Emotional illness in a paediatric population in Nigeria.

In a survey of 500 children aged between 5-15 years in a small rural community in Nigeria, 15.0% were found to suffer from mental morbidity. Disturbances of emotion and conduct disorder constituted 66.7% of total morbidity detected. Children from disrupted families (due to divorce, separation and widowhood) were found to be significantly more likely to suffer from psychiatric morbidity. The need to train primary health care worker and for them to work closely with families and school teachers in the provision of community mental health care services for children in the rural communities of dev

Health concerns of migrant backstretch workers at horse racetracks.

BACKGROUND: Backstretch workers are part of an itinerant labor community that cares for horses at racetracks across the U.S. Since the 1970s, this workforce has increasingly been composed of migrants, especially from Mexico and Guatemala. No studies have focused systematic attention on health concerns of this population and how illness is influenced by unique working and living conditions. METHODS: We administered a survey orally to 84 backstretch workers at a Florida racetrack to understand self-reported medical history and current major health concerns. RESULTS: The most frequent responses

Implementation of an efficacious intervention for high risk women in Mexico: protocol for a multi-site randomized trial with a parallel study of organizational factors.

BACKGROUND: Studies of implementation of efficacious human immunodeficiency virus (HIV) prevention interventions are rare, especially in resource-poor settings, but important, because they have the potential to increase the impact of interventions by improving uptake and sustainability. Few studies have focused on provider and organizational factors that may influence uptake and fidelity to core intervention components. Using a hybrid design, we will study the implementation of an efficacious intervention to reduce sexually transmitted infections (STIs) among female sex workers (FSWs) in 12 ci

Task shifting of point-of-care testing to lay health workers in rural primary health facilities in Nsanje, Malawi.

BACKGROUND: Point-of-care testing (POCT) can reduce diagnostic gaps, but because it frequently falls to overstretched frontline healthcare workers (HCW), its real-world effectiveness is diminished. Despite World Health Organization recommendations for task sharing (TS) with lay health workers (LHW), it remains underutilized. OBJECTIVE: To explore whether task-shifted POCT by LHWs can improve uptake compared to standard HCW-led care. METHODS: A retrospective study at 5 PHC facilities in Nsanje, Malawi, compared a pre-intervention (model 1) period where HCW conducted POCT (for rapid HIV, AHD,

Community education training to optimize the use of artemisinin-based combination therapy in Kamuli District, Uganda.

BACKGROUND: Community health education improves members health-seeking and utilization behaviours. To enhance the community knowledge and optimize the use of Artemisinin-based combination therapy (ACT), we carried out a community training in Kamuli District, Uganda. METHODS: The Analysis, Design, Development, Implementation and Evaluation (ADDIE) model was adopted. A total of 3420 community members were trained, 384 sampled to participate in pre-post-test assessment, with 76 healthcare workers (HCW). Community members were sampled by simple random sampling while the HCW were purposively selec

Prevention of postpartum hemorrhage at home birth in Afghanistan.

OBJECTIVE: To test the safety, acceptability, feasibility, and effectiveness of community-based education and distribution of misoprostol for prevention of postpartum hemorrhage at home birth in Afghanistan. METHODS: A nonrandomized experimental control design in rural Afghanistan. RESULTS: A total of 3187 women participated: 2039 in the intervention group and 1148 in the control group. Of the 1421 women in the intervention group who took misoprostol, 100% correctly took it after birth, including 20 women with twin pregnancies. Adverse effect rates were unexpectedly lower in the intervention

Developing a community-based neonatal care intervention: a health facility assessment to inform intervention design.

Community-based interventions are an important way of improving health in low-income countries. A necessary prerequisite for the design of such interventions is an understanding of the local health system. This will inform intervention design, help ensure the community-intervention forms part of a continuum of care, and provide information about health system strengthening activities that may be necessary for success. Such formative research processes, however, are seldom reported in the literature. We present the results of a health facility assessment used in the design stage of Newhints, a

Evaluation of the North Country on the Job Network: a model of facilitated care for injured workers in rural upstate New York.

We describe the evaluation of a community-based program designed to facilitate access to care and return to work for injured workers in a rural, medically underserved area in upstate New York. Providers are recruited to provide easily accessible care and are oriented to concepts of transitional duty and rapid return to work as medically appropriate; companies are recruited with the agreement to provide transitional work for injured employees. Registered nurses, hired by the local hospital, serve as case coordinators to facilitate care and coordinate communications among all parties. Over 3000

Occupational heat stress and adaptation among outdoor workers: a narrative review of global evidence and policy responses.

Occupational heat stress is an escalating global health concern, particularly for outdoor workers exposed to rising temperatures due to climate change. This narrative review synthesizes evidence from 42 peer-reviewed articles and 13 policy documents published between 2014 and 2025, focusing on the health impacts, determinants, and adaptation strategies for outdoor workers. The review identifies a high prevalence of heat-related illnesses-including heat exhaustion, dehydration, kidney disorders, and productivity loss-across sectors such as agriculture, construction, and informal labour. Key vul

Measles, mumps, rubella, and varicella immunization status of first responders--a policy proposal.

The responsibility of occupational health nursing often extends to the communities that workers serve. This is especially true for first responders (i.e., firefighters, paramedics, and emergency medical technicians) and their measles, mumps, rubella, and varicella status.

Redefining Worker Protection in the Algorithmic Era: From Algorithmic Justice to Insurance Systems for the Gig Economy.

The rapid expansion of the digital platform economy has introduced novel occupational hazards driven by algorithmic management. This short communication examines the occupational health challenges faced by gig workers in the context of algorithmic control, focusing on four key mechanisms: trade-offs between economic success and worker health, uncertainty in terms of effort and reward, unreliable assessment of job performance, and incorrect attribution of responsibility. These algorithmic hazards differ substantially from conventional occupational risks and cannot be adequately addressed by exi

Using a harm reduction lens to examine post-intervention results of medical abortion training among Zambian pharmacists.

Despite broad grounds for legal abortion in Zambia, access to abortion services remains limited. Pharmacy workers, a primary source of health care for communities, present an opportunity to bridge the gap between policy and practice. As part of a larger operations study, 80 pharmacy workers, both registered pharmacists and their assistants, participated in a training on medical abortion in 2009 and 2010. Fifty-five of the 80 pharmacy workers completed an anonymous, structured training pre-test, treated as a baseline questionnaire; 53 of the 80 trainees were interviewed 12-24 months post-traini

Systems, supplies, and staff: a mixed-methods study of health care workers' experiences and health facility preparedness during a large national cholera outbreak, Kenya 2015.

BACKGROUND: From December 2014 to September 2016, a cholera outbreak in Kenya, the largest since 2010, caused 16,840 reported cases and 256 deaths. The outbreak affected 30 of Kenya's 47 counties and occurred shortly after the decentralization of many healthcare services to the county level. This mixed-methods study, conducted June-July 2015, assessed cholera preparedness in Homa Bay, Nairobi, and Mombasa counties and explored clinic- and community-based health care workers' (HCW) experiences during outbreak response. METHODS: Counties were selected based on cumulative cholera burden and geog

Walking beyond our borders with frontline health workers in Guatemala.

In many developing countries, access to health care can be out of reach for many women and newborns. Trained frontline health workers serve as the first point of contact for medical care in many of these areas, and their efforts can prevent illness and death from conditions such as malaria, pneumonia and diarrhea. Their work is especially vital to reducing maternal and newborn morbidity and mortality. Due to the global shortage of health workers, nurses and organizations need to collectively advocate for the support and training of frontline health workers.

An interim report of an intervention strategy for diabetes care in Jamaica.

The high prevalence (17.9%) of diabetes mellitus and its attendant costs have been recognized for some time. The diabetic public has hitherto been too oriented to seeking health care at the secondary and tertiary health care centres; a much too costly approach. As a part of a wellness promotion thrust as well as an awareness and intervention strategy, a number of health care agencies have collaborated in a training programme of lay diabetes facilitators. This was intended to allow a domino effect of "each one teach one" within the community. This intervention programme was undertaken over an 1

Traditional health practitioners as primary health care workers.

The author conducted a field study in 1993 to evaluate the effectiveness of four projects that were training traditional health practitioners (THPs) to provide primary health care (PHC) services in Ghana, Mexico, and Bangladesh. The study, funded by a grant from the World Health Organization, Division of Strengthening Health Services, concluded that incorporating trained THPs in PHC programmes can be cost effective in providing essential and culturally relevant health services to communities. The main objective of the study was to evaluate how effective the training projects were and to determ

Addressing Racial Capitalism's Impact on Black Essential Workers During the COVID-19 Pandemic: Policy Recommendations.

Black Americans are more likely to be essential workers due to racial capitalism. Because of the COVID-19 pandemic, essential workers are less able to adhere to social distancing and stay-at-home guidelines due to the nature of their work, because they are more likely to occupy crowded households, and are more likely to possess pre-existing health conditions. To assist Black essential workers in preventing infection or reducing the intensity of symptoms if contracted, vaccination against the virus is essential. Unfortunately, Black essential workers face considerable barriers to accessing vacc

Agrichemical safety practices on farms in the western Cape.

In order to study agrichemical safety practices in a rural farming area in the western Cape, an audit of 45 randomly sampled farms was performed over 3 months in 1992. A response rate of 87% was achieved, and the survey results suggest that approximately 9% of permanent and 14% of seasonal farm workers are employed in jobs with potential exposure to agrichemicals. While protective equipment was widely available, gloves and masks were seldom used, with little enforcement or commercial support from the suppliers of the equipment. Farm workers receive little training on pesticide safety, but inte
