolerate long-term oral anticoagulation (OAC). Questions persist regarding effectiveness and safety of this treatment and the optimal post-interventional antithrombotic regimen after LAAO.
METHODS: We retrospectively gathered data from 428 patients who underwent percutaneous LAAO in 6 Italian high-volume centres, aimed at describing the real-world utilization, safety, and effectivene…

A 79-year-old female with chronic atrial fibrillation was being treated with dabigatran (Pradaxa). Pradaxa was discontinued after a significant bleeding episode. A WATCHMAN device was successfully implanted and Pradaxa was started. A transesophageal echocardiogram (TEE) 49 days later showed a 3.6×2.2 cm clot overlying the device. Pradaxa was switched to Coumadin. Subsequent TEEs showed the complete resolution of the thrombus after five months on Coumadin. Coumadin was discontinued. Six months later, TEE showed a large mobile thrombus attached to the left atrial appendage occlusion device (LAAO…

BACKGROUND: Leaks from incomplete ligation of the left atrial appendage (LAA) following closure with Lariat device are not uncommon and associated with higher stroke rate.
CLINICAL VIGNETTE: This clinical vignette highlights a procedure of closure of residual leak following closure with Lariat device. There was a persistent central leak of 3-4 mm with bidirectional flow, confirmed on TEE. The Amplatzer Talisman PFO Occluder was chosen for the closure of the leak because of its asymmetric discs with a smaller distal disc and expandable waist.
CONCLUSION: In situations where the options are li…

Routine postprocedural imaging with transesophageal echocardiography or cardiac computed tomography angiography is the most commonly used imaging modality for follow-up surveillance usually performed 1 to 6 months after the procedure. Imaging enables recognition of well-suited and sealed devices in the left atrial appendage as well as of potential harmful complications such as peri-device leaks, device-related thrombus, and device embolization, which may lead to further surveillance observation with recurrent imaging, reinitiation of oral anticoagulants, or additional interventional procedures…
