Spontaneous perforation of the oesophagus is a rare occurrence that is usually due to vomiting and is seldom associated with an oesophageal lesion. We report a case of the spontaneous perforation of a large oesophageal diverticulum not preceded by any precipitating event in a 75-year-old male who was not known to have achalasia. The diverticulum was repaired by emergency surgery. Achalasia was later diagnosed and successfully treated with botulin toxin injection. Surgery decision-making and the treatment of achalasia are discussed.

The purpose of this study was to evaluate the computed tomography (CT) and magnetic resonance imaging (MRI) features of juxtapapillary diverticulum. CT and/or MRI examinations of 12 patients were evaluated. The size, location of the diverticula and imaging findings of associated pancreaticobiliary disease were assessed. BACKGROUND: Meckel's diverticulum is an uncommon entity. A high index of suspicion is necessary for opportune diagnosis and prompt treatment. Technetium (TC) 99m pertechnetate scintigraphy is a sensitive and specific test for Meckel's diverticulum. In adults, the scan contributes little to clinical decision making and often will not change the need for surgical intervention. We describe our experience with four patients.

BACKGROUND: Leiomyosarcoma is an uncommon small bowel tumor. About 60 cases of a Meckel's diverticulum localization have been reported. We present a very rare form of leiomyosarcoma arising in a Meckel's diverticulum, mimicking a very large abdominal cyst.
CASE REPORT: Abdominal ultrasound evidenced a voluminous cystic mass in the abdomen of a 37-year-old man consulting for abdominal discomfort. Computed tomography visualized a tissular component with contrast uptake. Laparotomy was performed with resection of a 4 liter cyst on the Meckel's diverticulum. Histology reported leiomyosarcoma.

Caroli's disease or communicating ectasia of the intrahepatic biliary tree is a rare disease with unknown aetiology. The coexistence of this along with the uncommon condition of a gastric diverticulum has never been reported before. A deficiency in the fibromuscular matrix of both the bile ducts and the gastric wall may explain why these two pathologies may coexist in a single patient.

We report two cases of symptomatic Meckel's diverticulum in adults with recurrent abdominal pain and episodes of minor lower gastrointestinal bleeding. In case 1, the diagnosis was suggested by (99m)Tc pertechnetate scan and confirmed by laparoscopy; whereas in case 2, only diagnostic laparoscopy was performed because of suspected appendicitis. A segmental small bowel resection with attached diverticulum was performed extracorporeally after exteriorization through the umbilical port site in both cases.

BACKGROUND: Surgical correction of symptomatic Zenker's diverticulum is effective; however, elderly symptomatic patients may be denied surgical intervention because of perceived increased risks.
METHODS: To address this concern, we reviewed 75 patients (46 men and 29 women) found to have this condition during the past two decades.

There are few reports of the sonographic appearance of Meckel's diverticulum. We present a case of torsion of a Meckel's diverticulum that was suggested by sonography and confirmed pathologically. We discuss the sonographic differential diagnosis, which includes acute appendicitis, enteric duplication cyst and intestinal volvulus.

Seventy-two cases of Meckel's diverticulum were treated surgically in recent four years. There were 61 boys and 11 girls with a mean age of 5.2 years. Of the 65 symptomatic patients, 31(43%) had rectal bleeding, 16(23%) suffered from bowel obstruction and 17(24%) peritonitis. Patients with diverticulitis(8 cases) had acute abdominal pain in the periumbilical region or in the right lower quadrant compatible with appendicitis. Ectopic mucosa was found in 47 cases(65.3%). Meckel's diverticulum is a pathology not rarely found everyday clinical medicine especially when it present with one of its complications. The specific diagnosis is a bit difficult to put because of the low sensibility and specificity of symptoms of diagnostic and instrumental techniques used. The reported case shows a rare type of complication constituted by the phlogosis of Meckel's diverticulum, itself due to an extraneous thing ingested by the patient. The authors discuss the entity of the manifestations of this pathology and highlight every aspects.

The case of a 44-year-old woman with urethral diverticulum is described. The lesion was initially detected by ultrasonography. MRI proved to be very sensitive in localising and delineating the urethral diverticulum and could differentiate it from lesions arising from the vagina or bladder.
