The aim of the study was to determine final clinical and radiological results of epiphyseal fractures treated in our hospital. Thirty-five patients were included in the study (6 girls and 29 boys) aged from 5 to 17 years (mean 12.1 years). Follow up ranged from 2 to 20 years. According to Salter and Harris classification system 15 patients had type I and 20 had type II fracture. According to the Neer-Horowitz classification system of the proximal end of humerus one patient had grade III and three had grade IV fracture. Physeal fractures included: proximal end of humerus (n = 4), distal end of

Metaphyseal and physeal fractures of the distal radius are common in children. Most cases are best treated with closed reduction and cast immobilization. Long-term outcomes of these injuries are excellent when specific treatment principles of reduction and casting are followed. Surgical indications are limited and include open fractures, intra-articular fractures, non-reducible fractures, unstable fractures, and the presence of associated nerve injury. Closed reduction and percutaneous pin fixation is the most commonly used surgical option. The clinician should be aware of delayed complication

Since closed treatment is effective in 80% to 85% of distal femoral fractures, operative intervention is not recommended. Unnecessary surgery produces unwarranted complications. The cast-brace technique should be used in the first week or two after injury to prevent deformities commonly associated with distal femoral fractures. The thigh cast should be carefully molded well over the femoral condyles. Distal femoral traction is preferred to proximal tibial traction to correct fracture deformities. If the closed reduction technique is not satisfactory in the early period after injury, operative

BACKGROUND: The acetabulum exhibits an irregular anatomical structure with a dense concentration of critical blood vessels and nerves in its vicinity. Acetabular fractures accompanied by periarticular soft tissue injuries may lead to life-threatening complications. The Letournel-Judet classification system is currently the widely adopted standard for categorizing acetabular fractures, with the both-column fracture type recognized as the most complex pattern within this framework. Open reduction and internal fixation remains the gold standard for its management. The minimally invasive closed re

Compared with closed reduction or older fixation methods, open reduction and compression plate fixation has dramatically improved the outcomes of displaced diaphyseal forearm fractures. However, the procedure can be technically demanding, with implant choice, surgical approach, accuracy of reduction, and sufficient fracture stability to allow early postoperative motion all having been shown to affect outcome. The procedure is performed with the following steps:The patient is positioned supine on the operating room table with the arm on a hand-table and a tourniquet applied to the upper arm.Aft

BACKGROUND: Using conventional complex facial fracture management principles, confirmation of adequate facial fracture reduction can be achieved only by obtaining a postoperative computed tomography (CT) scan. If the CT scan revealed any discrepancy in fracture alignment, additional procedures for correction may be required. The concept of intraoperative CT scanning provides immediate postreduction or intraoperative information that orients the surgeon to the potential need for additional maneuvers for improved fracture reduction and osseous fixation. We assessed the early technical feasibilit

Trapezium fractures are unusual; however, they represent the third most frequent fracture of the carpal bones. As they usually follow a high-energy trauma, they are associated with distal radius, Bennett, or Rolando fractures in 80% of cases. Traditional treatment options include, closed reduction and percutaneous pinning, or open reduction and internal fixation. To minimize the additional surgical trauma, an arthroscopic technique has been developed for safe, minimally invasive management of complex injuries of the first carpo-metacarpal joint. Traumatic hip dislocations and dislocation of total hip arthroplasties are common injuries. Closed reduction of either a dislocated hip or a total hip arthroplasty can be a demanding procedure and may necessitate open reduction if unsuccessful or lead to further complications. These include fracture or neurovascular injury to the patient, as well as injury to the surgeon when reducing the hip. This article describes a technique that reliably reduces dislocations while minimizing complications.

We have illustrated instances in which open reduction with internal or external fixation should be used in the care of fractures in children. These methods should be used when closed reduction fails or in other circumstances in which they are the initial treatment of choice. At no time should the surgeon hesitate to utilize these operative options if they represent the best method of achieving the most functional end result in a pediatric fracture.

INTRODUCTION: Subtalar dislocations are rare injuries, representing around 1% of all dislocations. Defined as talonavicular and talocalcaneal separation, occurs in medial, lateral, anterior, or posterior direction. They are more common in young men, usually as a result of high energy trauma and are associated with posttraumatic arthritis, especially when accompanied with peritalar fracture (a cuboid fracture in this case), so prompt recognition is necessary in order all these complications to be avoided.
CASE PRESENTATION: This study presents a rare case of subtalar dislocation associated wit

BACKGROUND: According to some orthopedic surgeons, almost all supracondylar humerus fractures should be treated operatively by reduction and pinning. While according to others, closed reduction and immobolization should be used for Gartland type II and some type III fractures. However, the limit of this technique remains unclear. We present 74 patients with displaced extension-type supracondylar fractures treated by closed reduction and immobilization with a collar sling fixed to a cast around the wrist. The purpose of the study is to give a more precise limitation of this technique.

Anterior shoulder dislocation occurs in 95% of all shoulder dislocation. Pathological changes such as bone loss may occur in neglected cases, which can cause locking of the humeral head, complicating the procedure needed for reduction. A 26-year-old male patient presented to the clinic with pain and limited range of motion 2 months after a fall, with a history of unsuccessful treatment by a traditional bonesetter and recurrent right shoulder dislocations. The patient was diagnosed with bilateral neglected closed fracture dislocation of proximal humerus Neer 2-part and underwent open reduction

An 82-year-old man fell from a motorcycle and was admitted with superolateral dislocation of the bilateral mandibular condyles. The displaced condyles on the right and left were hooked in the posterior root of the zygomatic arch and in the temporal fossa beyond the zygomatic arch, respectively. There was a sagittal split fracture on the medial side of the right mandibular condyle and a right mandibular body fracture. Under general anesthesia, manual reduction of the dislocation was successfully performed the day after the injury. Bilateral superolateral dislocations hooked to the temporal foss

Nasal bone fracture is the most common of the fractures of the facial skeleton. For centuries, these injuries have been managed with closed reduction, but because of low surgeon satisfaction rates and high revision rates, open reduction is advocated in cases involving severe deviation of the nasal dorsum associated with septal fractures. There are many surgical approaches that can be used to expose the nasal bones, but we present a case where the subciliary incision was used to reduce and fixate the fracture in a patient with combined zygomatic fractures. Owing to the abundance of other concom

Fractures of the fifth metatarsal are treated conceptually based on anatomic location and character of the fracture site. Intra-articular disruptions require reconstruction, if possible. Malalignment of acute fractures requires either closed reduction or open reduction if the malalignment represents a load-bearing dysfunction to the forefoot. Segmental defects require bone grafting and stabilization with plate and screws. Jones fracture is most effectively managed with medullary lag screw delivery in the active or athletic patient. Casting can be considered for high-risk patients. Late bone gr

We compared results of lateral pinning procedure with crossed pinning, closed reduction, and open reduction in a retrospective review of 184 patients with displaced supracondylar humeral fractures. All patients had a minimum of 2 years follow-up (range 36-90 months). Patients were separated into 4 groups. Success was estimated by Flynn's criteria. We compared success of the lateral pinning to others procedures. Incidence of nerve palsy was recorded and compared. Esthetic effect of lateral pinning is significantly better than closed reduction (p=0.0007), but no significant difference was found

Avulsion fractures of the calcaneal tuberosity are predominantly seen in patients with poor bone quality, the commonly used lag screw fixation might not be strong enough even with bony fragments of sufficient size. We present a case of a closed displaced avulsion fracture of the calcaneal tuberosity due to blunt trauma to the calf in a 74-year-old female. Open reduction and internal fixation with two 3.5-mm cannulated cortical screws with washers was performed, and anatomic reduction was achieved. Without further trauma, secondary displacement of the fracture occurred on day 3. Revision was pe
