Data quality in Brazilian population-based cancer registries for gastrointestinal cancers.

BACKGROUND: Population-based cancer registries (PBCRs) are the primary source of information for cancer surveillance and monitoring. Currently, there are 30 active PBCRs in Brazil. The objective of this study was to analyze the data quality of five gastrointestinal cancers (esophagus, stomach, colorectal, liver, and pancreas) according to the criteria of comparability, validity, completeness, and timeliness in Brazilian cancer registries. METHODS: This study included data from Brazilian PBCRs with more than ten years of historical data starting in the year 2000, regardless of the type of defi

Risk of solid subsequent malignant neoplasms after childhood Hodgkin lymphoma-Identification of high-risk populations to guide surveillance: A report from the Late Effects Study Group.

BACKGROUND: Survivors of Hodgkin lymphoma (HL) in childhood have an increased risk of subsequent malignant neoplasms (SMNs). Herein, the authors extended the follow-up of a previously reported Late Effects Study Group cohort and identified patients at highest risk for SMNs to create evidence for risk-based screening recommendations. METHODS: The standardized incidence ratio was calculated using rates from the Surveillance, Epidemiology, and End Results program as a reference. The risk of SMN was estimated using proportional subdistribution hazards regression. The cohort included 1136 patients

Prior appendectomy does not protect against subsequent development of malignant or borderline mucinous ovarian neoplasms.

BACKGROUND: Due to concern that mucinous malignant or borderline ovarian neoplasms (MON) may represent metastatic deposits from appendiceal primaries, gynecologic oncologists routinely perform appendectomy in these cases. However, a multidisciplinary critique of this practice is lacking. METHODS: The New England Case-Control study database was utilized to compare the effect of prior appendectomy against known risk factors for MON. Pathology and operative reports of local cases of MON were reviewed to estimate the frequency of microscopic mucinous lesions in the appendix. Protein expression pa

A missed opportunity for preventing CMMRD: is it time to include Lynch syndrome genes in prenatal genetic testing?

This manuscript highlights the importance of incorporating Lynch syndrome-related genes into prenatal genetic testing to facilitate early detection of constitutional mismatch repair deficiency syndrome (CMMRD). CMMRD, linked to biallelic pathogenic variants in mismatch repair genes, is an aggressive hereditary cancer syndrome. A couple conceived through in vitro fertilisation (IVF), unaware they were carriers of Lynch syndrome. Their child, initially healthy, was diagnosed with acute bilineal leukaemia and later found to have CMMRD. His diagnosis highlighted gaps in prenatal testing. Advances

Lessons learnt from the implementation of a colorectal cancer screening programme for lynch syndrome in a tertiary public hospital.

BACKGROUND: Lynch syndrome (LS) is the first cause of inherited colorectal cancer (CRC), being responsible for 2-4% of all diagnoses. Identification of affected individuals is important as they have an increased lifetime risk of multiple CRC and other neoplasms, however, LS is consistently underdiagnosed at the population level. We aimed to evaluate the yield of LS screening in CRC in a single-referral centre and to identify the barriers to its effective implementation. METHODS: LS screening programme included individuals with CRC < 70 years, multiple CRC, or endometrial cancer at any age. Mi

Pathology and genetics of hereditary colorectal cancer.

Colorectal cancer (CRC) accounts for over 8% of all deaths annually worldwide. Between 2 and 5% of all CRCs occur due to inherited syndromes, including Lynch syndrome, familial adenomatous polyposis, MUTYH-associated polyposis, Peutz-Jeghers syndrome, juvenile polyposis and Cowden/PTEN hamartoma syndrome. In addition, serrated polyposis is a clinically defined condition characterised by multiple colorectal serrated polyps and an increased risk of CRC but the genetics are not known. In most hereditary CRC syndromes, polyps undergo carcinogenesis, but the exact route to carcinoma seems to differ

Nonlinear Association Between Subcutaneous Fat Area and All-Cause Mortality in Postoperative Patients with Stage I-III Colorectal Cancer: A Multicenter Retrospective Study.

BACKGROUND: Colorectal cancer (CRC) represents a significant global health burden as one of the most prevalent malignancies of the gastrointestinal tract. Recent research has increasingly focused on the prognostic implications of host-related factors, particularly body composition. Among these, subcutaneous adipose tissue (SAT) area, a critical biomarker reflecting nutritional status and metabolic homeostasis, has shown inconsistent associations with CRC outcomes, suggesting a potential non-linear relationship. This investigation aims to elucidate the non-linear correlation and identify potent

Shifting paradigms in 30,427 surgical colorectal cancer cases (2008-2023): the impact of endoscopic screening on caseload complexity and the value of minimally invasive quality.

BACKGROUND: While global registries report a rising incidence of early-onset colorectal cancer (EOCRC), the impact of widespread screening and endoscopic therapeutics on the case-mix of major surgical centers remains undercharacterized. We aimed to quantify the 16-year evolution of surgical demographics, pathological profiles, and quality metrics to delineate how modern practice reshapes surgical volume and outcomes. METHODS: We conducted a retrospective analysis of 30,427 consecutive patients undergoing colorectal cancer (CRC) resection at Fudan University Shanghai Cancer Center from 2008 to

[Surgical Management of Hereditary Colorectal Cancer Syndromes].

Colorectal cancer (CRC) is among the most common malignancies worldwide. Approximately 10% of all CRCs are caused by monogenic hereditary tumour syndromes, collectively referred to as hereditary colorectal cancers (hCRC). The increasing use of molecular diagnostics - such as microsatellite instability (MSI) testing - is expected to significantly raise the detection rate in the coming years. hCRC can be broadly divided into polyposis syndromes (e.g. FAP, MAP) and non-polyposis syndromes (primarily Lynch syndrome). Surgical management must be tailored to the specific syndrome, and must balance o

Special features of sarcomas developed in patients with Lynch syndrome: A systematic review.

Lynch syndrome (LS) is a genetic predisposition leading to colorectal and non-colorectal tumors such as endometrial, upper urinary tract, small intestine, ovarian, gastric, biliary duct cancers and glioblastoma. Though not classically associated with LS, growing literature suggests that sarcomas might develop in patients with LS. This systematic review of literature identified 44 studies (N = 95) of LS patients who developed sarcomas. It seems that most sarcomas developed in patients with a germline mutation of MSH2 (57 %) exhibit a dMMR (81 %) or MSI (77 %) phenotype, as in other LS-tumors. A

Hodgkin Lymphoma in a Case of Chronic Myeloid Leukemia Treated with Tyrosine Kinase Inhibitors.

Chronic myeloid leukemia (CML) is characterized by increased and unregulated proliferation of granulocytic lineage in the bone marrow and presence of these immature myeloid cells in the peripheral blood with presence of Philadelphia (Ph) chromosome. Tyrosine kinase inhibitors are the most important drugs in the CML therapy and provide long disease-free survival. Due to the increased survival of CML patients with continual administration of these drugs, the chance of development of secondary malignancies may increase. The most common secondary malignancies are prostate, colorectal and lung canc

Evaluation of a High-risk Patient Reminder System for Colonoscopy Surveillance.

Objectives: In this paper we report the evaluation of a proprietary recall system for promoting compliance with recommended follow-up in high-risk patients. Methods: We conducted a retrospective chart review for patients of an active private colon and rectal surgery practice having colonoscopy in 2006. Patients selected were <80 years of age and assessed to be high-risk by findings at exam or personal/family history of colorectal neoplasm with a recommendation for follow-up surveillance colonoscopy ranging from months to 5 years. Up to 6 months from recommendation was considered to be within c

A Diagnostic Pitfall: Retrocaval Nerve Sheath Tumor.

A 78-year-old female with a history of resected colorectal adenocarcinoma presented with progressively rising serum carcinoembryonic antigen (CEA) levels since 2022. An August 2023Positron Emission Tomography/Computed Tomography (PET-CT) revealed a 1.4 cm fluorodeoxyglucose (FDG)-avid retrocaval lesion, initially raising suspicion for metastatic lymphadenopathy given her cancer history. Despite receiving stereotactic body radiotherapy and remaining asymptomatic, her CEA levels continued to climb, and the lesion mildly enlarged to 1.8 cm by November 2024. This persistent elevation and lesion gr

Mucosa-associated lymphoid tissue (MALT) lymphoma of the rectum with chromosomal translocation of the t(11;18)(q21;q21) and an additional aberration of trisomy 3.

A rare case of primary mucosa-associated lymphoid tissue lymphoma (MALT) of the rectum is reported. A 56-yr-old man was referred to our hospital for further examination and treatment of rectal neoplasm. A physical examination and laboratory data showed no special abnormalities. However, endoscopic colorectal observation revealed multiple red and slightly elevated nodular lesions with erosive changes of the rectum. The lesions were composed of diffuse, small atypical lymphoid cells (i.e., centrocyte-like cells) and were stained with L26 and BCL-2 but not cyclin D1. Surface markers of cells obta

Ulcerative colitis with overexpression of p53 preceding overt histological abnormalities of the epithelium.

A 53-year-old man with a 22-year history of ulcerative colitis(UC) (pancolitis), had an ulcerating rectal tumor. Resection of the rectum and sigmoid colon was performed. Pathology showed an expansive ulcerating adenocarcinoma tumor (type 2) invading the adventitia against a background of UC in a resolving phase. Dysplasia was also found in granular and flat mucosa adjacent to the invading carcinoma. Immunostaining for p53 showed diffuse positivity in both the carcinoma and dysplasia, and also in the mucosa with indefinite dysplasia or no dysplasia neighboring the dysplasia and carcinoma. Mappi

Prevalence of KDM6A deficiency in human cancer: A tissue microarray study on 18,570 cancers from 153 different tumor types.

KDM6A is a critical part of the COMPASS-like complex. KDM6A deficiency may result in EZH2 dependency. KDM6A deficiency was analyzed by immunohistochemistry on a tissue microarray containing 14,814 samples from 153 different tumor entities and 76 different normal tissue types. In normal tissues, KDM6A staining was ubiquitously seen in nuclei. In tumors, KDM6A deficiency occurred in 58 of 153 tumor categories. KDM6A deficiency predominated in urothelial carcinomas (17.3-42.0%) and was seen in 4-10% of gastric, pancreatic, prostatic, and gallbladder adenocarcinomas, serous endometrial carcinomas,

Germline Mutations in DNA Repair Genes in Patients with Pancreatic Neuroendocrine Neoplasms: Diagnostic and Therapeutic Implications.

Pancreatic neuroendocrine neoplasms (pNENs) are the second most common type of pancreatic cancer after pancreatic ductal adenocarcinoma. Germline mutations in DNA repair genes drive several hereditary and sporadic cancers; however, their role in pNENs remains poorly defined. This pilot study aimed to assess the frequency and clinical relevance of germline DNA repair gene mutations in patients with pNENs, both with and without a family history of cancer. Germline DNA from 57 Polish patients with pNENs was analyzed using targeted next-generation sequencing to identify variants in a panel of DNA
