[The Beals-Hecht syndrome (congenital contractural arachnodactyly) revealed in a neonate].

A new case of congenital contracture arachnodactyly (CCA) revealed in the neonatal period is reported. CCA is a dominantly inherited syndrome associating arachnodactyly, kyphoscoliosis, multiple congenital joint contractures and crumpled ears. This condition differs from Marfan's syndrome by the usual absence of visceral involvement, although cardiac complications are possible. The neonatal forms result from new mutations are are generally severe.

A recessive form of congenital contractures and torticollis associated with malignant hyperthermia.

Two families are presented, each with two affected sibs, all four of whom seem to have a newly described and specific form of congenital contractures (arthrogryposis). The affected subjects have congenital torticollis, dysmorphic, asymmetrical, myopathic facial features, and progressive scoliosis. Two sibs had cleft palate. Malignant hyperthermia has occurred in two of the patients.

[Diagnosis of susceptibility to malignant hyperthermia using the in vitro contracture test].

Though a malignant hyperthermia (MH) crisis is still a critical event during general anesthesia, recent developments in prophylaxis and treatment should help in avoiding fatal episodes. The best means to avoid MH episodes would be early recognition of MH susceptibility. Today the only reliable test to identify MH susceptibility is the in vitro contracture test. Thus, to diagnose MH susceptibility we performed this test on muscle biopsies from 26 individuals who: (1) had an event during general anesthesia that may have been indicative of MH (4 patients); (2) had a family member with a medical h…

Salvage of severe recurrent Dupuytren's contracture of the ring and small fingers.

Severe recurrent digital Dupuytren's contracture, with pseudomotor changes and inadequate palmar tissue, is often treated with amputation. In this study, 11 patients had 14 proximal interphalangeal joint fusions with shortening done for salvage of recurrent Dupuytren's rather than amputation. Indications for operation were pseudomotor changes, severe palmar fibrosis, previous operations, and a flexion contracture of greater than 70 degrees.

Fibrin/fibrinogen and fibrinolytic activity of the palmar fascia in Dupuytren's contracture.

Considering the proved interaction of fibrin with fibroblasts and the seemingly decisive role of structural and functional changes ("modulation") of these cells in the evolution of Dupuytren's contracture, research has been carried out in order to investigate the fibrinolytic capacity and the possible presence of fibrin/fibrinogen in the palmar fascia of subjects operated upon for Dupuytren's Disease. Fibrin/fibrinogen were detected by a direct immunofluorescence technique and fibrinolytic activity was assessed by a fibrin plate method. A remarkable decrease of fibrinolytic activity and the pr…

Disuse contractures in a patient with tinea manuum and irritant contact dermatitis.

Disuse contractures are reported in a patient with tinea manuum and irritant contact dermatitis. The case is presented to alert the physician to the potential for this problem in any patient with a chronic fissured dermatosis of the hands.

[New aspects of the treatment of dermatogenous contractures of the hand in epidermolysis bullosa].

Epidermolysis bullosa dystrophica is a rare hereditary disease of skin. There are always flexion contractures and pseudosyndactylies of all fingers and adduction contractures of the thumb. Aims of surgical management are reconstruction and maintaining of hand function. Technical details of 18 operations in six children are described. Ultramicroscopical criteria help to choose patients for therapy with phenytoin. It is as yet too early to assess the results of the latter treatment.

Part I. Amyoplasia: a common, sporadic condition with congenital contractures.

A specific congenital contracture (arthrogryposis) syndrome has been recognized in 135 out of 350 patients with various kinds of congenital contractures. This sporadic syndrome, designated amyoplasia is characterized by absence of limb muscles that are replaced by fibrous and fatty tissue. At birth there is characteristic positioning of the limbs with internal rotation at shoulders, extension at elbows, and flexion of the hands at the wrists. Severe equinovarus deformities of the feet are usually present. Contractures at knees and hips occur in a variety of positions. Typically, the face is ro…

Open capsulotomy for capsular contracture: a new procedure for the prevention of recurrence.

Capsular contracture is a frequent complication after breast augmentation with alloplastic implants. A technique has been developed to correct this contracture, performed through the previous operative scar. Use of a portion of the matured capsule wall as petal flaps appears to be a contributing factor in the success of this procedure. Open capsulotomy has been performed on 23 patients with good results in 20. The procedure should be performed at least 6 months after augmentation mammoplasty has been performed.
