We sought to characterize the obstetric patient who required more than the 1 or 2 units of blood products typically available by autologous donation. Medical records from 1988 were reviewed retrospectively. During this period, 16,462 deliveries were performed. Twenty-seven patients (0.16%) received more than 2 units of blood products during their pregnancies. In 116 patients treated with azathioprine and prednisolone, 2-year patient survival and primary renal allograft survival was analyzed with regard to the effects of pretransplant blood transfusion and of matching for HLA-A, HLA-B, and HLA-DR. Cyclosporine-treated recipients of primary cadaver donor renal transplants had a one-year graft survival rate of 79% if they received pretransplant blood transfusions (n = 5308). The one-year survival rate for nontransfused recipients (n = 709) was significantly lower at 69% (P less than 0.001). The consequences of administrations of A-O-incompatible blood transfusions during porcine orthotopic liver transplantations (OLT) are described. Two series, both subjected to the same standard procedure except for the administration of compatible or incompatible blood, are compared. The striking differences in peroperative and direct post-operative morbidity, mortality, and causes of death between the two series are presented. Although not generally applied, blood transfusions in experimental surgery should undergo the same precautionary measures as blood transfusions in humans.

A total of 21 patients with thalassaemia major between 2 and 20-years-old were on a "hypertransfusion" regimen for 1 to 14 year periods. The use of the transfusion quotient (TQ) allowed a precise survey of the blood requirement. Anti-Rd (Radin) was first described in association with five cases of hemolytic disease of the newborn. The authors report an anti-Rd that was demonstrated following the transfusion of a Radin positive unit of blood. The antibody appears to be of the IgG immunoglobulin class and increased in titer following the transfusion.

The influence of blood and blood-product therapy was studied in two groups of children: 1) 90 children who had exchange transfusion after birth because of serologic incompatibility (aged 5 months to 5 years).
