Long-term outcomes of cochlear implantation in children with cochlear nerve deficiency: auditory speech performance and predictive factors over 5 years.

PURPOSE: While cochlear implantation (CI) is increasingly offered to children with cochlear nerve deficiency (CND), long-term outcomes and their predictors remain understudied. We evaluated 5-year auditory-speech performance and identified factors influencing outcomes. METHODS: A retrospective review of the UKM Cochlear Implant Program (2007-2024) included 12 children with CND (CI aged 14-52 months). Primary outcomes measured were unaided/aided hearing thresholds, Categories of Auditory Performance II (CAP-II), Speech Intelligibility Rating (SIR) scores preoperatively and at 1/3/5 years post-…

The (in)visibility of deafness: Identity, stigma, quality of life and the potential role of totally implantable cochlear implants.

INTRODUCTION: The disclosure of deafness is complex, given the historic and on-going stigma associated with being deaf. The aim of this study was to explore how identity, stigma, and quality of life may be impacted when using cochlear implants (CIs) and totally implantable cochlear implants (TICIs). The physical difference between these two assistive listening devices is significant, given many CI users opt to hide their sound processor behind hair or headwear, in contrast to TICIs (an emerging technology) whereby all components are implanted internally and thus invisible. METHODS: This quali

Effects of chronic implantation and long-term stimulation of a cochlear implant in the partial hearing cat model.

The expansion of criteria for cochlear implantation has resulted in increasing numbers of cochlear implant subjects having some level of residual hearing. The present study examined the effects of implantation surgery and long-term electrical stimulation on residual hearing in a partially deafened cat model. Eighteen animals were partially deafened, implanted and chronically stimulated. Implantation resulted in a pronounced loss evident 2-weeks post implantation of up to 30-40 dB at 4 & 8 kHz which was statistically significant (2-way RM ANOVA (Time, Frequency): p(Time) = 0.001; p(Frequency) <…

Inside implant criteria or not? - Detection of non-organic hearing loss during cochlear implant assessment.

OBJECTIVES: The diagnosis of non-organic hearing loss (NOHL) is a difficult but important issue during the assessment process for cochlear implantation (CI). We aim to identify the key factors in identifying patients with NOHL during CI assessment and present our local screening protocol for NOHL. METHODS: A retrospective review of patients referred to the Yorkshire Auditory Implant Service (YAIS) between 2003 and 2015 who were subsequently diagnosed with NOHL during the assessment. Patient demographic data, audiological and functional assessments were assessed. RESULTS: Thirty-two patients …

Cochlear implantation in children: labyrinthitis following pneumococcal otitis media in unimplanted and implanted cat cochleas.

Pneumococcal otitis media is frequent in young children and could lead to labyrinthitis post-implantation. To assess the risk, and methods o … e of fascia and Gelfoam to graft the round window entry point. As a result, the data indicates that cochlear implantation does not increase the risk of labyrinthitis following pneumococcal otitis media, but it is desirable to use fascia as a graft to the round window around the electrode entry point.

Hearing outcomes of cochlear implantation in IP-II (Mondini dysplasia) - A systematic review.

OBJECTIVES: Incomplete Partition type II (IP-II), or Mondini dysplasia, accounts for half of congenital cochlear malformations and results in profound sensorineural hearing loss. Cochlear implantation restores hearing in patients with IP-II however low numbers mean reporting of auditory performance outcomes is limited. Here we review English language articles between 2010 and 2025 reporting hearing outcomes following cochlear implantation in individuals IP-II. RESULTS: Categories of Auditory Performance (CAP), Speech Intelligibility Rating (SIR) and parental reports of cochlear implantation i…

Shape of the hemodynamic response function in deaf infants eligible for cochlear implantation.

SIGNIFICANCE: Deaf and hard-of-hearing infants' hemodynamic response function (HRF) has not yet been characterized. However, without an appropriate estimate of their HRF, neuroimaging modalities relying on hemodynamic responses, e.g., functional magnetic resonance imaging (fMRI) or functional near-infrared spectroscopy (fNIRS), cannot be used reliably in this population, e.g., prior to and following cochlear implantation. We contribute to better theoretical models of and more suitable therapeutic interventions for the neural changes induced by auditory and language deprivation in deaf infants.…

Cochlear Implantation in Sickle Cell Disease: A Systematic Review of Feasibility and Outcomes.

INTRODUCTION: Sickle cell disease (SCD) is associated with systemic complications, including sensorineural hearing loss (SNHL) from microvascular occlusion and chronic inflammation. Although reports link SCD to higher rates of SNHL, current evidence is limited by small sample size, varied audiologic methods, and lack of standardized screening. This systematic review summarizes available literature on SNHL and cochlear implantation (CI) in SCD. METHODS: A literature search of PubMed MEDLINE, Embase, Scopus, Web of Science, and CINAHL identified 79 citations. After removal of duplicates, 35 rec…

Various Surgical Techniques for Cochlear Implantation in an Ossified Cochlea: A Systematic Review.

BACKGROUND: Cochlear ossification, otherwise known as labyrinthitis ossificans, is not an uncommon encounter in candidates for cochlear implantation (CI) that may necessitate alternative surgical approaches due to the insertion difficulties. We aimed to review the literature for the documented approaches for CI in cases with cochlear ossification. METHODS: This review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines on articles that described their surgical approach for CI in cases with cochlear ossification. A comprehensive…

Outcomes of Cochlear Implantation in Mumps-Induced Single-Sided Deafness: A Retrospective Analysis.

BACKGROUND Mumps, a viral infection from the Paramyxoviridae family, is a rare but significant cause of sensorineural hearing loss (SNHL), especially in children. While typically unilateral, mumps-induced SNHL can result in profound single-sided deafness, for which cochlear implantation may be considered when hearing aids are ineffective. This study aimed to evaluate outcomes of cochlear implantation in patients with single-sided deafness caused by mumps. MATERIAL AND METHODS This retrospective study included 14 patients (10 women, 4 men; mean age 39.4) with single-sided deafness caused by mum…

Cochlear implantation in a patient with hydrocephalus and ipsilateral non-programmable ventriculoperitoneal shunt.

Cochlear implantation (CI) in patients with ventriculoperitoneal (VP) shunts poses unique challenges, including the risk of shunt infection, shunt displacement and modifications in implant placement. A female patient in her late 20s presented with the history of left cerebellopontine angle epidermoid tumour which has been excised along with sacrifice of the cochlear nerve followed by VP shunt placement on the contralateral side for hydrocephalus. Post shunt placement, the patient developed profound sensorineural hearing loss on the right side. CI was successfully performed on the side with the…

Long-term outcomes of cochlear implantation in adolescents with early-onset hearing loss.

OBJECTIVE: Neonatal hearing screening (NHS) has long-term beneficial effects on the outcomes of cochlear implantation (CI); however, some adolescents still require CI evaluation because of progressive hearing loss. This study aimed to investigate long-term postoperative effects and to identify prognostic factors in adolescents with CI. METHODS: Thirty-four patients with pre- or perilingual severe-to-profound hearing loss who underwent CI after the age of 10 years (adolescent group) were included. The pre- and postoperative outcomes of this group were compared with those of 138 patients with p…
