Urinary tract complications with rectal surgery.

The possibility of urinary tract injury should always be considered in the course of anterior resection or combined abdominoperineal resection of the lower colon and rectum. Controlled studies of ureteral and other injuries and fistulas cannot be made; but fortunately, unanticipated damage to the lower urinary tract does not occur often. The surgeon operating in the pelvis should be aware of the problems rarely encountered and should be capable of their management. If a urologic surgeon is available, consultation often is desirable. Prompt intraoperative recognition is most important so remedi

Surgical treatment of acute diverticulitis by staged procedures.

A retrospective 10-year experience with the traditional three-stage plan (diverting colostomy, resection, colostomy closure) for perforated diverticulitis of the colon in four urban hospitals was reviewed to accurately assess the mortality rate. Only patients who were admitted in a non-elective manner with signs of an acute abdomen or who were already hospitalized with another illness and developed an acute abdomen were considered. Fecal or generalized purulent peritonitis, or pelvic peritonitis with abscess were observed at laparotomy in all instances. Two hundred and eight patients represent

The laparoscopic approach in the treatment of diverticular colon disease.

BACKGROUND AND OBJECTIVES: The experience with treatment of diverticular colon disease (DCD) by the laparoscopic method is analyzed. METHODS: Between January 1994 and July 1997, a group of 22 patients with criteria for symptomatic diverticular disease in the descending and sigmoid colon underwent laparoscopy with average resections of 40 cm. Intra-abdominal mechanical anastomosis completed the procedure. RESULTS: The operative morbidity was 28%. Two cases, in acute diverticulitis phase, were reconverted to open surgery, and three cases presented postoperative rectorrhagia which ceased sponta

Outcomes Following Robotic Surgery for Complicated and Uncomplicated Diverticulitis.

BackgroundLaparoscopic colectomy is standard for uncomplicated diverticulitis (UD) but has higher conversion and morbidity rates in complicated diverticulitis (CD). Robotic colectomy (RC) is increasingly used for both UD and CD. This study compared outcomes of RC for CD and UD and evaluated factors contributing to adverse outcomes.MethodsUsing 2014-2021 NSQIP data, we identified patients with CD or UD who underwent RC. The primary endpoint was textbook outcome (TO), defined as absence of prolonged length of stay (LOS), major morbidity, readmission, anastomotic leak, or mortality within 30 days

[Infected Brachiocephalic Arterial Aneurysm Involving Ascending Aorta:Report of a Case].

The infected brachiocephalic arterial aneurysm is a very rare but highly lethal, life-threatening, and difficult-to-treat condition. We report a case of infected brachiocephalic arterial aneurysm involving ascending aorta. An 81-year-old woman with a history of colon diverticulitis was referred to our hospital because of infected brachiocephalic aneurysm. Emergent operation was performed because the aneurysm grew rapidly. The aneurysm was excised, extra-anatomical grafting was performed with a prosthetic vascular graft and the ascending aorta was repaired with bovine pericardium patch. The aor

Perforated sigmoid diverticulitis presenting as Fournier's gangrene.

Fournier's gangrene (FG) is a fulminant necrotising fasciitis of the perineum and genital region. We report a rare case of FG secondary to retroperitoneal perforation of sigmoid diverticulitis in a male patient in his late 60s. The patient presented with septic shock, scrotal necrosis and faecal discharge. Cross-sectional imaging revealed retroperitoneal air and oral contrast tracking from a perforated sigmoid colon into the scrotum. He underwent emergency scrotal debridement followed by sigmoidectomy with end colostomy. Early surgical source control, broad-spectrum antibiotics and vacuum-assi

[Surgical therapy of enterovaginal fistulae following gynecologic primary procedures (author's transl)].

This is a report on 14 enterovaginal fistulae following gynecologic operations. The fistulae were caused by a hysterectomy in 8 cases. There were 3 other enterovaginal fistulae following exploratory laparotomy for ovarian tumors, which proved to be pseudotumors caused by diverticulitis of the sigmoid colon. Rare causes for fistulae were cesarian section, episiotomy, and incision of an abscess of Bartholins' gland. The time interval between operation and manifestation of the fistula was usually about 10 days. The operative management required a two-or three-step procedure. Only 2 of 14 patients

Laparoscopic sigmoid resection for diverticulitis decreases major morbidity rates: a randomized control trial: short-term results of the Sigma Trial.

BACKGROUND: No randomized controlled trial has compared laparoscopic sigmoid resection (LSR) to open sigmoid resection (OSR) for symptomatic diverticulitis of the sigmoid colon. This study tested the hypothesis that LSR is associated with decreased postoperative complication rates as compared with OSR. METHODS: This was a prospective, multicenter, double-blind, parallel-arm, randomized controlled trial. Eligible patients were randomized to either LSR or OSR. Endpoints included postoperative mortality, and complications were classified as major and minor. The generator of the allocation sequen

[Use of the Ravo bypass in high-risk colonic anastomoses].

The Authors report a case of high risk colonic anastomosis treated by intracolonic by-pass procedure. The technique of Coloshield application is described. Its use prevents fecal contamination of the anastomotic site and, in case of dehiscence, anastomotic fistula formation.

Colovesical fistulas.

Colovesical fistula often presents with recurrent or persistent urinary tract infection, especially in men. The commonest cause is diverticular disease. Other causes include carcinoma of the colon, Crohn's disease, radiotherapy and trauma. Barium enema may suggest the pathology but cystoscopy is the best investigation to confirm the presence of a fistula. One-stage resection and anastomosis is suitable for most patients with diverticular disease.

[Exploratory laparotomies in geriatric-gynaecological patients (author's transl)].

In 17 gynaecological departments of West-Berlin hospitals, 2940 laparotomies were done between 1960 and 1969 in women of at least 60 years of age, of which 331 (10,9%) were exploratory. Of these the postoperative mortality was 39,2% (130 cases). The fatal cases could be divided into 120 women suffering from inoperable malignant tumors, 7 from sigmoid diverticulitis and 3 from chromical gynaecological inflammatory tumors. The frequency of such critical exploratory laparotomies could be lowered partly by intensifying cancer routine check-up by specialists by more complex diagnostic measures befo

Asymptomatic sigmoid diverticulum perforation-induced subcutaneous, mediastinal, and retroperitoneal emphysema: A case report and literature review.

BACKGROUND: Mediastinal and subcutaneous emphysema usually result from spontaneous rupture of the alveolar wall. We present an educational case of subcutaneous, mediastinal, and retroperitoneal emphysema discovered during a routine medical check-up resulting from an asymptomatic perforation of the sigmoid diverticulum. CASE PRESENTATION: A 66-year-old man presented to our hospital for his health check-up. A chest X-ray revealed mediastinal emphysema during a physical examination. The patient had no subjective symptoms, fever, or hemodynamic instability. Physical examination revealed a snow gr

Toward an evidence-based step-up approach in diagnosing diverticulitis.

BACKGROUND: The lack of pathognomonic findings and the chance of complicated disease have resulted in the widespread use of additional imaging to diagnose acute colonic diverticulitis (ACD). The added value of additional imaging in the diagnostic workup of patients suspected of ACD is not well defined. AIMS: The aim of this study was to systematically review the literature of the accuracy of the clinical evaluation and diagnostic modalities for patients with suspected ACD, to come to an evidence-based approach to diagnose ACD. METHODS: A systematic review and meta-analysis of studies that re

A rare case of epiploic appendagitis of the vermiform appendix.

Epiploic appendagitis is a rare and often overlooked cause of acute abdominal pain that can mimic more common surgical conditions such as acute appendicitis or diverticulitis. Most reported cases involve the sigmoid or descending colon, whereas epiploic appendagitis occurring near the vermiform appendix is extremely rare. We report the case of a 57-year-old woman who presented with acute right lower quadrant pain initially suspected to be acute appendicitis. Computed tomography revealed inflammation of the epiploic appendage adjacent to the base of the appendix, with mild reactive appendicitis

Colo-ovarian fistula masquerading as tubo-ovarian abscess: a case report.

BACKGROUND: Colo-ovarian fistulas are exceptionally rare complications of diverticulitis, with very few cases documented. Their nonspecific presentation often mimics gynecologic pathology, frequently leading to misdiagnosis and delayed treatment. Currently, there are no standardized guidelines for diagnosis or management of colo-ovarian fistulas. This unique case describes a patient who initially presented with findings suggestive of a tubo-ovarian abscess (TOA), but contrast-enhanced computed tomography (CT) with oral and rectal contrast revealed a colo-ovarian fistula, illustrating a potenti
