A novel heterozygous mutation of ANKRD11 causes KBG syndrome in a preterm neonate: a case report and literature review.

The KBG syndrome (KBGS) affects several systems caused by the mutation of the ANKRD11 gene. The main manifestations of KGBS included hearing loss, feeding difficulties, craniofacial abnormalities, tooth deformity, and developmental delay (delayed overall development, convulsions, and intellectual abnormalities). Only 10%-26% of patients with KBG syndrome have congenital heart disease, including atrial and ventricular septal defects. Here, we report a case of KBG syndrome in a preterm newborn with low birth weight, a huge ventricular septal defect, and a congenital chylothorax. Whole-exome sequ…

Retrograde transvenous lymphatic embolization for postoperative chylous ascites: A report of three cases and literature review.

Percutaneous transabdominal lymphangiography and embolization have been reported as useful approaches for intractable chylothorax or chylous ascites. However, they are often difficult to perform after extensive lymph node dissection because disruption of the antegrade lymphatic flow makes leaks identification difficult. When the leakage point cannot be identified or percutaneous transabdominal lymphangiography and embolization fail, a retrograde transvenous approach to the thoracic duct can be used instead. We report 3 cases of refractory chylous ascites after retroperitoneal operation or exte…

Successful treatment of a patient with chyluria due to lymphangioleiomyomatosis using sirolimus.

Lymphangioleiomyomatosis (LAM) is a rare and progressive neoplastic disease of young woman, characterized by the proliferation of abnormal smooth muscle-like cells (LAM cells) in the lungs and axial lymphatics. A 44-year-old woman was referred to our hospital because pleural effusion was detected during a health checkup. She had chylothorax, chylous ascites, and chyluria, and her computed tomography scan showed a solid tumor in the pelvis. Surgical biopsy was performed; she was diagnosed as having LAM. We could not control the fluid collection and chyluria using standard medical treatments. Th…

Gastrointestinal Tract Involvement of Gorham's Disease with Expression of D2-40 in Duodenum.

We present a case of a 13-year-old boy with Gorham's disease involving the thoracic and lumbar spine, femur, and gastrointestinal (GI) tract, which was complicated by recurrent chylothorax and GI bleeding. The presenting symptoms were intermittent abdominal pain, back pain, and melena. Esophagogastroduodenoscopy and colonoscopy showed no abnormal lesions, but duodenal biopsy showed marked dilation of the lymphatics in the mucosa and submucosa, which revealed positive staining with a D2-40 immunohistochemical marker. In cases of GI bleeding with osteolysis, the expression of a D2-40 marker in t…

Persistent atrial standstill in a cat.

A domestic shorthaired cat was presented with a 1-month history of cardiomegaly and recurrent chylothorax. The heart rate was 130 beats/min and no P waves were present on a surface electrocardiogram. Thoracic radiographs and an echocardiogram demonstrated severe biatrial dilatation, pleural effusion and restrictive pleural disease. Permanent atrial standstill was suspected. Pleurocentesis was performed and therapy was started with enalapril, frusemide and aspirin. Intracardiac electrograms revealed no atrial activity, and atrial pacing failed to elicit atrial or ventricular depolarisations. Th…

Salvage and reinfusion of chyle in closed chest injury.

The aim of this study was to describe a system of salvaging and reinfusing chyle which accumulated in the right pleural cavity of a patient after a thoracic duct lesion caused by a closed chest injury associated with amyelic fracture of the dorsal spine D10-D11. The chyle was collected in a reservoir (BT 844 Dideco), transferred by an electronic pump (BT 797 recovery Dideco) to a storage bag, microfiltered and then reinfused to the patient A solution was needed to prevent the patient with severe chylothorax, from having immunological and metabolic imbalance. The long period of conservative tre…

Yellow nail syndrome: a syndrome with prominent manifest involvement of the respiratory system.

BACKGROUND: Yellow Nail Syndrome (YNS) is a rare disorder characterized by the classic triad of yellow nails, lymphedema, and chronic respiratory involvement. CASE PRESENTATION: We report a 42-year-old male with YNS and childhood-onset symptoms lasting for over three decades. He presented with refractory, recurrent chylothorax that progressed to organized pleural effusion and pleural calcification, complicated by respiratory failure and pericardial effusion. Systemic lymphangiography confirmed severe lymphatic reflux and obstruction. CONCLUSION: This case highlights the diagnostic and therap…

Chyle leakage after blunt trauma.

Three patients developed chylous leakage after major blunt trauma. In one patient with non-remitting right-sided chylothorax, lymphangioscintigraphy as well as conventional oil contrast lymphography demonstrated disruption of the thoracic duct at the aortic hiatus which eventually required transpleural mediastinal ductal ligation proximal to the fistula. The other two patients had chylous retroperitoneum and/or chylous peritoneum which was self-limited although one patient (a three-year-old boy) died of multiorgan failure from associated pulmonary contusions and cervical spine injuries. Chylou…

A 67-Year-Old Woman With Abdominal Pain and Dyspnea.

A 67-year-old woman presented to her primary physician with a year of periumbilical abdominal discomfort with diarrhea, bloating, and unintentional weight loss. While undergoing workup for the abdominal pain, she presented to the ED with dyspnea that worsened over a few weeks. She had no associated chest pain, lower extremity edema, fevers, or chills, but endorsed orthopnea. The patient's medical history included hypothyroidism and diabetes mellitus. Her family history was significant for coronary disease in her father and hypertension in her mother. She had no recent travel; she is a nonsmoke

Lymphatic dysplasias in newborns and children: the role of lymphoscintigraphy.

We performed lymphoscintigraphy in 15 patients (newborns and children) affected by congenital lymphatic dysplasia. We suggest that lymphoscintigraphy is mandatory in all patients with signs of lymphatic dysplasia, including those with minimal and initial signs of lymphatic impairment, to obtain very early diagnosis and begin treatment.

A novel technique of management of high output chyle leak after neck dissection.

The occurrence of postoperative chyle leak after neck dissection or vascular surgery of the subclavian and vertebral artery is a well-known but rare complication. The management of chyle leak ranges from utilizing a myriad of conservative procedures to surgical exploration. Occasionally on surgical exploration the chyle leak is diffuse and ligation of the bed of thoracic duct is not adroit to stopping the chyle leak. We describe a technique, which will be useful in this circumstance and in recurrent chyle leak after a previous exploration.

Platelet-derived growth factor receptor-beta in Gorham's disease.

BACKGROUND: A 17-year-old male presented with pain in his lower-left chest. He had no significant medical history and was previously in good health. He had a fractured ninth left anterior rib and the tenth, eleventh and twelfth ribs were absent, which was thought to be a congenital anomaly. Several months later, he presented again with back pain, an enlarging mass in the lower-left chest wall, erosion of the lateral pedicles of the lower thoracic vertebrae and pleural effusion. INVESTIGATIONS: Physical examination, chest X-ray, MRI of the spine, incisional biopsy, serial CT imaging of the hem

Efficacy of Sirolimus in Treating Refractory Lymphatic Malformation in Noonan Syndrome: A Case Study.

A female patient with Noonan syndrome, attributed to a pathogenic RIT1 variant, presented with white perineal discharge at 8 years and 5 months of age. She had a history of chylothorax and recurrent respiratory infections requiring hospitalization. Discharge analysis revealed 99.5% lymphocytes and elevated triglyceride levels (1939 mg/dL; 21.9 mmol/L). Magnetic resonance imaging and contrast-enhanced computed tomography identified lymphatic abnormalities extending from the thoracic to the pelvic region. We suspected that the chylous ascites was being discharged through the genitals. Administra…

Sarcoidosis of the pleura--A case report.

INTRODUCTION: Pleural involvement is an uncommon manifestation of sarcoidosis. It may manifest as pleural effusion, pneumothorax, pleural thickening and nodules, hydropneumothorax, trapped lung, hemothorax, or chylothorax. The incidence of pleural effusion with sarcoidosis ranges from 0% to 5% but has been reported to be as high as 7.5%. Pleural effusions complicate sarcoidosis in < 3% of patients. CASE REPORT: We reported a 64-year-old male patient with chronic multiorgan sarcoidosis. This patient developed pleural sarcoidosis with massive pleural effusion several years after the diagnosis o…

Syncope associated with paroxysmal atrioventricular block an ventricular standstill in a cat.

A four-and-a-half-year-old neutered male shorthair cat was presented with a three-week history of episodic fainting. Twenty-four hour electrocardiographic (Holter) recording revealed frequent prolonged episodes of complete atrioventricular (AV) block with ventricular standstill. The fainting episodes coincided with the longest periods of ventricular inactivity. A permanent transjugular pacemaker was placed to prevent further life-threatening episodes of syncope. Three months after discharge, the patient was re-presented with sudden onset dyspnoea associated with chylothorax and electrocardiogr…
