OBJECTIVE: We reviewed the presentation, management, and late events in a recent experience with aortic graft-enteric communications (secondary aortoenteric fistula) to identify variables associated with poor outcomes.
METHODS: Since 1991, 29 patients (19 men, 10 women; mean age, 70) presented with a secondary aortoenteric fistula. The duration from aortic graft implantation to aortoenteric fistula development ranged from 8 months to 180 months (mean, 47 months). OBJECTIVE: To present the technique of and indications for serial excision of facial hemangiomas.
METHODS: Clinical review of patients cared for by the senior author (M.H.) in a private-practice specialty center.
RESULTS: Forty-six patients underwent serial excision of facial infantile hemangiomas. The average number of excisions was 3 (range, 2-5), with a mean interoperative period of 4 months (range, 3-7 months). We describe the rare cases of a 44-year-old woman and a 28-year-old woman each presenting with a large posterior sacrococcygeal mass and alternating symptoms of high and low intracranial pressure. The first patient underwent excision of her large sacral meningocele and simple ligation of the neck, resulting in resolution of all her associated symptoms. The second patient suffered traumatic rupture of the meningocele; she underwent excision of the redundant sac and repair of the dural defect using a musculofascial flap, also resulting in resolution of her symptoms.

A 10-year-old boy presented with long-standing urethral bleeding from a cavernous haemangioma of the penis associated with Klipper-Weber syndrome. Bleeding was controlled by limited local excision and the urethra was successfully reconstructed with a full-thickness skin graft applied directly on to this cavernous haemangioma. From time to time split-skin grafts have been used to repair the defects following excision of simple haemangiomata but we could find no case report in the literature of a free skin graft applied directly on to a cavernous haemangioma.

A 7-year-old boy with a history of fish bone ingestion presented with a progressively enlarging, painless right-sided neck mass one year after the initial event. Contrast-enhanced CT angiography revealed a giant 5 × 5 cm pseudoaneurysm originating from the right common carotid artery. Repairs in 49 cases of vesicovaginal fistulae were performed during the last 12 years. Obstetric cause for vesicovaginal fistulae was noted in 81.63% cases and the most important being obstructed labour (73.46% cases). Repair was successful in 71.42% cases. Factors favourable for success were a non-obstetric fistula, small size fistula (< 10 mm) and absence of fibrosis. Repair operations were performed mainly by vaginal flap technique. The fistulous track was not excised. Polyglactin suture gave the best results.

Damaged DNA can be repaired by three different mechanisms: photoreactivation, excision repair and postreplication repair. Each mechanism is regulated by a highly specific set of enzymes. Defects within these systems result in diseases which have one common feature: affected individuals are cancer prone. Recently, newly developed methods not only make it possible to diagnose affected patients but also to detect individuals at risk. Furthermore, the results obtained elucidate some mechanisms of carcinogenesis. Clinical applications are discussed.

BACKGROUND: The purpose of this study was to determine the patient-driven and surgical characteristics that drive day-of-surgery (DOS) costs for repair of isolated supraspinatus tears.
METHODS: All patients who underwent primary rotator cuff repair (RCR) for isolated supraspinatus tears within one health care system from 2016 to 2022 were inclusion eligible. Exclusion criteria included concomitant tear of infraspinatus, teres minor, or subscapularis; previous RCR or fractures of affected shoulder; diagnosis of rheumatoid arthritis; and incomplete baseline or 1-year patient-reported outcomes.

The molecular mechanisms of in vivo inhibition of mammalian DNA replication by exposure to UV light (at 254 nm) was studied in monkey and human cells infected with simian virus 40. Twenty-six of 163 patients with infective endocarditis treated between 1969 and 1979 required operation. The consequences of infection in these 26 patients included leaflet destruction, annular "ring" abscess, fistula formation with ventricular and atrial septal perforation, and myocardial abscess with heart block. Surgical principles used to repair these abnormalities successfully were excision of necrotic tissue, valve replacement, repair of annular defects, and closure of perforations. Temporary and permanent cardiac pacing were used also. The operative mortality was 13%.
