BACKGROUND: The "terrible triad" of the elbow is characterized by a combination of elbow dislocation with fractures of the radial head and the coronoid process of the ulna. This injury is notoriously complex due to the rupture of key elements that ensure elbow stability, resulting in poorer outcomes compared to simpler joint injuries. Despite advances in treatment protocols, an optimal surgical approach and fixation technique standard have not yet been established.
METHODS: This descriptive study focused on reviewing the anatomical and clinical characteristics of the elbow joint, considering

Elbow arthroscopy is a technically demanding procedure. A comprehensive understanding of the elbow neurovascular anatomy combined with a very detail-oriented technique for portal placement helps to minimize risk and facilitates performing elbow arthroscopic procedures effectively and safely for the appropriate indications. The technique for establishing all portals is critical regardless of which portals are used. Care should be taken to thoroughly palpate the bony landmarks before establishing any portal site, and only blunt trocars should be used for elbow arthroscopy. Identification of the

BACKGROUND: Heterotopic ossification is the most common extrinsic cause of elbow contracture and may lead to clinically important stiffness, and rarely, complete bony ankylosis. Surgery sometimes is performed to treat this problem, and published reports differ regarding the factors that are associated with success or failure after this operation and whether the procedure is effective for patients with elbow ankylosis.
QUESTIONS/PURPOSES: We wished (1) to identify potential patient characteristics and modifiable risk factors that are associated with improvements in ROM after surgery for hetero

Baseball pitching imposes a dangerous valgus load on the elbow that puts the joint at severe risk for injury. The goal of this study was to develop a musculoskeletal modeling approach to enable evaluation of muscle-tendon contributions to mitigating elbow injury risk in pitching. We implemented a forward dynamic simulation framework that used a scaled biomechanical model to reproduce a pitching motion recorded from a high school pitcher. The medial elbow muscles generated substantial, protective, varus elbow moments in our simulations. For our subject, the triceps generated large varus moments

Fifteen children with an untreated posterior dislocation of the elbow were seen between 1965 and 1980. Three had a useful range of painless flexion and were not treated by operation. Twelve had a stiff elbow and had an open reduction between three weeks and three years after injury. The triceps was lengthened when it prevented reduction and Kirschner wires were used when necessary to stabilize the elbow. Complications included a transient paralysis of the hand in one patient and myositis ossificans with a rigid elbow in another. The length of follow-up ranged from one to six years. In eleven p

BACKGROUND: Elbow wounds pose a reconstructive challenge. Prior studies have described the vascular anatomy of both the brachioradialis and flexor carpi ulnaris muscle flaps. The goal of this study was to describe the distal flap perfusion of the flexor carpi radialis, with a direct comparison of the brachioradialis, flexor carpi ulnaris, and flexor carpi radialis muscle flaps for coverage around the elbow.
METHODS: Six fresh-frozen upper extremity specimens were dissected for brachioradialis, flexor carpi radialis, and flexor carpi ulnaris flaps. Vascular data from prior studies were combine

Increased valgus stresses on the medial aspect of the elbow can result in a number of bony and soft-tissue abnormalities. The most common abnormalities are ulnar neuropathy and valgus instability secondary to injury of the medial collateral ligament. Diagnosis and treatment of these disorders are discussed in this article.

The surgical therapy of the rupture of a distal biceps tendon and a modified postoperative treatment is described. Mainpoint is the early treatment in anaclisis of Kleinert as it is standardized in the treatment of ruptures of the flexor tendons of the hand. A forearm orthesis with a simply ROM-joint without reboundmechanism was used. Possible benefits are shown and further industrial evolution seems to be indicated.

Traumatic distal avulsion of triceps brachii is a rare injury, usually due to a fall on the outstre ched elbow, with sudden contraction of the triceps muscle. Examination reveals pain, swelling, and a palpable depression just proximal to the olecranon. Lateral X rays show avulsed osseous material. Surgical therapy was advisable for complete avulsion. In this case, the authors used absorbable sutures for this reinsertion. Immediate surgical repair always achieves good results.

BACKGROUND: Diagnosing simple valgus instability of the elbow currently involves time- and cost-intensive imaging modalities such as magnetic resonance imaging or magnetic resonance arthrography. Previous studies have demonstrated that stress radiography represents an alternative diagnostic tool for such conditions. The aim of this study was to investigate whether standardized valgus stress radiography can identify soft-tissue lesions of the medial elbow.
METHODS: A telos stress device (telos GAIII/E; telos Arzt- und Krankenhausbedarf GmbH, Woelfersheim-Bernstadt, Germany) was used to apply 5

BACKGROUND: Management of any injury is expensive to the individual and the community. For this reason modification of methods of managing trauma very often is necessary in resource poor countries.
OBJECTIVE: To verify the effectiveness of metal hinge joints in prevention of contractures and ankylosis of the elbow joint in cast immobilisation of reduced open forearm fractures.
PATIENTS: 20 patients who had sideswipe injuries were managed from the year 01/01/2000 to date (20/5/2004) by the orthopaedic unit of Usmanu Danfodiyo University Teaching Hospital, Sokoto, Nigeria. They were referred f

PURPOSE: To determine if specific coronoid fractures relate to specific overall traumatic elbow instability injury patterns and to depict any relationship on fracture maps and heat maps.
METHODS: We collected 110 computed tomography (CT) studies from patients with coronoid fractures. Fracture types and pattern of injury were characterized based on anteroposterior and lateral radiographs, 2- and 3-dimensional CT scans, and intraoperative findings as described in operative reports. Using quantitative 3-dimensional CT techniques we were able to reconstruct the coronoid and reduce fracture fragme

BACKGROUND: Heterotopic ossification (HO) occurs frequently in the elbow in burn patients, and extends beyond the anatomical structure. HO of the elbow can cause joint contracture and adversely affect activities of daily living.Currently, there is no effective prophylaxis for HO as the precise underlying mechanism remains unknown. Therefore, there is no choice but to treat HO after it has developed. To date, however, no effective standard treatment has been reported, and therefore treatment methods vary between different facilities. Surgical resection is widely accepted as the only therapeutic

We present the case of an 11-year-old boy who sustained an anterior elbow dislocation associated with fractures of the medial epicondyle and humeral condyle following a fall from playground equipment. Anterior elbow dislocation is an exceptionally rare injury in the pediatric population, and its association with concomitant articular fractures further increases its clinical complexity. Early diagnosis and prompt surgical management were essential to prevent anatomical and functional sequelae. The patient underwent closed reduction followed by open reduction and internal fixation with Kirschner

INTRODUCTION: Isolated acute distal radioulnar joint (DRUJ) dislocation is a rare injury that should be early recognized and treated promptly to avoid the limitation and disability associated with delayed diagnosis and management.
CASE PRESENTATION: We present a patient with a traumatic dorsal isolated DRUJ dislocation who was successfully treated with a closed reduction and k-wire pinning along with cast immobilization.
DISCUSSION: Previous reports of distal radioulnar joint dislocation have described the mechanics of this injury as well as a guidance to diagnosis and treatment. Closed redu

Using any imaging modality, the elbow is a complex joint to evaluate. The use of scintigraphy in the evaluation of the effects of acute or chronic trauma has largely been confined to epicondylitis. We developed a skyline view of the elbow that minimized the effect of overlap and assessed its incremental value in the scintigraphic assessment of several pathological conditions. Thirty-four abnormalities were evaluated in 20 patients with a range of pathologies, including epicondylitis, ligamentous avulsion injury, articular injury, fractures and synovitis. The incremental value of addition of th
