Resection and reconstruction for bone tumours.

For the surgical treatment of tumours in long bones a technique in which the resected bone segment is autoclaved and replaced is described. The results in three patients who had undergone this operation 7 to 11 years previously, one for chondrosarcoma and two for giant cell tumour, are reported. In all three cases sound union has been achieved. The advantages of boiled autografts are the immediate availability and the excellent fit of the graft. In order to avoid the problem of slow union of the autoclaved bone segment rigid fixation with long-term stability, preferably with a Küntscher nail a

Adolescent locked femoral nail for recurrent femoral fractures in children with osteogenesis Imperfecta: a case report and literature review.

BACKGROUND: Osteogenesis imperfecta (OI) is a hereditary connective tissue disorder characterized by increased bone fragility and recurrent fractures. The femur is the most frequently fractured site in pediatric OI patients. Traditional internal fixations such as plates and elastic intramedullary nails carry high complication rates. Although extendable intramedullary nails represent the gold standard for children with OI, they show insufficient stability in adolescent patients with large medullary cavities and severe osteoporosis. Adolescent Locked Femoral Nail (ALFN) is a fixation system desi

Limb lengthening for deformities in Ollier's disease: a systematic review.

BACKGROUND: The management of limb deformity, shortening, and bone defects in treatment of Ollier's disease is a major challenge. This study aims to summarize and compare the different surgical treatments, and to evaluate the outcome and possible prognostic factors of leg lengthening in these patients. MATERIALS AND METHODS: A systematic review of the literature from 1993 to 2017 was performed. Nineteen articles were found including a total of 121 patients with limb deformities because of Ollier's disease. The mean patients' age at the time of first surgery was 12 years. A total of 272 segmen

Malformations of the index nails.

We report a 52-year-old woman with micronychia of the index fingers. Radiographic examination revealed a Y-shaped bifurcation of the distal phalanx of both index fingers. She was diagnosed with congenital onychodysplasia of the index fingers (COIF) or Iso-Kikuchi syndrome. COIF is a rare condition characterized by a variety of nail dysplasia of the index fingers. Five criteria characterize COIF: congenital occurrence, unilateral or bilateral index finger involvement, variability in nail appearance, hereditary involvement and frequently associated bone abnormalities. Micronychia, polyonychia, a

Fractures of the proximal part of the femur.

The orthopaedic surgeon has a multitude of internal fixation devices and techniques available for use in the treatment of subtrochanteric fractures of the proximal femur. The successful use of second-generation locking nails is technically demanding. Close attention to positioning of the patient, reduction of the fracture, placement of the guide-wire, and insertion of the nail and of the proximal and distal locking screws is mandatory. The newer, high-strength hip-screws allow good fixation of a fracture that extends into the piriformis fossa. If medial comminution is present, this technique i

Does the Helical Blade Lead to Higher Rates of Fixation Failure as Compared to Lag Screw in the Cephalomedullary Nailing Treatment of Hip Fractures? A Systematic Review and Meta-Analysis.

OBJECTIVES: Recently, several studies have suggested that blade-type cephalomedullary nails (CMNs) have a higher risk of fixation failure than that of lag screws, but no clinical consensus exists. This study compared fixation failure between helical blade-type and lag screw-type CMNs with cut-out and cut-through rates as primary outcomes and degree of sliding length, time to union, and nonunion rate as secondary outcomes. DATA SOURCES: MEDLINE, Embase, and Cochrane Library were systematically searched for studies published before March 4, 2020, using the PRISMA guidelines. STUDY SELECTION: S

Preventing Wedge Deformities When Treating Intertrochanteric Femur Fractures With Intramedullary Devices: A Technical Tip.

Intertrochanteric femur fractures are common injuries associated with significant patient morbidity and mortality and high societal costs. Although intramedullary devices have shown promises for treating a wide range of intertrochanteric femur fractures, improper technique can lead to malalignment, fracture displacement, and/or iatrogenic fractures. In particular, a "wedge effect" has been described, in which the passage of conical reamers and the intramedullary nail itself results in the distraction of intertrochanteric fragments with lateralization of the shaft from the femoral neck and varu

In-out-in pinning for phalangeal fractures.

Although a commonly used technique, percutaneous pinning of phalangeal fractures is tricky because of the difficulties associated with getting the correction pin orientation when entering the medullary canal of one fragment from the outer cortex of the other fragment. This has led us to develop the "in-out-in" pinning technique. The principle consists of entering the medullary canal of one of the fragments directly from the fracture site. The pin is inserted and drilled outward through the outer cortex. The second pin is inserted in the same manner. The fragments are then aligned and the pins

A novel technique for reducing intertrochanteric hip fractures.

Intertrochanteric hip fractures typically become deformed by the muscular and gravitational forces acting on the 2 main bony fragments. Traditional use of a fracture table for anatomical reduction normally corrects for the varus angulation, external rotation, and posterior sag that can occur, but, in select unstable and comminuted fractures, reduction may not be possible because of posterior sag and external rotation of the proximal fragment. These aspects of malreduction have been addressed in multiple ways, including use of unscrubbed assistants, crutches, internal rotation of the distal fra

Case Report: Stress Fracture in an International Triple Jumper: Importance of an Integrated Care Approach Which Also Incorporates Biomechanics.

Athletes fear stress fracture (SF) injuries as they can put a premature end to their athletic careers. Understanding any mechanical constraints can suggest preventive management approach. Specifically, for the triple jump, the mechanical stresses that occur during the event appear to be the main factors for risk of injury. This clinical case describes three successive episodes of anterior tibial fracture in an international triple jumper between 2011 and 2013. The first fracture received surgical treatment involving intramedullary nailing. The second fracture occurred in the same location and

[Knee arthrodesis using a customized intramedullary nail: 14 cases].

We report our experience with knee arthrodesis using a customized intramedullary nail implanted in 14 patients. Indications for knee arthrodesis were: recurrent prosthesis infection (n=11), post-traumatic septic arthritis (n=1), aseptic loosening of a hinge prosthesis (n=1), and nonunion (n=1). A two-stage procedure was used for the 12 patients with infected joints. Mean follow-up was 19 months. Weight bearing began during the first week after arthrodesis in 13 patients. First intention bone healing was achieved in 13 patients. Mean time to healing was three months. All patients rapidly recove

Treatment of femoral fractures after irradiation.

Twelve fractures of the femur occurred after irradiation in 12 patients during the past 15 years. All 12 patients had excision of a soft tissue sarcoma of the thigh. The periosteum was stripped in every case. The fractures had a deceptively innocuous radiographic appearance, which showed minimal comminution, displacement, and shortening. Treatment of the fractures was difficult and demanding. At a mean followup of 37 months, bony union was achieved in only four patients, and in all four cases union was delayed beyond 12 months. Because fracture healing typically is delayed, a loadsharing devic

A modified second toe nail-skin flap for refined reconstruction of the distal index finger defect.

BACKGROUND: The defect of the distal index finger may cause tissue necrosis, osteomyelitis, even dysfunction, disability in hand, and psychological problems. This study aimed to present our experiences using a modified second toe nail-skin flap to repair and reconstruct the distal index finger defect. METHODS: From February 2018 to April 2022,48 patients with the distal index finger defects received the modified second toe nail-skin flap to reconstruct the defect. Among them, 35 males and 13 females, with a mean age of 39.4 years (ranged, 11∼48 years) and irregular wound, and exposed or damag

Fast pinless external fixation for open tibial fractures: preliminary report of a prospective study.

A major drawback of conventional fixator system is the penetration of fixator pins into the medullary canal. The pins create a direct link between the medullary cavity and outer environment, leading to higher infection rates on conversion to intramedullary nailing. This study was designed to prospectively evaluate the role of new rapid pinless external fixators in primary stabilization of open tibial shaft fractures. In our study, a prospective study of 96 consecutive patients of open tibial shaft fractures treated with new rapid pinless external fixator and reamed intramedullary nail was carr

Tibiotalocalcaneal arthrodesis: anatomic and technical considerations.

In the first of this two-part cadaver investigation, we inserted a specially designed, pointed device (simulating a 12-mm nail) in an antegrade fashion in each of eight fresh-frozen cadaver tibial specimens; the tibial isthmus was used as a centralizing guide. The exit point was noted, and the specimen was dissected to identify the structures at risk. In all specimens, we found that the device placed the lateral plantar artery and nerve at risk (average minimal distance from device to structure, 0 mm) and that damage to the flexor hallucis brevis and plantar fascia occurred. In addition, in si

[Pathological fracture of the femur in a patient with a venous malformation].

We observed a pathological fracture of the femur in a 21-year-old woman with a venous malformation. Physical examination and plain x-rays disclosed a pathological bone. Biopsy revealed the venous malformation. The extent of the lesion that involved the entire quadriceps muscle was detailed with duplex-Doppler, computed tomography and arteriography. The patient was treated with an intramedullary nail. Intra-operative bleeding necessitated transfusion (packed cells and frozen plasma). Outcome was favorable. Bone healing was achieved at seven months.
