Screening for Lynch syndrome in young Saudi colorectal cancer patients using microsatellite instability testing and next generation sequencing.

Individuals with Lynch syndrome (LS) have germline variants in DNA mismatch repair (MMR) genes that confer a greatly increased risk of colorectal cancer (CRC), often at a young age. Identification of these individuals has been shown to increase their survival through improved surveillance. We previously identified 33 high risk cases for LS in the Saudi population by screening for microsatellite instability (MSI) in the tumor DNA of 284 young CRC patients. The aim of the present study was to identify MMR gene variants in this cohort of patients. Peripheral blood DNA was obtained from 13 individ…

Immunohistochemical testing of conventional adenomas for loss of expression of mismatch repair proteins in Lynch syndrome mutation carriers: a case series from the Australasian site of the colon cancer family registry.

Debate continues as to the usefulness of assessing adenomas for loss of mismatch repair protein expression to identify individuals with suspected Lynch syndrome. We tested 109 polyps from 69 proven mutation carriers (35 females and 34 males) belonging to 49 Lynch syndrome families. All polyps were tested by immunohistochemistry for four mismatch repair proteins MLH1, MSH2, MSH6 and PMS2. Detailed pathology review was performed by specialist gastrointestinal pathologists. The majority of polyps (86%) were conventional adenomas (n=94), with 65 tubular and 28 tubulovillous adenomas and a single v

Mutations of the 'minor' mismatch repair gene MSH6 in typical and atypical hereditary nonpolyposis colorectal cancer.

Mutations of the mismatch repair (MMR) genes MLH1 and MSH2 are associated with hereditary nonpolyposis colorectal cancer (HNPCC), a highly penetrant autosomal dominant condition characterized by hypermutability of short tandemly repeated sequences in tumor DNA. Mutations of another MMR gene, MSH6, seem to be less common than MLH1 and MSH2 defects, and have been mostly observed in atypical HNPCC families, characterized by a weaker tumor family history, higher age at disease onset, and low degrees of microsatellite instability (MSI), predominantly involving mononucleotide runs. We have investiga

Comparative Study of DNA ploidy and BRAF Immunohistochemistry between Colonic Adenocarcinoma and Inflammatory Colonic Lesions.

OBJECTIVES: To evaluate DNA ploidy and S-phase fraction (SPF) in non-Lynch colonic adenocarcinoma, ulcerative colitis (UC), Crohn disease (CD) which are known as risk factors, and colitis. We correlated ploidy and SPF with tumor grading, staging and BRAF expression. METHODS: All studied adenocarcinomas have intact mismatch repair genes as proved by immunohistochemistry. All were assessed for ploidy by automated image-based DNA cytometry and histograms were drawn. Immunostaining by anti-BRAF V600E was performed. Diagnostic laparoscopy (DL) was done as a preliminary step for staging GI cancers.

Integrated Molecular and Clinical Analysis of Thymic Epithelial Tumors.

PURPOSE: Thymic epithelial tumors (TETs), which include thymomas and thymic carcinomas (TCs), are the most common neoplasms of the anterior mediastinum. To develop more effective therapeutics, it is necessary to better understand their molecular characteristics. METHODS: TET samples were profiled using next-generation sequencing (NGS) of DNA (592-gene panel or whole-exome sequencing) and RNA (whole-transcriptome sequencing). Pathogenic single nucleotide variations/indels, deficient mismatch repair/microsatellite instability (dMMR/MSI) status (via immunohistochemistry and NGS), PD-L1 expressio

DNA double-strand breaks by Cr(VI) are targeted to euchromatin and cause ATR-dependent phosphorylation of histone H2AX and its ubiquitination.

Hexavalent chromium is a human respiratory carcinogen that undergoes intracellular activation in vivo primarily via reduction with ascorbate. Replication of Cr-adducted DNA triggers mismatch repair that generates toxic DNA double-strand breaks (DSBs) as secondary lesions. Here, we examined the intranuclear distribution of chromate-induced breaks and a central DSB signaling branch targeting histone H2AX. Using ascorbate-restored cells (H460 human lung epithelial cells, normal human lung and normal mouse embryonic fibroblasts (MEFs)), we found that Cr(VI) produced a typical DSB-associated spectr

Prevalence of germline mutations of MLH1 and MSH2 in hereditary nonpolyposis colorectal cancer families from Spain.

HNPCC is an autosomal dominantly inherited cancer-susceptibility syndrome that confers an increased risk for colorectal cancer and endometrial cancer at a young age. It also entails an increased risk of a variety of other tumors, such as ovarian, gastric, uroepithelial and biliary tract cancers. The underlying pathogenic mutation lies in 1 of the 5 known DNA MMR genes (MSH2, MLH1, PMS1, PMS2 and MSH6). We screened a total of 140 individuals from 56 Spanish families with suspected HNPCC for mutations in the DNA mismatch repair genes MLH1 and MSH2, using DGGE and direct DNA sequencing. Families …

Approaches to identification of HNPCC suspected patients in Slovak population.

Patients with hereditary non-polyposis colorectal cancer (HNPCC) have a DNA mismatch repair defect (MMR) in their tumor tissue that results in instability of microsatellite DNA sequences (MSI). Thus, MSI analysis may effectively indicate this form of cancer that should be then proved by analysis of germline mutations in MMR genes. The aim of this study was to identify HNPCC suspected patients in the Slovak population by investigating microsatellite instability in colorectal tumor tissues. MSI was studied at 5-11 loci in matched tumor and normal DNA using radioactively labeled PCR products sepa…

Autophagy inhibition enhances PD-1 blockade efficacy in mismatch repair-proficient colorectal cancer.

BACKGROUND: Colorectal cancer (CRC) remains one of the most common and lethal malignancies worldwide, with metastatic disease accounting for the majority of CRC-related mortality. Immunotherapy with programmed death (PD)-1/PD-ligand 1 (PD-1/PD-L1) inhibitors has shown remarkable success in treating metastatic colorectal cancers (CRC) with deficient mismatch repair (dMMR) but is less effective in cancers with proficient mismatch repair (pMMR). Recent studies suggest that enhanced autophagy may negatively regulate the effects of PD-1/PD-L1 inhibitors in pMMR tumors. This study aims to investigat

Universal screening for Lynch syndrome and molecular classification of patients with endometrial cancer.

OBJECTIVE: Lynch syndrome (LS) is a cancer susceptibility syndrome characterized by a high risk table of multiple cancer types, most commonly endometrial cancer (EC). Currently, universal tumor screening to identify LS in women with EC remains uncommon. This study aimed to determine the incidence of LS and its molecular landscape by conducting a retrospective analysis of a cohort of patients with EC in China. METHODS: A total of 220 patients with EC were identified from the Hospital between March 2022 and May 2025. Germline variants in LS-associated genes ( MLH1, PMS2, MSH2, MSH6 , and EPCAM

Molecular analysis of the evolutionary history of endometrial and ovarian carcinoma in Lynch syndrome.

Lynch syndrome (LS) is a prevalent cause of hereditary gynecological cancers. DNA mismatch repair (MMR) defects are important players in LS tumorigenesis, but the developmental steps leading to malignancy are incompletely understood. We undertook a deep sequencing approach with a panel of ~1,000 cancer-associated genes to detect somatic changes in retrospective specimens from 33 LS carriers who had developed endometrial carcinoma (EC) or ovarian carcinoma (OC). Consecutive samples of atypical endometrial hyperplasia (AH) and EC or OC (64 samples plus blood) were available from a screening peri…

The Role of Predictive and Prognostic Biomarkers in Lower Female Genital Tract Pathology: PD-L1, MMR, HER2, p16, p53, and Beyond.

Biomarkers play a crucial role in the diagnosis, treatment planning, and prognosis of premalignant and malignant lesions and are increasingly used in neoplasia of the lower female genital tract (LFGT) including the cervix, vagina, and vulva. This review will discuss key biomarkers routinely used in LFGT pathology, including programmed cell death ligand 1 (PD-L1), mismatch repair (MMR), and tumor mutational burden (TMB) testing, which are FDA-approved companion diagnostics for anti-PD-1 checkpoint inhibitors. Recent developments in HER2 testing as a marker for anti-HER2 therapies, and prognosti

Impact of universal immunohistochemistry on Lynch syndrome diagnosis in an Australian colorectal cancer cohort.

BACKGROUND: Current guidelines recommend a step-wise screening algorithm for all colorectal carcinomas (CRC) to identify patients with Lynch syndrome (LS). AIM: To describe the frequencies of mismatch repair deficiency (dMMR), BRAFV600E mutations and MLH1 methylation in resected CRC, and evaluate the impact of universal screening on LS detection. METHODS: Retrospectively, 1171 consecutive cases of resected CRC were identified between 2010 and 2017 from a large multi-centre pathology service. Testing for dMMR by immunohistochemistry (IHC) was initiated by the reporting pathologist from 2010,

A novel panel of short mononucleotide repeats linked to informative polymorphisms enabling effective high volume low cost discrimination between mismatch repair deficient and proficient tumours.

Somatic mutations in mononucleotide repeats are commonly used to assess the mismatch repair status of tumours. Current tests focus on repeats with a length above 15bp, which tend to be somatically more unstable than shorter ones. These longer repeats also have a substantially higher PCR error rate, and tests that use capillary electrophoresis for fragment size analysis often require expert interpretation. In this communication, we present a panel of 17 short repeats (length 7-12bp) for sequence-based microsatellite instability (MSI) testing. Using a simple scoring procedure that incorporates t

Screening for Lynch syndrome using risk assessment criteria in patients with ovarian cancer.

PURPOSE: Despite advances in neoadjuvant chemotherapy (NACT) for breast cancer, surgical outcomes following immediate breast reconstruction (IBR) remain inconsistent, and institutional protocols vary widely. Although patients exhibit heterogeneous physiological responses to chemotherapy, few studies have evaluated treatment-related toxicity as an objective risk marker of postoperative complications. The aim of this study was to assess whether patient-specific responses to NACT can serve as clinically relevant markers of complication risk and guide individualized surgical planning. METHODS: A

Connections between constitutional mismatch repair deficiency syndrome and neurofibromatosis type 1.

Constitutional mismatch repair (MMR) deficiency (CMMRD) is a rare childhood cancer susceptibility syndrome resulting from biallelic germline loss-of-function mutations in one of the MMR genes. Individuals with CMMRD have high risk to develop a broad spectrum of malignancies and frequently display features reminiscent of neurofibromatosis type 1 (NF1). Evaluation of the clinical findings of genetically proven CMMRD patients shows that not only multiple café-au-lait macules but also any of the diagnostic features of NF1 may be present in a CMMRD patient. This phenotypic overlap may lead to misdi

A novel complex mutation in MSH2 contributes to both Muir-Torre and Lynch Syndrome.

Mutations in mismatch repair genes lead to Lynch Syndrome, the most common form of inherited colorectal cancer. In this report, we describe a novel complex germline mutation c.[1601_1661+92dup; 1591_1611del] of the mismatch repair gene, MSH2. This mutation, which segregates with the disease phenotype, was discovered in a Lynch syndrome kindred that also shows a history of the Muir-Torre syndrome. Interestingly, several tumors from this family displayed microsatellite instability, a hallmark of Lynch syndrome tumors but no consistent, concomitant loss of MSH2 protein expression. In addition, a
