A Partial Posterior Bundle Ulnar Collateral Ligament Tear in a 10-year old Boy.

Medial-sided elbow pain is becoming more common among pediatric overhead sport athletes. One potential cause of this is a partial or complete tear of the ulnar collateral ligament (UCL). Because the growth plate remains open in many of these athletes, the most common injury experienced is an avulsion at the medial epicondyle. However, although rare, there is a potential to tear the UCL, with the most common tears occurring at the anterior bundle. However, tears to the posterior bundle are quite rare in pediatric patients. We aim to describe the case of a 10-year old boy who was diagnosed with

[Cardioversion by internal electric shock of permanent atrial fibrillation].

A new technique for cardioversion of chronic atrial fibrillation was used in 17 patients whose arrhythmia had resisted all attempts at electrical and pharmacologycal cardioversion. Atrial fibrillation was badly tolerated by all patients despite digitalis administered alone (8 patients) or combined with amiodarone (9 patients). Twelve patients had left atrial dilatation at echocardiography. The 200, 300 or 400 joule electrical shock was delivered between the proximal pole of a quadripolar catheter (cathode) and a back plate (anode). The catheter was positioned at the His bundle recording site t

[Ventricular fibrillation and asystole precursors in myocardial infarct].

Ventricular fibrillation and asystole are a frequent cause of death in myocardial infarction. The data of continuous monitoring the heart in 134 patients with acute myocardial infarction and ventricular fibrillation and asystole. The immediate precursors of ventricular fibrillation were predominantly ventricular extrasystoles. A frequent precursor of ventricular fibrillation consists in paroxysmal ventricular tachycardia. In some patients the ventricular fibrillation was preceded by the block of the bundle of His, non-paroxysmal ventricular tathycardia, escaping contractions and some other arr…

Supraventricular tachycardia: diagnosis and management.

Supraventricular tachycardia (SVT) includes all forms of tachycardia that either arise above the bifurcation of the bundle of His or that have mechanisms dependent on the bundle of His. We conducted a review of the techniques used to differentiate the mechanisms of SVT. We searched the PubMed and MEDLINE databases for English-language literature published from 1970 to 2008. Articles were selected for either their historical importance or up-to-date clinical data. This review focuses on techniques for scrutinizing electrocardiograms of patients, analyzing in particular the onset of tachycardia,…

Performance of an active fixation stylet-driven lead in left bundle branch area pacing: Results from INSIGHT-LBBA.

BACKGROUND: Left bundle branch area pacing (LBBAP) has been rapidly adopted despite a lack of leads with approved indications for LBBAP. OBJECTIVE: This study aims to analyze the performance of a 6Fr active fixation, stylet-driven lead in LBBAP. METHODS: The multicenter INSIGHT-LBBA registry analyzed consecutive LBBAP implant attempts with INGEVITY+ leads. Data were retrospectively collected with standardized entry via REDCap. Prespecified safety (lead-related complication [LRC]-free rate with performance goal >90% at 3 months) and effectiveness (pacing capture threshold [PCT] ≤2 V@ 0.4 ms a

Change in left bundle branch potential-ventricular interval during lead implantation: what is the mechanism?

A 75-year-old male with sick sinus syndrome (SSS) underwent left bundle branch (LBB) pacing (LBBP) implantation. Intraoperative recordings showed that the LBB potential-ventricular (LBB-V) interval gradually decreased from 47 to 19 ms (ms). Two-year follow-up indicated stable pacing parameters and consistent electrocardiogram (ECG) results. Despite potential conduction delays caused by lead implantation, a watchful waiting strategy demonstrated recovery potential without impacting long-term prognosis or pacing parameters.

Left Bundle Branch Pacing Versus Biventricular Pacing for Acute Cardiac Resynchronization in Patients With Heart Failure.

BACKGROUND: Left bundle branch pacing (LBBP) has emerged as an alternative to biventricular pacing (BVP) for delivering cardiac resynchronization therapy. We sought to compare the acute improvement of electrical and mechanical synchrony, and hemodynamics between LBBP and BVP in patients with heart failure and left bundle branch block. METHODS: LBBP and BVP were performed and compared in a crossover fashion in patients with heart failure and left bundle branch block undergoing cardiac resynchronization therapy implantation. Electrical synchrony was assessed by QRS duration and area, mechanical

Clinical implications of electrophysiology study findings in patients with chronic bifascicular block and syncope.

Electrophysiology study was performed in 93 patients with bifascicular block and unexplained syncope. Clinical evidence of organic heart disease was present in 33 (35%). Electrophysiological abnormalities were detected in 45 patients (48%). Of these, 36 had distal conduction disease, including 28 with an HV interval greater than 55 ms (mean 76.4 ms), and eight who developed infraHisian block following either intravenous procainamide (four) or atrial pacing (four). Sick sinus syndrome was evident in six patients and a further two had carotid sinus hypersensitivity. Sustained monomorphic ventric

The mechanism of A-V conduction delay in A-V nodal gap.

Initial block in the His-Purkinje system was demonstrated in 17 of 71 patients during atrial extrastimulus testing. Of the 17 patients who demonstrated initial block in the His-Purkinje system, ten patients had A-V nodal gap (a type 1 gap). Of these ten patients, progressive increase in A-V nodal conduction was responsible for a resumption of A-V conduction in six patients, dual pathways within the A-V node was responsible in three patients, and the presence of an extra nodal pathway of the A-V node was responsible in one patient.

Comparative effects of overdrive on sinus and subsidiary pacemaker function.

Recent evidence has suggested a difference in response to overdrive pacing dependent on the location of the pacemaker within the A-V conduction system. To test this hypothesis, the effects of overdrive pacing were evaluated in five anesthetized dogs with experimentally induced A-V block and in seven patients with advanced A-V block. In the animals, sinoatrial node recovery times were studied over wide ranges of rates (130 to 210 beats per minute) and durations (30 to 180 seconds) of atrial pacing. All sinus node recovery times were less than 600 msec. with a mean maximum pause of 0.540+/-0.043

Refractory ventricular fibrillations after surgical repair of atrial septal defects in a patient with CACNA1C gene mutation - case report.

BACKGROUND: Congenital long QT syndrome (LQTS) can cause ventricular arrhythmic events with syncope and sudden death resulting from malignant torsades de pointes (TdP) followed by ventricular fibrillations (VFs). However, the syndrome is often overlooked prior to the development of arrhythmic events in patients with congenital heart diseases demonstrating right bundle branch block on electrocardiogram (ECG). We present a case of an adult patient with congenital heart disease who developed VFs postoperatively, potentially due to his mutation in a LQTS related gene, which was not identified on p

[Heart involvement in progressive external ophthalmoplegia (Kearns-Sayre syndrome): electrophysiologic, hemodynamic and morphologic findings].

The Kearns-Sayre syndrome is combined with a progressive external ophthalmoplegia (PEO), retinal pigmentary degeneration and heart block. In some patients, progression of incomplete forms has been described and potentially fatal conduction disturbances may occur. The disease is considered as a systemic mitochondrial disorder. As part of an ongoing prospective study 6 patients (3 female, 3 male; age 32 +/- 9 years) with PEO and typical ultrastructural changes of a mitochondrial myopathy in their skeletal muscle were examined. The ECG disclosed atrio/intraventricular conduction defects in 5 pati

Isolated rupture of the popliteus with posterior tibial nerve palsy.

We report the case of a 59-year-old man with severe knee pain and inability to flex his toes or invert his plantar flexed foot after an external rotation injury to his knee. MRI showed rupture of the popliteus with a haematoma compressing the neurovascular bundle in the proximal calf, and electromyography demonstrated signs of an axonotmesis of the posterior tibial nerve. There was progressive nerve recovery over 24 weeks. Isolated rupture of the popliteus should be considered in any patient with an acute haemarthrosis, lateral tenderness and a stable knee, especially after an external rotatio

Left bundle branch area pacing vs right ventricular pacing for atrioventricular block: the MELOS RELOADED study.

BACKGROUND AND AIMS: Left bundle branch area pacing (LBBAP) promotes physiological synchronous activation of the left ventricle and may be particularly beneficial in patients with atrioventricular block (AVB), but its mortality benefit remains unclear. This study aims to compare long-term survival in AVB patients receiving either LBBAP or right ventricular pacing (RVP) and to analyse predictors of mortality during LBBAP. METHODS: MELOS RELOADED, a multicentre European collaboration, was a registry-based study of pacemaker patients with AVB, left ventricular ejection fraction (LVEF) >40% and v

Apparent Unmasking of Left Anterior Fascicular Ventricular Tachycardia After Prior Left Posterior Fascicular Ablation: Possible Alternative Exit From a Shared Septal Purkinje Substrate.

INTRODUCTION: Fascicular ventricular tachycardia (FVT) is a Purkinje-mediated ventricular tachycardia classically associated with verapamil sensitivity and reentry within the left ventricular conduction network. Although catheter ablation is usually effective, recurrent tachycardia with a different fascicular QRS axis after prior ablation raises important mechanistic questions. Contemporary models suggest that the septal fascicular network may provide a shared, interconnected substrate with multiple potential fascicular exits. CASE REPORT: We report a 39-year-old man with complete heart block

Echocardiographic response from left bundle branch area pacing optimized cardiac resynchronization therapy (LOT-CRT) vs traditional CRT.

BACKGROUND: Traditional cardiac resynchronization therapy (CRT) with biventricular pacing (BVP) may be less effective in patients with nonspecific intraventricular conduction delay (NIVCD). Left bundle branch area pacing (LBBAP) combined with left ventricular (LV) coronary venous lead pacing (LOT-CRT) may be more effective in these patients. OBJECTIVE: We assessed the echocardiographic response of LOT-CRT in patients with left bundle branch block (LBBB) or NIVCD and compared it with a propensity-matched BVP cohort. METHODS: Patients with conventional CRT indications and preferentially NIVCD
