[Atlanto-axial dislocation--instability index and indications for surgery (author's transl)].

Thirty cases of atlanto-axial dislocation were investigated and the results are reported. The may be divided into 2 groups; (a) 15 cases of abnormalities of the odontoid process, i. e. 7 cases of os odontoideum, 7 cases of fractured odontoid, 1 cases of deformed odontoid, and (b) 15 cases with no abnormalities of the odontoid process. Neurological signs were noted in 14 cases which may be divided into the following three types, i.e. acute, recurrent, and chronic-progressive. The spinal canal diameter at the atlanto-axial level was measured in the position of flexion and extension using cervica

IMMU-NO versus yes: Applicability of immune checkpoint inhibitors in gynecologic cancers.

Immune checkpoint inhibitors (ICIs) have transformed the treatment paradigm for patients with gynecologic malignancies. ICIs have led to improved survival outcomes in uterine and cervical cancer, although with only modest success in ovarian cancer. In endometrial cancer, The Cancer Genome Atlas provided the framework for the molecular classification of endometrial cancer, enabling identification of the predominant predictive biomarker: mismatch repair (MMR) status. US Food and Drug Administration approvals for ICI monotherapy and in combination with targeted therapy based on MMR status quickly

Comparative morphofunctional analysis of axial skeleton excluding the skull of primates based on the anatomical, radiographic, and tomographic description of the black-striped capuchin (Sapajus libidinosus Spix, 1823).

Comparative anatomy the basis for studies of evolution, and radiographic and tomographic aspects, as auxiliary methods in the investigation of anatomical particularities, reinforce evolutionary research. Therefore, the aim of this study was to describe the vertebrae, sternum, and ribs of the capuchin monkey (Sapajus libidinosus) by means of anatomical dissection and radiographic and tomographic images. To this purpose, four cadavers were used in the anatomical analysis and five living animals for the imaging exams. The bones were described and compared with data from other primates species fou

Sport Preparticipation Screening for Asymptomatic Atlantoaxial Instability in Patients With Down Syndrome.

Down syndrome (DS) is a clinical syndrome comprising typical facial features and various physical and intellectual disabilities due to extra genetic material on chromosome 21, with one in every 1000 babies born in the United Kingdom affected. Patients with Down syndrome are at risk of atlantoaxial instability (AAI). Although AAI can occur in other conditions, such as rheumatoid arthritis, this position statement deals specifically with patients with DS and asymptomatic AAI. Atlantoaxial instability, also referred to as atlantoaxial subluxation, is defined as increased movement between the firs

First evidence of spinal arthropathy and congenital block of the cervical vertebrae in Temnospondyli.

Despite recent reports, the fossil record of pathologies in vertebrates is still patchy in terms of phylogeny, geological age and paleogeography. Here, we report several cases of pathological vertebrae in several specimens of the Late Triassic temnospondyl Metoposaurus krasiejowensis from Krasiejów. The presence of cervical block vertebrae (atlas-axis complex) is the oldest such case reported for any fossil tetrapod and the first such case reported in temnospondyls. Spinal arthropathy (or spinal arthritis) is the first such case for a non-amniote. Based on different levels of fusion among the

Progress in the pathological arena of gynecological cancers.

This review covers the significant new developments in the pathological classification of gynecological tumors. Many of these were included in the updated World Health Organization Classification of Female Genital Tract Tumours, published in 2020. Topics include the compelling evidence that a large majority of extrauterine high-grade serous carcinomas arise from the fallopian tube; the Cancer Genome Atlas (TCGA) Classification of endometrial carcinomas; the discovery that most so-called synchronous endometrial and ovarian endometrioid carcinomas represent metastasis from the endometrium to the

Antiviral T Cell Receptor Complementarity Determining Region-3 Sequences Are Associated with a Worse Cancer Outcome: A Pancancer Analysis.

Human papilloma virus has a clearly demonstrated role in cervical and head and neck cancers, but viral etiology for other solid tumors is less well understood. To expand this area of research, we obtained and analyzed the immune receptor recombinations available from both blood and tumor samples, through mining of exome files produced from those sources, for 32 cancer types represented by the cancer genome atlas (TCGA). Among TCGA data sets, the recovery frequency for antiviral complementarity determining region-3 sequences (CDR3s), for T cell receptor-alpha and T cell receptor-beta, ranged fr

Case report: nonoperative treatment of an unstable Jefferson fracture using a cervical collar.

The treatment of unstable burst fractures of the atlas (Jefferson fractures) is controversial. Unstable Jefferson fractures have been managed successfully with either immobilization, typically halo traction or halo vest, or surgery. We report a patient with an unstable Jefferson fracture treated nonoperatively with a cervical collar, frequent clinical examinations, and flexion-extension radiographs. Twelve months after treatment, the patient achieved painless union of his fracture. The successful treatment confirms prior studies reporting unstable Jefferson fractures have been treated nonopera

Atlanto-occipital fusion and spondylolisthesis in an Anasazi skeleton from Bright Angel Ruin, Grand Canyon National Park, Arizona.

The skeleton of a middle-aged female showing an unusual pattern of congenital, traumatic, and degenerative pathology was recovered from a small Kayenta Anasazi site located near the confluence of Bright Angel Creek with the Colorado River in the Inner Gorge of Grand Canyon. The atlas is fused with the base of the skull and C2 is fused with C3. The cervical region was subjected to hyperextension, perhaps through use of a tumpline, with resultant reduction of the neural canal to 8 mm, a condition that quite likely led to neurological problems. The skeleton also includes a depression fracture of

Nomogram for predicting the risk of cervical lymph node metastases and recurrence in papillary thyroid carcinoma based on the thyroid differentiation score system and clinical characteristics.

BACKGROUND: This study aimed to identify independent predictors of cervical central lymph node metastasis (CLNM), cervical lateral lymph node metastasis (LLNM), and recurrence in patients with PTC, which could help guide the surgical management of these patients. METHODS: This retrospective study analyzed data from 542 patients with PTC, who underwent thyroid surgery and were enrolled in The Cancer Genome Atlas and Gene Expression Omnibus databases. Patients were categorized into two groups based on the presence or absence of cervical LNM, classified as CLNM or LLNM. Data were randomly partit

Association of cancer mortality with postdiagnosis overweight and obesity using body mass index.

Although overweight and obesity increase cancer risk, it is still controversial with respect to cancer mortality. In the current study, we enrolled 2670 patients of 14 tumor types from the Cancer Genome Atlas (TCGA) project, to identify the prognostic role of overweight and obesity in cancer patients. After dividing the patients into different groups by the body mass index (BMI), we found significant lower mortality in the obesity group. In addition, we also treat BMI value as a binary categorical variable or continuous variable, respectively. We found significant lower mortality in the higher

Postsurgical pneumoencephaly in a patient with Klippel-Feil syndrome.

A 49-year-old woman presented with occipital headaches, dizziness, photophobia and vomiting for 2 weeks' status post posterior scalp mole removal by a general surgeon. The physical examination revealed occipital lesions with foul smelling purulent discharge mixed with clear fluid drainage, webbed neck with decreased range of motion, facial asymmetry and a low posterior hairline. A CT of head showed occipital skull defect providing a communicating pathway for scalp infection, an atlas fusion defect and an extensive pneumoencephaly. Further imaging showed low-lying cerebellar tonsils and fusion

Atlantoaxial rotary subluxation in children.

Traumatic torticollis is an uncommon complaint in the emergency department (ED). One important cause in children is atlantoaxial rotary subluxation. Most children present with pain, torticollis ("cock-robin" position), and diminished range of motion. The onset is spontaneous and usually occurs following minor trauma. A thorough history and physical examination will eliminate the various causes of torticollis. Radiographic evaluation will demonstrate persistent asymmetry of the odontoid in its relationship to the atlas. Computed tomography, especially a dynamic study, may be needed to verify th

Imaging of childhood torticollis due to atlanto-axial rotatory fixation.

Atlanto-axial rotatory fixation is a rare cause of childhood torticollis. It may occur spontaneously or may be associated with trauma, upper respiratory tract infection or congenital abnormality of the cervical spine. Presentation is usually with persistent torticollis and "cock robin" deformity of the neck. In this paper the radiological experience in three patients is presented. Investigations included plain radiographs, plain anteroposterior tomography, CT and MRI. Displacement of the lateral mass of the atlas and the eccentric position of the odontoid peg can be seen in the plain films. CT

Vertical settling in rheumatoid arthritis. Diagnostic value of the Ranawat and Redlund-Johnell methods.

The usefulness of the recently developed Ranawat and Redlund-Johnell craniometric methods was compared with that of the conventional McGregor method for diagnosing vertical settling (VS) of the skull and the atlas on the axis in 209 patients with rheumatoid arthritis (RA). Statistical analysis of the values obtained from roentgenograms revealed close correlations among the three methods. The first two methods were superior to the McGregor method because the measuring points could be identified on plain roentgenograms of the cervical spine in virtually all cases. The McGregor value could not be

Mapping of Cervical and Upper Mediastinal Lymph Node Recurrence for Guiding Clinical Target Delineation of Postoperative Radiotherapy in Thoracic Esophageal Squamous Cell Carcinoma.

BACKGROUND: The lower neck and upper mediastinum are the major regions for postoperative radiotherapy (PORT) in thoracic esophageal squamous cell carcinoma (TESCC). However, there is no uniform standard regarding the delineation of nodal clinical target volume (CTVnd). This study aimed to map the recurrent lymph nodes in the cervical and upper mediastinal regions and explore a reasonable CTVnd for PORT in TESCC. METHODS: We retrospectively reviewed patients in our hospital with first cervical and/or upper mediastinal lymph node recurrence (LNR) after upfront esophagectomy. All of these recurr

Isolated unilateral rupture of the alar ligament.

Only 6 cases of isolated unilateral rupture of the alar ligament have been previously reported. The authors report a new case and review the literature, morbid anatomy, and pathogenesis of this rare injury. The patient in their case, a 9-year-old girl, fell head first from a height of 5 feet off the ground. She presented with neck pain, a leftward head tilt, and severe limitation of right rotation, extension, and right lateral flexion of the neck. Plain radiographs and CT revealed no fracture but a shift of the dens toward the right lateral mass of C-1. Magnetic resonance imaging of the cervic
