[Sudden deafness. A clinical analysis with special reference to tympanoscopy].

From a total of 720 patients with acute deafness, 85 patients suffered from unilateral deafness with a hearing loss of at least 80 dB in the speech frequency range. Under conservative therapy in 58 patients, there was an improvement in hearing in 19 cases and a return to normal in 6 patients. In 27 patients, the clinical history was suggestive of a possible tympanic membrane rupture, which was confirmed by tympanoscopy in 5 cases. On 5 other occasions, the intraoperative findings were highly suggestive of tympanic membrane rupture. In each case, a membranoplasty was performed. The results of o…

Probational treatment of sudden deafness with prostacyclin: a pilot study.

Sudden idiopathic hearing loss has occasionally been supposed to be caused by a disturbed microcirculation in the inner ear of unknown origin. Little is known about the regulation of cochlear blood flow and the effectiveness of drugs in cochlear microcirculation. Because animal experiments gave evidence that prostacyclin (PGI2) might be one biochemical substratum of local regulators in the flow of blood in the stria vascularis, 11 patients with sudden idiopathic hearing loss were treated once for 6 h with prostacyclin (10 ng/kg body weight/min) in a first open clinical trial. In most cases pro

[An autopsy case of generalized seizures, myoclonus, blindness and deafness].

We have reported the clinical and autopsy findings in a case with generalized seizures, myoclonus, blindness and deafness which was accompanied by stroke-like episodes. This case was diagnosed as mitochondrial encephalomyopathy, lactic acidosis & stroke-like episodes (MELAS) from these findings. Solitary and continuous lesions of softening were distributed in both hemispheres, more severely in the frontal and occipital poles. These lesions did not correspond to a vascular supply. The pulvinar, lateral and medial geniculate body of the thalamus, cerebellar vermis and dentate nucleus had small l…

Reissner's membrane in aging, Menière's disease, and profound sensorineural deafness.

The characteristics of Reissner's membrane from 47 human cochleae with mild endolymphatic hydrops, profound sensorineural deafness and normal ears were studied by light microscopy. The highest cell densities were observed in the zones adjacent to the limbus spiralis and the stria vascularis. The cell density of Reissner's in normal ears decreased with age concomitant with an increased formation of epithelial cell clusters. In hydrops, the density increased with the exception of the apical turn. However, sporadic loss of the cells in isolated areas of the membrane was observed. The ears with pr…

[The onset of sudden deafness and Bell's palsy--statistical analysis].

Existing studies on sudden deafness and Bell's palsy have shown no statistically significant differences in the numbers of patients by the times of the onsets, namely, days of the week, weeks of the month, months of the year and seasons of the year. However, accumulation of collective data in various areas and years may produce averaged data. Correlation between the occurrence of sudden deafness and that of Bell's palsy has not been studied so far. The yearly differences in the onsets of these diseases for the past 10 years were investigated using the Kendall's coefficient of concordance. In s…

[Borderline cases of sudden deafness and so-called vestibular dysfunction].

Forty-seven patients (31 male, 16 female) have been treated at the ENT University Clinic of Marburg during the last 13 years for sudden sensorineural hearing loss combined with disorders of equilibrium. The epidemiological data, history, audiological and electronystagmographic findings of those patients with cochleovestibular lesions are reported. The results of follow-up examination are compared with those of the patients who suffered from sudden deafness and vestibular neuronitis. In patients with combined lesions, special attention was paid to any correlation between the hearing loss and th…

Immunological study on association between mumps and infantile unilateral deafness.

The incidence of unilateral deafness in primary school children has been reported to be about 0.1%, but the cause of most of these cases has not been clarified, because parents and children themselves cannot recognize the onset of hearing loss occurring in one ear alone. This study was performed to clarify the possibility that mumps causes infantile unilateral deafness. The results were: 1) on dividing a total of 78 cases of infantile unilateral deafness according to age from 3 to 6 years, mumps infection rate in the 4-year-old group was nearly twice that in a randomly selected group; 2) on ch…

Infantile X-linked ataxia and deafness: a new clinicopathologic entity?

We describe an X-linked disorder of the CNS, characterized by onset, in infancy, of hypotonia, ataxia, sensorineural deafness, developmental delay, esotropias, and optic atrophy, and by a progressive course leading to death in childhood. Pathologically, neuron loss and gliosis of the dentate nucleus and inferior olive are conspicuous; involvement of the cerebellar cortex is less prominent. In the proband, the red nucleus, dorsal motor nucleus of the vagus, and central auditory pathways were severely affected. The mother of the proband, now 33, has self-limited episodes of ataxia, and cerebella…

Fenestration of the horizontal semicircular canal in congenital conductive deafness.

Fenestration of the horizontal semicircular canal enables the otologic surgeon to restore hearing in those patients with congenital conductive deafness who are not candidates for stapedectomy, ossicular reconstruction, or tympanoplasty. Since the development of stapedectomy, much of the finesse technique of fenestration surgery has been lost, and many otologic surgeons today are unfamiliar with the fenestration operation. The surgical technique for creating a permanently patent fenestra in the horizontal semicircular canal is described. The causes of failure in fenestration surgery are reviewe…

Diagnostic evaluations in pediatric cases of congenital bilateral sensorineural deafness.

The earliest possible diagnostic evaluation of hearing impairment among infants and children is of the utmost importance for their future well-being. The essential features of the family history and the pediatric physical examination are reviewed, discussed, and illustrated in studies and actual case histories of congenital bilateral sensorineural hearing loss.

Previously undescribed syndrome of craniofacial, hand anomalies, and sensorineural deafness.

We describe in a mother and her infant daughter a previously unreported syndrome of unusual craniofacial, hand, and digital anomalies. Both mother and child have a flat facial profile, hypertelorism, hypoplastic nose with slit-like nares, and a sensorineural hearing loss. Common radiographic manifestations include small maxilla, absent or small nasal bones, and ulnar deviation of the hands. This is either an autosomal dominant or X-linked trait.

Childhood deafness in Australia. Incidence and maternal rubella, 1949-1980.

A survey of children born between 1949 and 1980 who are known to have been fitted with hearing aids in Australia by the age of 18 years reveals a ratio of 2.605 cases for every 1000 liver births. This incidence of childhood deafness is higher than previously reported, as a result of the greater number of birth years considered. Approximately 41.2% of these children have severe or profound deafness. A history of maternal rubella was reported in 11.06% of cases, but a significant reduction in the number of reported rubella cases after 1976 has led to correspondingly fewer diagnoses of severely a…

Priapism: an unusual complication of heparin therapy for sudden deafness.

Two cases of priapism occurring during heparin therapy for sudden hearing loss are reported. The association of anticoagulants and priapism is increasingly recognized. Literature studying possible mechanisms of the role of heparin in priapism is reviewed.

[Nystagmus findings during sudden deafness and their estimation (author's transl)].

Seventy-three of 172 patients with sudden deafness had a vestibular participation. The found forms of spontaneous and provoked nystagmus, their partial combination with thermic under- or non-excitability of the labyrinth as well as vertigo complaints of the patients are described, especially in their temporal course during the illness. The frequent vestibular participation during sudden deafness entitles to introduce the term "range of forms of peripheral-vascular conditioned labyrinthine disturbances", whose cornerstones are the sudden isolated peripheral deafness and the sudden isolated peri…
