Quantitative evaluation of coronary perfusion in man.

The usefulness and limitations of currently available techniques for quantitating coronary flow in ischemic heart disease are summarized. There are appreciable difficulties in assessing coronary flow solely from an arteriographic evaluation of the epicardial arteries. There is a considerable reserve mechanism for vasodilation at the arteriolar level, and a proximal occlusive lesion produces a reduction in flow only after this distal reserve has been exhausted. In addition, small increments in the severity of established lesions sometimes cause profound reductions in flow. The development of cl

Sudden Cardiac Arrest as Initial Presentation of Familial Combined Hyperlipidemia in a Young Adult.

BACKGROUND: Familial combined hyperlipidemia (FCHL) affects 0.5% to 2% of the population and is a significant risk factor for premature myocardial infarction, accounting for up to 16% of cases. CASE SUMMARY: A 46-year-old man experienced cardiac arrest while surfing. Emergency services found him pulseless with ventricular fibrillation requiring 2 defibrillations and 10 minutes of resuscitation before return of spontaneous circulation. Bloodwork revealed severe dyslipidemia (low-density lipoprotein cholesterol 358 mg/dL, triglycerides 1,622 mg/dL) consistent with FCHL. Coronary angiography dem

Myocardial ischemia: current concepts and future perspectives.

Ischemic heart disease is the leading cause of morbidity and mortality in a worldwide epidemic. Myocardial ischemia is characterized by an imbalance between myocardial oxygen supply and demand, causing cardiac dysfunction, arrhythmias, myocardial infarction, and sudden death. Various clinical ischemic manifestations are caused by obstruction of coronary blood flow by coronary plaques, thrombosis, and/or hyperconstriction/vasospasm of epicardial and microvascular coronary arteries, in which gender difference also is involved due in part to estrogen hormonal state. The coronary circulation match…

Angiogenic biomarker concentrations and their relation to coronary atherosclerosis in women with a history of preeclampsia: Findings from the CPH-PRECIOUS study.

INTRODUCTION: Preeclampsia is known to be associated with a two- to fourfold elevated risk of developing cardiovascular disease in the first 15 years following pregnancy. In this cross-sectional study of 892 women with prior preeclampsia, we aimed to assess differences in circulating angiogenic biomarker concentrations according to CT-assessed coronary atherosclerosis in women up to 28 years after an index pregnancy complicated by preeclampsia. MATERIAL AND METHODS: Women were identified from clinical databases consisting of data from all deliveries within the Capital Region of Denmark from 1

Coronary Artery Anomaly With Absent Common Iliac Artery and Genitourinary Malformation: A Rare Case Report.

Congenital coronary artery anomalies are rare, and while often asymptomatic, some anomalies can pose life-threatening risks. Peripheral vascular anomalies, particularly involving the iliac arteries, are even rarer and may go undetected unless complications arise. Their presence can significantly impact interventional and surgical strategies, particularly when extracorporeal circulatory support is required during cardiac surgery. We report a rare case of a 37-year-old male presenting with acute coronary syndrome and an anomalous left circumflex coronary artery originating from the right coronar

Multiple coronary artery fistulas with a huge right coronary artery showing exacerbation during 16 years of follow-up.

A 38-year-old Japanese woman underwent cardiac evaluation to assess an arrhythmia detected after the delivery of her fourth baby. At the age of 22 years, she had undergone cardiac evaluation, including catheter examination, because of a heart murmur. Coronary angiography showed a huge right coronary artery (RCA) draining to the right atrium and a normal left coronary artery. Because the left to right shunt ratio was trivial and the patient was asymptomatic, conservative observation was undertaken. On the current admission to hospital, there was a grade III continuous murmur at the second left

Isolated reversal of flow in the vertebral artery - A missed entity.

Isolated reversal of flow in the vertebral artery is a rare phenomenon occurring in the general population and is due to intrinsic anatomical defects. The most common cause is subclavian stenosis; however, reversal of flow in the vertebral artery can occur regardless of any detection of anatomic deterioration. In this series, we report three asymptomatic cases where the extracranial Doppler scan preceding off-pump coronary arterial bypass grafting showed isolated reversal of flow in the vertebral artery. No signs of subclavian stenosis or steal syndrome were elicited. However, all patients sub

Off-pump versus on-pump coronary artery bypass grafting.

Coronary artery bypass grafting (CABG) remains the preferred treatment in patients with complex coronary artery disease. However, whether the procedure should be performed with or without the use of cardiopulmonary bypass, referred to as off-pump and on-pump CABG, is still up for debate. Intuitively, avoidance of cardiopulmonary bypass seems beneficial as the systemic inflammatory response from extracorporeal circulation is omitted, but no single randomized trial has been able to prove off-pump CABG superior to on-pump CABG as regards the hard outcomes death, stroke or myocardial infarction. A long-term survivor of Bland-White-Garland syndrome with systemic collateral supply: a case report and review of the literature.

BACKGROUND: Bland-White-Garland syndrome (anomalous origin of the left coronary artery from the pulmonary artery) is a rare disease which may result in myocardial infarction, congestive heart failure and sometimes death during the early infantile period. CASE PRESENTATION: A successfully treated case of a 45-year-old mother of 2 children with Bland-White-Garland syndrome and concomitant severe mitral regurgitation is presented. Subsequent therapy consisted of ligation of the anomalous origin of the left coronary artery, anastomosis of the left internal mammary artery to the left anterior desc

Paradoxic pulmonary vasoconstriction in response to acetylcholine in patients with primary pulmonary hypertension.

Pulmonary vascular reactivity was assessed during diagnostic heart catheterization in two patients with pulmonary hypertension unexplained by pulmonary or cardiac disease and in five patients with atypical chest pain and normal coronary arteriograms. Acetylcholine, an endothelium-dependent vasodilator that also has a direct contracting effect on vascular smooth muscle cells, was infused in the right atrium in a step-wise increasing dose in order to obtain final blood concentrations in the pulmonary circulation ranging from 10(-6) mol/L to 10(-4) mol/L. In the five control patients, acetylcholi

[The influence of arterial partial pressure of co2 and of the arterial value of ph on endogenous catecholamines during extracorporal circulation in regard to oxygen consumption of the whole body (author's transl)].

Ten patients with coronary heart diseases were studied during total hypothermic extracorporal circulation relating changes of the endogenous catecholamines epinephrine and nor-epinephrine to arterial partial pressure of CO2 and to the arterial value of pH. Both of the catecholamines increased significantly during respiratory or metabolic acidosis whereas respiratory or metabolic alkalosis was accompanied by significantly lower catecholamine levels in blood. The changes in total peripheral resistance due to the endogenous blood levels of catecholamines depended proprotionally. Total peripheral

Coronary leukocyte activation in relation to progression of coronary artery disease.

Leukocyte activation has been linked to atherogenesis, but there is little in vivo evidence for its role in the progression of atherosclerosis. We evaluated the predictive value for progression of coronary artery disease (CAD) of leukocyte activation markers in the coronary circulation. Monocyte and neutrophil CD11b, neutrophil CD66b expression and intracellular neutrophil myeloperoxidase (MPO) in the coronary arteries were determined by flow cytometry in patients undergoing coronary angiography. The primary outcome included fatal and nonfatal myocardial infarction or arterial vascular interve…

Dobutamine stress echocardiography: prevalence of a nonischemic response in a low-risk population.

The problems of population referral bias in the calculation of specificity in diagnostic testing for coronary artery disease have been previously described. Previous studies investigating the sensitivity and specificity of dobutamine stress echocardiography (DSE) have been subject to pretest and posttest referral biases, largely as a result of the requirement for coronary arteriography. This study determines the normalcy rate for DSE by examining a population at statistically low risk for coronary artery disease. The probability of significant coronary artery disease was determined for 828 con

[Cerebral contrast medium extravasation after coronary angioplasty].

We report on an 82-year-old woman who suffered a sudden loss of consciousness, right-sided hemiparesis, and aphasia during a coronary angiographic procedure. Computed tomography (CT) of the brain performed immediately revealed an edema and extravascularly localized contrast media in the left hemisphere. Within 6 h, neurological symptoms had disappeared, and a second CT after 48 h revealed normal results. Usually, extravasation of contrast media affects the posterior circulation with cortical blindness. This case demonstrates that contrast media may affect the blood-brain barrier also outside t

Analyzing the role of ATP-dependent potassium channels in the regulation of coronary metabolic vasodilation during exercise.

The purpose of this meta-analysis is to compile, integrate, and assess studies that have investigated the role of ATP-dependent potassium channels (KATP channels) in coronary metabolic vasodilation in vivo. KATP channels are important modulators of membrane potential and vascular reactivity and are commonly proposed as regulating coronary blood flow in response to myocardial metabolism. For example, the electro-metabolic hypothesis of Lederer and colleagues (Zhao et al., Proc Natl Acad Sci 117:7461-70, 2020) suggests a pathway where myocardial metabolic activity reduces intracellular ATP, thus

Diseases of the Coronary Microcirculation: Diagnosis and Treatment.

BACKGROUND: Coronary microvascular dysfunction (CMD) comprises a variety of pathogenic mechanisms that impair the microcirculation of the heart. Clinical studies have shown that 30-50% of patients suffering from myocardial ischemia without significant coronary artery stenosis have CMD. The disease is associated with ele - vated mortality and poor quality of life. Whenever a patient presents with symptoms of angina pectoris and no underlying disease is detected by the usual methods, CMD should be considered a possible cause. METHODS: This review is based on publications retrieved by a selectiv

Comparison of Doppler Flow Velocity and Thermodilution Derived Indexes of Coronary Physiology.

OBJECTIVES: The aim of this study was to compare Doppler flow velocity and thermodilution-derived indexes and to determine the optimal thermodilution-based diagnostic thresholds for coronary flow reserve (CFR). BACKGROUND: The majority of clinical data and diagnostic thresholds for flow-based indexes are derived from Doppler measurements, and correspondence with thermodilution-derived indices remain unclear. METHODS: An international multicenter registry was conducted among patients who had coronary flow measurements using both Doppler and thermodilution techniques in the same vessel and dur
