OBJECTIVE: Lithium remains the cornerstone of maintenance therapy for bipolar disorder despite growing use of other agents, including divalproex, lamotrigine, carbamazepine and the atypical antipsychotics. BACKGROUND: Suicidal thinking and behavior are common in individuals with bipolar disorder.
METHOD: Suicidal ideation and suicide attempts were examined in 175 patients with bipolar I disorder (diagnosis confirmed with the Schedule for Affective Disorders and Schizophrenia or the Structured Clinical Interview for DSM-IV) participating in the Pittsburgh Study of Maintenance Therapies in Bipolar Disorder. Annual rates of bipolar and schizoaffective disorder (SA) subtypes from 1981 to 1993 at a private university psychiatric hospital were reviewed. Annual total diagnostic rates for all bipolar disorders (BPD) remained constant between 1981 and 1993, (22%), while the frequency of SA rose 6-fold, from 1.4% to 8.7%. Several cytogenetic surveys and literature reviews have suggested a possible association between Klinefelter's syndrome and bipolar illness. We describe a manic patient with Klinefelter's syndrome who was treated successfully with lithium. Our patient represents the 32nd reported case of bipolar disorder in an individual with a known chromosomal abnormality, the 15th with Klinefelter's syndrome, and the 11th with a karyotype of 47XXY.
