BACKGROUND: Thyroid eye disease (TED) presents heterogeneous clinical manifestations requiring nuanced, individualized management. Large language models (LLMs) have shown potential in clinical reasoning, but their consistency with physician decision-making in nonsurgical TED management remains unclear.
METHODS: A structured 19-item questionnaire covering medical, injection-based, and radiotherapeutic treatments for TED was distributed to 17 oculoplastic surgeons across the Asia-Pacific region. Responses were binarized (1 = selected, 0 = not selected) and compared with standardized outputs fro…

This paper examines how clinicians in Hospice and Palliative Medicine (HPM) help patients and colleagues resolve ambiguous clinical situations at the end of life. 'Clinical ambiguity' is advanced as a concept to describe how the heuristics used to match medical knowledge to patient interests produce diverging imperatives when patients are terminally ill. Separate from uncertainty, clinical ambiguity captures not a lack of definitive information or knowledge, but the limits of routinised knowledge and practice when applied to patient care. HPM clinicians, typically at the request of other clini

INTRODUCTION: Hypertensive disorders of pregnancy, such as pre-eclampsia (PE), are one of the leading causes of maternal and perinatal deaths in low- and middle-income countries (LMICs). Although clinical practice guidelines (CPGs) for PE prevention and management are available, there is limited information on their implementation in LMIC contexts. This realist synthesis therefore aims to uncover the causal explanations underpinning the implementation of CPG recommendations for PE prevention and management in Eastern, Central and Southern African contexts. By developing and refining initial pr

Patients are increasingly turning to large language models for health information, which makes the consistency of these systems' outputs across patient groups a public health concern. Demographic bias is harder to see than fabrication because it escapes accuracy benchmarks and shows up instead in the language the models produce. Gender bias is the form most consistently reported, with effects on clinical documentation and reasoning that often compound with ethnicity, socioeconomic status and other attributes. Drawing on evidence from gender medicine, we argue that it calls for sex- and gender-

Detecting tuberculosis on X-rays remains complex and challenging for clinicians. In Burkina Faso, the scarcity of radiologists and their hea … is article, we present our three-step approach for tuberculosis detection, directly inspired by the clinical reasoning of the radiologist. First, we classify the images into those that are normal and those that are not. Next, we segment the images to keep only the pulmonary area. Finally, in this area, we perform a detection of tuberculosis-related anomalies.

Candida glabrata is a pathogenic yeast with several unique biological features. This article provides an up-to-date review on current data and reasoning aspects of this clinically problematic organism. Haploidy, absence of pseudohyphae, facultative anaerobe growth of C. glabrata, as well as its intrinsically low susceptibility to azole antifungals require specific consideration in diagnosis and treatment approaches. As C. glabrata today represents a sizeable percentage of pathogens in candidaemia, the use of azole antifungals in upfront therapy of invasive yeast infections is discouraged by re

Careful study of the phenotype can have implications at several levels, namely clinical diagnosis, pathophysiological reasoning, management planning, and outcome measurement. Behavioural phenotypes involve cognition, communication, social skills, and motor control. They can be documented in a host of neurodevelopmental conditions and approached with the recently refined perception-action-cognition-environment (PACE) paradigm, which focuses on the neurodevelopmental processes that underlie learning and adaption to the environment through perception, action, and cognitive processing. Although th

Comprehensive neuropsychological assessment was performed on a series of patients with treated classical phenylketonuria. All patients showed a similar cluster of performance deficits in motor and visual-motor functions, spatial operations, academic skill areas, and select areas of higher level reasoning. The results are consistent with other neuropsychological studies of patients with early and late diet termination that have used differing assessment methods, thus allowing for greater confidence in the findings. Examination of underlying factors indicates that poor motor coordination, limits

Ethics education in biomedical research in India is frequently limited to regulatory compliance, with inadequate emphasis on ethical reasoning, contextual vulnerability, and deliberative review processes. As part of the Silver Jubilee of the Institutional Ethics Committee of St John's Medical College Hospital, a one-day preconference workshop titled "Responsible Researchers and Reviewers: Ethics of Good Clinical Practice" was conducted during the 13th International Conference of Ethics Education. The workshop employed case-based, interdisciplinary discussions focusing on vulnerability, ethics

Spinocerebellar ataxia type 32 (SCAR32) is a rare autosomal neurodegenerative disorder caused by mutations in the peroxiredoxin 3 (PRDX3) gene, which encodes a mitochondria-specific antioxidant enzyme critical for maintaining cellular redox homeostasis. Here, we reported a case of a homozygous nonsense mutation (c.619 C > T; p.Arg207*) in PRDX3 of a 12-year-old Chinese boy. This mutation was predicted to result in premature termination of protein translation. Consequently, the patient presented with slowly progressive gait ataxia and cerebellar vermis atrophy. Mild cognitive impairment was als

A study was based on the survey of 419 neurologists and 1189 their patients with different forms of cerebrovascular diseases using a specially developed questionnaire. In most cases, vasobral was used as a monotherapy or in a complex treatment. Twenty-two percent of physicians reported that vasobral was the most effective compared to other drugs. The good tolerability of treatment (18%) and the broad spectrum of indications and clinical effects (17%) were reported as well. The large percentage (75%) of patients indicated the positive effect of vasobral on memory, reasoning, vertigo etc The max

This study examined the pattern of results on the Wechsler Intelligence Scale for Children (WISC-IV; French version) for 60 French children with dyslexia, from 8 to 16 years of age. Although use of WISC-III failed to clearly identify typical profiles and cognitive deficits in dyslexia, WISC-IV offers an opportunity to reach these objectives with new indexes and subtests. The mean performance analysis showed a Working Memory Index (WMI) at a limit level, significantly lower compared to the three other indexes. The WMI was the lowest index for 68% of the population studied and was significantly

BACKGROUND AND PURPOSE: Cranial venous outflow insufficiency, a model of brain dysfunction based on partial or intermittent obstruction to cranial venous drainage, is an attempt to explain a clinical phenotype characterised by multiple complex symptoms, including headache, fatigue and cognitive dysfunction, that can be responsible for long term neurological disability. This concept, however, has been received with some scepticism first, because its supposed symptoms seem mainly non-organic and, secondly, because its supposed lesions can be regarded as no more than variants of normal craniocerv

This article examines a recent Canadian case in which a chiropractor is found liable after a patient experienced cervical artery dissection and stroke allegedly linked to cervical spinal manipulation. Two questions dominate: whether informed consent is valid and whether the final treatment caused the injury. Using a clinician-oriented lens applied to the trial reasons, the review maps the courts' reasoning against epidemiology, biomechanics, and contemporary clinical guidance. The trial court finds that the consent disclosure minimizes material risk, and the court of appeal upholds that inform
