Charged particle radiotherapy of paraspinal tumors.

Between 1976 and 1987, 52 patients with tumors adjacent to and/or involving the cervical, thoracic, or lumbar spinal cord were treated with charged particles at the University of California Lawrence Berkeley Laboratory. The histologies included chordoma and chondrosarcoma (24 pts), other bone and soft tissue sarcoma (14 pts), and metastatic or unusual histology tumors (14 pts). Radiation doses ranged from 29 to 80 Gray-equivalent (GyE), with a median dose of 70 GyE. Twenty-one patients received a portion of their treatment with photons. Median followup was 28 months. For 36 previously untreate

Location of sites of 5-hydroxytryptamine storage and metabolism by radioautography.

1. A technique for the radioautographic identification, localization, and study of the turnover of cellular depots of 5-hydroxytryptamine (5-HT) has been evaluated. A light microscopic survey was made of the uptake and turnover of 5-HT, synthesized in vivo from its administered tritium-labelled precursor, 5-hydroxytryptophan (5-HTP).2. 5-HT was taken up rapidly and retained for long periods of time by adrenal medullary chromaffin cells, gastric enterochromaffin cells, blood platelets, thyroid parafollicular cells, beta cells of pancreatic islets, mast cells, and septal cells of the lung. Glymphatic dysfunction in Parkinson's disease: Aging-associated impairments, imaging biomarkers, and therapeutic strategies.

The accumulation of α-synuclein (α-syn) is a key pathophysiological feature of Parkinson's disease (PD), and one contributing factor to this aggregation is impaired clearance mechanisms. Recent research has identified the glymphatic system as the brain's intrinsic waste clearance pathway. This review examines glymphatic dysfunction in PD, with a focus on its impact on α-syn clearance and neurodegeneration. We summarize current evidence showing that disrupted aquaporin-4 (AQP4) polarization, reduced meningeal lymphatic drainage, and sleep disturbances collectively impair glymphatic flow, thereb

Characterization of the Prenatal Ultrasound Phenotype Associated With 7q11.23 Microduplication Syndrome and Williams-Beuren Syndrome.

OBJECTIVE: This study aimed to characterize the intrauterine phenotype of fetuses with 7q11.23 microduplication syndrome and Williams-Beuren syndrome (WBS) to provide insight into prenatal genotype and phenotype correlations in the 7q11.23 region. METHODS: Seven fetuses with 7q11.23 microduplication syndrome and sixteen with WBS were diagnosed via array comparative genomic hybridization (array CGH) or copy number variation sequencing (CNV-seq) at our center. Clinical data were also systematically collected and analyzed, including intrauterine phenotype, pregnancy outcome, and inheritance. RE

Postmortem Analysis of Dolutegravir, Tenofovir, Lamivudine, and Efavirenz Penetration in Multiple Central Nervous System Compartments.

BACKGROUND: Central nervous system (CNS) compartmentalization provides opportunity for human immunodeficiency virus (HIV) persistence and resistance development. Differences between cerebrospinal fluid (CSF) and cerebral matter regarding HIV persistence are well described. However, CSF is often used as surrogate for CNS drug exposure, and knowledge from solid brain tissue is rare. METHODS: Dolutegravir, tenofovir, lamivudine, and efavirenz concentrations were measured across 13 CNS regions plus plasma in samples collected during autopsy in 49 Ugandan decedents. Median time from death to autop

The Conventional Dendritic Cell 1 Subset Primes CD8+ T Cells and Traffics Tumor Antigen to Drive Antitumor Immunity in the Brain.

The central nervous system (CNS) antigen-presenting cell (APC) that primes antitumor CD8+ T-cell responses remains undefined. Elsewhere in the body, the conventional dendritic cell 1 (cDC1) performs this role. However, steady-state brain parenchyma cDC1 are extremely rare; cDCs localize to the choroid plexus and dura. Thus, whether the cDC1 play a function in presenting antigen derived from parenchymal sources in the tumor setting remains unknown. Using preclinical glioblastoma (GBM) models and cDC1-deficient mice, we explored the presently unknown role of cDC1 in CNS antitumor immunity. We de

Outcome of Revascularizing the Left Subclavian Artery via Carotid-Subclavian Bypass.

BACKGROUND: Endovascular thoracic aortic treatment frequently requires extending the proximal landing zone up into the aortic arch with consecutive covering of the left subclavian artery orifice. Our aim was to report on our outcome of left subclavian artery revascularization using carotid-subclavian bypass via lateral access to the subclavian artery. METHODS: Patients' charts in our aortic center were screened for all those who had undergone carotid-subclavian bypass during endovascular thoracic aortic repair procedures. We analyzed perioperative complications such as cervical plexus nerve o…

Utility of Preoperative Electrodiagnosis Together with Peripheral Nerve High-Resolution Ultrasound: A Complex Case Report of Neurofibromatosis Type I.

Our case report underscores the importance of electroneuromyography (ENMG) combined with peripheral nerve high-resolution ultrasound (HRUS) in the evaluation of neurofibromatosis type 1 (NF1). A 49-year-old woman affected by NF1 came to our attention because of new-onset left arm weakness and atrophy. Debulking of a cervicothoracic C7-T1 neurofibroma had been performed 8 years earlier. On current admission, magnetic resonance imaging disclosed increased lesion volume that was thought to cause the neurologic deficits by compressing the C8 root. Findings from intraoperative neurophysiologic moni

[Pediatric Iatrogenic Vertebral Arteriovenous Fistula Successfully Treated with Endovascular Treatment:A Case Report].

OBJECT: We report a relatively rare case of pediatric iatrogenic extradural vertebral arteriovenous fistula. CASE: A five-year-old boy with a history of multiple surgeries for Total Anomalous Pulmonary Venous Return(TAPVR)exhibited vascular engorgement of his right neck from about 4 years old. We thought that this was caused by his past operations. He demonstrated swollen blood vessels in the right neck and pulmonary hypertension with increased right heart load. A right extradural vertebral arteriovenous fistula was seen on angiography. A high flow shunt was present at the V1 segment, at the

[Significance of sensory evoked potentials in the diagnosis of polyneuropathy].

In 172 patients suffering from neuropathies of different aetiologies (diabetic, uraemic, inflammatory, hereditary, alcoholic, cryptogenic) the SEP findings (cortical median and sural nerve SEP, cervical median nerve SEP, Erb's point potential) were compared with the results of conventional sensory and motor electroneurography (ENG) and with clinical signs. SEP's yielded a high percentage of abnormalities. Thus in 5 of the 6 groups the sural nerve SEP presented an unequivocal latency prolongation in 55 to 75% of the patients, in HMSN-I-patients even in 100%. Also well over 50% of the median ner

Randomized comparison between interscalene and costoclavicular blocks for arthroscopic shoulder surgery.

BACKGROUND: This randomized trial compared ultrasound-guided interscalene block (ISB) and costoclavicular brachial plexus block (CCB) for arthroscopic shoulder surgery. We hypothesized that CCB would provide equivalent analgesia to ISB 30 min after surgery without the risk of hemidiaphragmatic paralysis. METHODS: All 44 patients received an ultrasound-guided block of the intermediate cervical plexus. Subsequently, they were randomized to ISB or CCB. The local anesthetic agent (20 mL of levobupivacaine 0.5% and epinephrine 5 µg/mL) and pharmacological block adjunct (4 mg of intravenous dexamet…

Appearance of the noradrenergic markers tyrosine hydroxylase and neuropeptide Y in cholinergic nerves of the iris following sympathectomy.

Selective autonomic denervations of the iris have been used to study the possible redistribution of adrenergic markers within adult nerve fiber systems and to reveal the cellular origin of a nonsympathetic fiber plexus induced to express such markers. The presence and distribution of fibers showing neuropeptide Y (NPY)- and tyrosine hydroxylase (TH)-like immunoreactivity was studied in the rat iris using stretch-prepared whole mounts. Normal irides contained a dense regular network of NPY-positive varicose fibers. Such fibers were regularly seen innervating blood vessels. The choroid membrane

[Clinical study of cross-reinnervation in obstetrical paralysis].

It is known that cross-reinnervation occurs when regenerating nerve fibers recombine in brachial plexus injury, especially in obstetrical paralysis. This cross-reinnervation causes abnormal contraction in many muscles simultaneously during voluntary movement. To date, however, little has been reported on its pathophysiology, severity and prognosis. In order to clarify these problems, 362 cases of obstetrical paralysis were studied clinically and electromyographically, and abnormal muscle contractions during voluntary movement were analyzed. The results are as follows; Cross-reinnervation is se

BDNF-TrκB-PLCγ signaling drives BPA-induced mechanical allodynia through activating mechanosensitive Piezo2 channels.

BACKGROUND: Brachial plexus avulsion (BPA) induced neuropathic pain (NP) affects up to 90% patients with BPA, which is often characterized by mechanical allodynia. The gradual chronicity of the pain and the lack of effective treatment methods make it a challenging issue for researchers and clinicians. Mechanosensitive channel, Piezo2, has been reported to participate in several physiological processes including NP. However, whether it plays a role in BPA-induced NP and related mechanisms at the cervical dorsal root ganglion (DRG) level remains unclear. In this study, we focused on the peripher

Do cervical medial branch blocks have a therapeutic role? a prospective cohort study.

BACKGROUND: Cervical medial branch blocks (CMBB) are used for the diagnosis of facet joint-related pain. There have been several reports suggesting that they can provide a benefit that significantly outlasts the expected duration of diagnostic blocks. We undertook this study to determine the frequency and extent of this effect in clinical practice. METHODS: 179 patients undergoing 830 individual block levels and who had previously reported 50% relief following a CMBB were recruited using a prospective cohort design and blocks were performed in an outpatient setting. Outcomes monitored at base

The Great Auricular Nerve Trigger Site: Anatomy, Compression Point Topography, and Treatment Options for Headache Pain.

BACKGROUND: Peripheral nerve decompression surgery can effectively address headache pain caused by compression of peripheral nerves of the head and neck. Despite decompression of known trigger sites, there are a subset of patients with trigger sites centered over the postauricular area coursing. The authors hypothesize that these patients experience primary or residual pain caused by compression of the great auricular nerve. METHODS: Anatomical dissections were carried out on 16 formalin-fixed cadaveric heads. Possible points of compression along fascia, muscle, and parotid gland were identif

The Greater Occipital Nerve and Obliquus Capitis Inferior Muscle: Anatomical Interactions and Implications for Occipital Pain Syndromes.

BACKGROUND: The compression/injury of the greater occipital nerve has been identified as a trigger of occipital headaches. Several compression points have been described, but the morphology of the myofascial unit between the greater occipital nerve and the obliquus capitis inferior muscle has not been studied yet. METHODS: Twenty fresh cadaveric heads were dissected, and the greater occipital nerve was tracked from its emergence to its passage around the obliquus capitis inferior. The intersection point between the greater occipital nerve and the obliquus capitis inferior, and the length and
