Orbital wall restoring surgery with primary orbital wall fragments in blowout fracture.

Most orbital surgeons believe that it's difficult to restore the primary orbital wall to its previous position and that the orbital wall is so thin that cannot be firmly its primary position. Therefore, orbital wall fractures generally have been reconstructed by replacing the bony defect with a synthetic implant. Although synthetic implants have sufficient strength to maintain their shape and position in the orbital cavity, replacement surgery has some drawbacks due to the residual permanent implants. In previous studies, the author has reported an orbital wall restoring technique in which the

Confocal reflectance imaging of head and neck surgical specimens. A comparison with histologic analysis.

BACKGROUND: Confocal reflectance microscopy (CRM) is an optical method of imaging tissue noninvasively without the need for fixation, sectioning, and staining as in standard histopathologic analysis. Image contrast is determined by natural differences in refractive indices of organelles and other subcellular structures within the tissues. Gray-scale images are displayed in real time on a video monitor and represent horizontal (en face) optical sections through the tissue. We hypothesized that CRM is capable of discerning histologic characteristics of different tissues in the head and neck. OB

Purified Pasteurella multocida protein toxin reduces acid phosphatase-positive osteoclasts in the ventral nasal concha of gnotobiotic pigs.

To study the in vivo response of conchal (turbinate) osteoclasts to Pasteurella multocida toxin, four gnotobiotic pigs (7 days of age) were inoculated subcutaneously with 0.2 microgram/kg of purified toxin. One toxin-treated pig along with one control pig were necropsied at 2, 5, 9, and 14 days postinoculation. The entire length of nasal concha from the nasal planum toi ethmoid region was removed, blocked by transverse cuts into five areas, decalcified, sectioned, and then stained with tartrate-resistant acid phosphatase (TRAP) to identify osteoclasts. In each section, total area of concha, to

Mandibular rotation and remodeling changes during early childhood.

OBJECTIVE: To describe the mandibular rotation and remodeling of younger children. MATERIALS AND METHODS: The sample included 43 males and 43 females who participated in the Bolton-Brush Growth Study at Case Western Reserve University in Ohio. They were chosen on the basis of having Class I (n = 45) or Class II (n = 41) molar relationships and longitudinal lateral cephalograms at three developmental stages of the dentition: late primary (T1: 5.7 +/- 0.5 y), early mixed (T2: 8.4 +/- 0.6 y), and full permanent dentition (T3: 15.4 +/- 0.5 y). Each subject's cephalograms were traced and four land

A novel approach for successful closure of sinonasal fistulae.

UNLABELLED: Medial canthus sinonasal fistula is associated with troublesome aesthetic and functional issues. Corneal irritation and drying results from escape of both air and secretions from the nasal cavity across the ocular surface as well as misting of spectacles if worn. Reconstruction of such fistulae is associated with a high rate of recurrence and thus presents a difficult surgical challenge. METHODS: This paper describes the senior author's surgical approach to manage medial canthus sinonasal fistulae utilizing an interpolated forehead flap combined with extended galeafrontalis and pe

Late reconstruction of orbital and naso-orbital deformities.

Acute orbital fractures and naso-orbital ethmoid fractures can result in chronic orbital and naso-orbital deformities. Understanding the acute injury is the first step in reconstructing the established late deformity. The best management strategy for reconstruction of orbital hypertelorism is to avoid late complications by repairing these deformities early near the time of the original fractures. New technologies from computer-guided surgical planning and additive manufacturing technology produce passive fitting implants tailored for patient-specific needs.

Telecanthus and hypertelorism in frontoethmoidal meningoencephaloceles and the surgical correction of these conditions: Part I. An orbital anthropomorphometric evaluation of the Khmer subpopulation of Cambodia.

This is the first of the two parts of a paper concerning a novel method on the surgical treatment of orbital dysmorphisms, especially telecanthus (TC) and hypertelorism (HT). The normal orbital values of a given ethnic group is an essential factor in determining the degree of correction intended in that patient population. We did not find any data related to the normal orbital values in Khmer-Cambodians in whom we performed the corrective surgeries. Thus, the aims of this article are to evaluate the orbital morphometric data procured in Khmer-Cambodians and to analyze the contradictory definit

Use of rigid external distraction device in treatment of complex maxillofacial fractures.

Collapse of maxillofacial skeletal structures after trauma results in inadequate anteroposterior projection and increase in facial width with deformed facial aesthetics and various functional problems. After successful use of rigid external distractor for treatment of congenital facial hypoplasia, they have been used for treating more complex craniofacial anomalies. Six patients with posttraumatic orbitozygomaticomaxillary deformity were managed with intraoperative and/or postoperative use of rigid external distractor device. All the patients maintained the early postoperative aesthetic and fu

Cranial base growth and morphology in second-trimester normal human fetuses and fetuses with cleft lip.

OBJECTIVE: The present radiographic study describes the size and shape of the cranial base from the sagittal aspect for a sample of 77 second-trimester "normal" control fetuses (n = 61) and fetuses (n = 16) exhibiting isolated, unilateral clefts of the lip (CL), ranging in fertilization age from 10 to 22 weeks. METHODS: Fetuses were placed in a cephalostat, and standardized, lateral head radiographs were taken. The radiographs were traced, and 15 cephalometric landmarks were identified and digitized for analysis. Growth curves for cranial base lengths, angles, and areas were compared between

Configuration of the Ethmoidal Roof: A Radiologic Anatomical Study.

BACKGROUND: Currently endoscopic sinus surgery is the standard operation for rhinosinusitis and nasal polyps, including tumors near the anterior skull base. During the surgery, the iatrogenic penetration into the anterior skull base may be at risk. Therefore, information of the anterior skull base configuration is useful for the sinus surgeon. OBJECTIVE: To determine the patterns and their prevalence of slopes of the ethmoidal roof assessed from the Computed Tomography (CTs) of paranasal sinuses. MATERIAL AND METHOD: A descriptive study was conducted at Srinagarind Hospital, Faculty of Medic

Ethmoidal meningoencephalocele and CSF leak after posthaemorrhagic ventricular dilatation in a newborn child.

BACKGROUND: Rhinoliquorrhoea suggests a communication between the subarachnoid space and the sinonasal tract. Clinical presentation includes clear nasal discharge, headache, pneumocephalus, meningitis or brain abscess. Cerebrospinal fluid (CSF) leaks are mostly of traumatic origin (skull base fractures), iatrogenic (secondary to endoscopic endonasal surgery) or associated with tumour aetiology. Occasionally, hydrocephalus has been the cause of rhinoliquorrhoea in adults, presumably secondary to the chronically raised intracranial pressure with skull base erosion and meningocele. To our knowled

Ethmoidal dural arteriovenous fistula with unusual drainage route treated by transarterial embolization.

Ethmoidal dural arteriovenous fistulas (AVFs) are rare intracranial lesions associated with a high risk of intracranial hemorrhage. In particular, this entity with reflux drainage directly into the ophthalmic vein is extremely rare. We report a case of ethmoidal dural AVF with direct drainage of the superior ophthalmic vein (SOV) and inferior ophthalmic vein (IOV), successfully treated by endovascular surgery. A 58-year-old man presented with progressive diplopia. Angiography and contrast-enhanced CT showed an ethmoidal dural AVF supplied via the bilateral anterior ethmoidal arteries and venou

Accuracy of transnasal cannulation and dilation of the maxillary ostium in cadavers with intact uncinates.

Despite the essential role played by the scapula in shoulder function, current concepts in shoulder training and treatment regularly neglect its contribution. The 'scapular dyskinesis' is an alteration of the normal scapular kinematics as part of scapulohumeral rhythm, which has been shown to be a nonspecific response to a host of proximal and distal shoulder injuries. The dyskinesis can react in many ways with shoulder motion and function to increase the dysfunction. Thoracic kyphosis, acromio-clavicular joint disorders, subacromial or internal impingement, instability or labral pathology can

The neglected cranial nerve: nervus terminalis (cranial nerve N).

The nervus terminalis (NT; terminal nerve) was clearly identified as an additional cranial nerve in humans more than a century ago yet remains mostly undescribed in modern anatomy textbooks. The nerve is referred to as the nervus terminalis because in species initially examined its fibers were seen entering the brain in the region of the lamina terminalis. It has also been referred to as cranial nerve 0, but because there is no Roman symbol for zero, an N for the Latin word nulla is a better numerical designation. This nerve is very distinct in human fetuses and infants but also has been repea

Traumatic evulsion of the globe: a very rare complication of maxillofacial trauma.

Maxillofacial trauma may cause eye injury besides fractures of the facial skeleton. However, evulsion of the globe is a rare event, and there are only a few reports of true total enucleation of the globes in the literature. We report a 23-year-old man who was a victim of motorcycle accident, with enucleation of the globe, evulsion of the septal cartilage, and midfacial fractures. We reviewed and discussed the literature of traumatic evulsion of the globes in maxillofacial traumas.

Transsphenoidal optic nerve decompression: an endoscopic anatomic study.

The objective was to study the relationships among the optic nerve, posterior paranasal sinuses, and ophthalmic artery. The identification of reliable anatomic landmarks would provide an endoscopic anatomic basis for optic nerve localization and effective prevention of ophthalmic artery injury during transsphenoidal optic nerve decompression surgery. The Messerklinger technique was performed to open the posterior ethmoid sinus and sphenoid sinus in 8 adult-head specimens. The opticocarotid recess and optic canal were identified under endoscopy, and the relationships between the optic nerve and
