INTRODUCTION: Spinal cord injury (SCI) disrupts autonomic control of the cardiovascular system, which may lead to autonomic dysfunction. Growing amounts of evidence support the possibility that systemic and cerebral hemodynamic dysfunctions may contribute to cognitive deficits in patients with SCI.
CASE PRESENTATION: We present a case of autonomic cardiovascular dysfunction in a 55-year old female patient following non-traumatic cervical SCI. This case illustrates how a simple arithmetic test may elicit fluctuations in blood pressure causing cognitive disturbances.
DISCUSSION: Clinical aware

Investigations are reviewed which are suitable for use in clinical situations in which lesions of the sympathetic nervous system are suspected. Particular attention is given to the problems of cardiovascular regulation as orthostatic hypotension is one of the commoner manifestations of sympathetic failure. Tests of the autonomic innervation of the eyes are also discussed. Tests of parasympathetic function are briefly described, as both parts of the autonomic nervous system may be affected by the same disease process.

The purpose of this paper is to provide an overview of autonomic nervous system and gastrointestinal changes that occur during motion sickness. It is important to consider that motion sickness is a syndrome that can vary between individuals and within individuals and between and within motion sickness-inducing stimuli. Vomiting should be considered a discrete event of the motion sickness syndrome. Given so much variability in motion sickness symptoms, it should be expected that the physiological response to motion sickness will vary as well. This appears to be the case with the autonomic nervo…

Rett syndrome is a neurodevelopmental disorder that in most cases is consequent to a mutation in the MECP2 gene. The central nervous system is the primary organ system involved in Rett syndrome. Neurophysiologic evaluations provide information concerning the developmental aspects of Rett syndrome and the character and extent of involvement of the central, peripheral, and autonomic nervous system pathways. Evoked potentials typically demonstrate intactness of peripheral auditory and visual pathways and suggest dysfunction of central or "higher" cortical pathways. Somatosensory evoked potentials…

The enteric nervous system not only innervates the colon to execute various functions in a semi-autonomous manner but also receives neural input from three extrinsic sources, (1) vagal, (2) thoracolumbar (splanchnic), and (3) lumbosacral (pelvic) pathways, that permit bidirectional communication between the colon and central nervous system. Extrinsic pathways signal sensory input via afferent fibers, as well as motor autonomic output via parasympathetic or sympathetic efferent fibers, but the shared and unique roles for each pathway in executing sensory-motor control of colon function have not

In 1969, Young et al. described a patient with an acquired, pure pan-dysautonomia of unknown etiology, involving both the sympathetic and the parasympathetic nervous systems without other neurological manifestations. Since then, a number of similar cases have been described. More recently, reports have appeared of predominantly cholinergic dysautonomia, of dysautonomia associated with infection, and of dysautonomia with other impairment of nervous function. As an etiology, immunological disorder has been suggested but the precise mechanism has not been clarified. By accumulating more cases, a

PURPOSE OF REVIEW: Cardiovascular autonomic dysfunction (AD) among cancer survivors is increasingly being recognized. However, the mechanisms and incidence are poorly understood. In this review, the clinical features, diagnostic modalities, proposed mechanisms, and currently available treatments of cardiovascular AD in cancer survivors are described.
RECENT FINDINGS: Much of our current understanding of cardiovascular AD is based on disease states such as diabetes, multisystem atrophy, and Parkinson's disease. Several non-invasive tests, measurements, and scoring systems have been developed a

The autonomic nervous system plays an integral role in the pathophysiology of ventricular arrhythmias. In the modern era, several therapeutic interventions are available to the clinician for bedside and procedural/surgical management, and there are many ways in which modulation of the autonomic nervous system can provide life-saving benefit. This review discusses some of the current treatment options, the supporting evidence, and also introduce some of the emerging therapies in this expanding field of electrophysiology.

Epileptic networks are intimately connected with the autonomic nervous system, as exemplified by a plethora of ictal (during a seizure) autonomic manifestations, including epigastric sensations, palpitations, goosebumps and syncope (fainting). Ictal autonomic changes might serve as diagnostic clues, provide targets for seizure detection and help us to understand the mechanisms that underlie sudden unexpected death in epilepsy (SUDEP). Autonomic alterations are generally more prominent in focal seizures originating from the temporal lobe, demonstrating the importance of limbic structures to the…

Neurogenic pulmonary oedema (NPE) is a life-threatening complication that develops rapidly and dramatically after an injury to the central nervous system (CNS). The autonomic system imbalance produced by severe brain damage may play an important role in the development of NPE. Activation of the sympathetic nervous system and inhibition of the vagus nerve system are essential prerequisites for autonomic system imbalance. The more severe the damage, the more pronounced the phenomenon. Sympathetic hyperactivity is associated with increased release of catecholamines from peripheral sympathetic ner

The α-synucleinopathies are a group of neurodegenerative diseases characterized by abnormal accumulation of insoluble α-synuclein in neurons and glial cells, comprising Parkinson's disease (PD), dementia with Lewy bodies (DLB) and multiple system atrophy (MSA). Although varying in prevalence, symptom patterns, and severity among disorders, all α-synucleinopathies have in common autonomic nervous system dysfunctions, which reduce quality of life. Frequent symptoms among α-synucleinopathies include constipation, urinary and sexual dysfunction, and cardiovascular autonomic symptoms such as orthos…

The diagnosis and follow-up of peripheral neuropathies involving small-diameter nerve fibers require specific examinations beyond conventional nerve conduction studies which only concern large-diameter nerve fibers. Among these tests, some are dedicated to the investigation of cutaneous innervation by the autonomic nervous system, mainly by unmyelinated sympathetic C fibers. To this end, various laboratory tests have been proposed, but the measurement of electrochemical skin conductance (ESC) by Sudoscan® is increasingly becoming the most widely used technique, because it allows a quick and si…

Respiratory complaints are not uncommon in patients with Parkinson's disease (PD). While many are explained by pulmonary and cardiovascular problems unrelated to PD, secondary effects of PD, such as kyphoscoliosis, respiratory muscle rigidity, repeated pneumonias, or side effects of medication such as dyskinesias, there is a small group of patients with paroxysmal dyspnea for whom neither anxiety or other explanation has been found. This Point of View was written to call attention to this neglected, uncommon, but very distressing symptom.

BACKGROUND: Rett syndrome (RTT), a debilitating neuropsychiatric disorder that begins in early childhood, is characterized by impairments in the autonomic nervous system that can lead to sudden unexpected death. This study explores the mechanisms of autonomic dysfunction to identify potential risk factors for sudden death in patients with RTT.
METHODS: Following the Reporting Items for Systematic Review and Meta-Analyses (PRISMA) criteria, we undertook comprehensive systematic reviews using the PubMed, Scopus, Cochrane, PsycINFO, Embase and Web of Science databases.
RESULTS: We identified an…

Ethanol and caffeine are two of the oldest human drugs. Their pervasive integration into the modern human diet may reflect behavioral attempts to correct maladaptations induced by evolutionary displacement of the autonomic system. The dietary adoption of caffeine may parallel the emergence of cognition as an independent basis of competition. Enhancement of the cognitive ability to gather and process information likely evolved as a valuable adjunct to physical behavior in prehistoric fight-or-flight encounters. Caffeine effectively exploits this pre-existing association between adrenergic activ

PURPOSE OF REVIEW: To update recent advances in the pathogenesis, pathophysiology and treatment of some autonomic neuropathies.
RECENT FINDINGS: When evaluating a patient with subacute autonomic neuropathy, certain autoantibodies are important in diagnosis and may influence management. Ganglionic antibody may be pathogenetically important while the paraneoplastic antibodies alert the clinician to the presence of an occult neoplasm. Autonomic failure is an integral component of diabetic neuropathy. Sildenafil is safe and efficacious in treating erectile dysfunction in diabetic patients. Sympat
