Pseudohypoparathyroidism type 1B (PHP1B) is characterized by renal tubular resistance to parathyroid hormone (PTH) leading to hyperphosphatemia, hypocalcemia, elevated PTH, and hyperparathyroid bone changes. PHP1B is an imprinting disorder that results from loss of methylation at the maternal GNAS gene, which suppresses transcription of the alpha subunit of the stimulatory G protein of the PTH receptor. Emerging evidence supports an association between assisted reproductive technologies (ART) and imprinting disorders; however, there is currently little evidence linking PHP1B and ART. We presen

BACKGROUND: We aimed to determine if a modified Dunn osteotomy could be safely performed without surgical dislocation and consequent preservation of the ligamentum teres.
METHODS: All patients undergoing a modified Dunn osteotomy for a slipped capital femoral epiphysis over an 8-year period were included in this study, and all had a severe slip with an open physis.
RESULTS: The modified Dunn procedure was performed on 34 hips in 34 patients. The mean age was 13.1 years (range, 11 to 16 y) with a mean follow-up time of 54 months (range, 15 to 102 mo). All slips were severe (grade 3) with a me

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is usually treated with percutaneous in situ screw fixation to prevent further progression of deformity. The purpose of this investigation is to compare computer navigation (CN) techniques with traditional fluoroscopic (fluoro) techniques for in situ fixation of SCFE.
METHODS: This study was an IRB-approved prospective study of 39 hip pinnings in 33 children. CN techniques were used in 22 cases and fluoro in 17. The CN and fluoro groups were statistically similar in terms of grade and acuity of the slip. Children were assigned to the groups

BACKGROUND: The purpose of the present study was to determine whether the modified Oxford bone score can be used as a predictor for the risk of developing contralateral slipped capital femoral epiphysis (SCFE) in children who present with a unilateral slip.
METHODS: We identified 260 patients treated for unilateral SCFE between 1980 and 2002 and followed them up to skeletal maturity or until development of contralateral slip. Exclusion criteria included patients with endocrine or metabolic disorder, Down syndrome, and those with radiographs inadequate to determine the modified Oxford bone sco

PURPOSE: Slipped capital femoral epiphysis (SCFE) represents the preliminary stage of osteoarthritis. Reliable tools for outcome evaluation should be developed to prevent persisting defects. The functional outcome of SCFE-patients after growth arrest is analysed by instrumented 3D-gait analysis (GA). The results are matched to the clinical examination findings.
METHODS: A total of 39 SCFE patients after growth arrest (18.8 years; BMI 26.5 kg/m²) with unilateral affection were included. The clinical results were classified according to Harris hip and clinical Heyman Herndon scores. 3D-GA-param

Multiple radiographic images may be necessary during the standard procedure of in-situ pinning of slipped capital femoral epiphysis (SCFE) hips. This procedure can be performed with the patient positioned on a fracture table or a radiolucent table. Our study aims to look at any differences in the amount and duration of radiation exposure for in-situ pinning of SCFE performed using a traction table or a radiolucent table. Sixteen hips in thirteen patients who were pinned on radiolucent table were compared for the cumulative radiation exposure to 35 hips pinned on a fracture table in 33 patients

The following finite element study was conducted to determine whether increased body weight, femoral retroversion, and varus hip loading could sufficiently raise physeal shear strains and stresses above the yield point and predispose an adolescent hip to a slip. A computer tomography scan of a 13-year-old child with slipped capital femoral epiphysis was used to generate a solid model of the proximal femur and physis. The model was parameterized using 3-dimensional software to generate three difference angles of femoral neck version-neutral, 15 degrees retroversion, and 15 degrees anteversion.

In a 10-year prospective study, we were able to examine 93 children with 109 hips suffering from Perthes' disease and 6 hips with Meyer's dysplasia epiphysealis by magnetic resonance imaging (MRI) follow-up studies. Routine imaging was done by coronary T1- and T2-enhanced sequences and sagittal T1-enhanced sequences. In total of 400 MR images, the following criteria were evaluated: staging of the disease according to MRI signal changes, extent of the necrosis, prognostic criteria, differential diagnosis of Meyer's dysplasia epiphysealis, and proposals for therapy. Typical signal changes in MRI

The efficiency of spin echo tomography in the differential diagnosis of hip joint pain in children in comparison with other imaging methods was tested in 115 spin echo tomography examinations of 68 patients. By means of the different modes available it was possible to determine joint effusion, necrosed areas and the formation of abscesses with great accuracy. In the case of Legg-Calve-Perthes disease the MRT is especially in the early diagnosis superior to the other Methods, already in the early stage a classification in the four Catterall groups was possible. Furthermore the "head at risk" si

A retrospective study of 80 patients with chronic slipped capital femoral epiphysis was performed, analyzing the clinical results with regard to pin placement and pin number. Follow-up averaged greater than 2 years, with 86% of the 80 patients obtaining a satisfactory clinical outcome. Serious complications occurred in 10 patients. The severity of complications increased as the number of pins used increased (p less than 0.07). A varus pin position resulting in a more inferior pin placement in the proximal femoral epiphysis was found to be associated with the fewest complications. When the pin

Awareness of the age at which various pediatric hip disorders present and familiarity with their characteristics greatly aid in early detection. A properly performed physical examination is still the mainstay of early diagnosis. The earlier a hip disorder is discovered and treated, the less the chance of lasting harm to the child.

BACKGROUND: Small joint reconstruction of the hand poses a significant challenge, especially in children where both functional motion and preservation of the epiphysis are desired. Auto-transplantation of whole joints is the only way to reconstruct a functional joint that maintains growth potential. Historically, nonvascularized toe-to-finger joint transfer has been criticized for high rates of avascular necrosis and joint dissolution, while vascularized transfers seemingly resulted in increased durability of the joint space and epiphysis. However, certain populations remain poor candidates fo

Recent expansion of the use of recombinant growth hormone (GH) to non-GH-deficient patients demands close attention to possible complications in these patients, including effects on bone, recent studies on the use of GH in children with chronic renal failure (CRF) provide some early data. Animal models demonstrate that GH stimulates chondrocyte proliferation. Experimental data further suggest that GH can weaken the epiphyseal plate. Slipped capital femoral epiphysis (SCFE) has been reported in GH-deficient patients, having been detected before, during and after GH therapy. In CRF patients trea

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a pediatric hip disorder affecting roughly 1 in 10,000 children and adolescents, with delayed diagnosis and treatment leading to poor outcomes. This study compares the association between presenting hip or knee pain symptomatology and its effects on time to diagnosis and treatment of SCFE, mid- to long-term complications, and risk of requiring hip reconstructive surgeries.
METHODS: A retrospective cohort study was conducted comparing SCFE presentations with chief complaints of either hip or knee pain that led to SCFE surgery. Propensity

In situ pinning of slipped capital femoral epiphysis (SCFE) is a safe and effective treatment modality, but often results in residual deformity leading to femoroacetabular impingement, which may limit patient activities and predispose to early onset arthritis. Enhanced understanding of the implications of femoroacetabular impingement and new surgical techniques have prompted interest in treating post-slipped capital femoral epiphysis deformity to both improve current symptoms and delay or prevent hip arthrosis.

BACKGROUND: In situ pinning, a low-risk treatment for slipped capital femoral epiphysis (SCFE), leaves the slipped femoral head in place and may reduce range of motion (ROM) and cause impingement. It is unclear when a more complex surgery should be considered, because the relationships between severity, slip stability, remodeling, impingement, and ROM are unknown.
RESEARCH QUESTIONS: (1) Do more severe acute SCFE deformities (no bony remodeling) result in a greater loss of flexion ROM?(2) Does the presence or location of impingement on the pelvis vary with severity of acute SCFE deformity?
