WHAT IS KNOWN AND OBJECTIVE: There have been concerns about a potential link between long-term use of bisphosphonates (BPs) for the treatment of osteoporosis and rare serious adverse effects, such as atypical femoral fractures. Cases discontinuing bisphosphonates(BPs)include cases due to adverse effects, non adherence, no effectiveness, and lower risk of fragile fractures. For patients with osteonecrosis of the jaw or atypical femoral fracture, discontinuation of BPs and the switch to Teriparatides may be recommended until the treatments are completed. For the patients of discontinuation of BPs due to lower risk of fractures, the treatment strategy will be determined based on the presence or absence of fracture, the change of bone mineral density and bone resorption markers.

Anti-osteoporotic drugs directly control bone metabolism. Then, they are possibly involved in interactions between bone and many other tissues. It is not, however, clear how they influence several organs other than bone, or what clinical significance they have. Recently, accumulating evidence suggests that osteocalcin as a humoral factor from bone is actively involved in glucose metabolism. Since interaction between osteoporosis and diabetes mellitus is an emerging clinical issue, there are several clinical questions under vigorous investigations.

Postmenopausal osteoporosis is associated with microarchitectural deterioration and increased risk of fracture. Osteoporosis therapy effectively reduces the risk of vertebral, nonvertebral, and hip fracture and has been associated with increased survival. Currently approved treatments for osteoporosis include bisphosphonates, denosumab, selective estrogen receptor modulators, and teriparatide. Patients with osteoporosis often have chronic kidney disease (CKD). INTRODUCTION: Bisphosphonate therapy (BT) is used commonly in the management of osteoporosis. A systematic review was conducted investigating delayed union of lower limb, long bone fractures in patients on BT. We specifically assessed whether BT increases the risk of delayed union or non-union in lower limb, long bone fractures.
