[Case report: a traumatic facial paralysis caused by a fish].

BACKGROUND: Due to the increase in worldwide tourism more and more unusual injuries are observed. CASE: We report on a patient who was diving near the Maledives, where a fish weighing 30 kg collided with his head. Four to five hours later he suffered from paralysis of the facial nerve. During an operation performed after his return a part of the jaw of the fish was removed from the retromandibular fossa. The fish jaw had compressed the facial nerve. RESULT: Three months later the function of the nerve was normal again. CONCLUSION: Even simple accidents require a detailed patient history and

Facial fractures and snowmobile accidents.

Snowmobile accidents are now common in winter; there is a local pattern of facial fractures associated with accidents involving these machines. Facial fractures connected with snowmobile accidents over a period of two successive winters were reviewed; 14 were found to be of special interest because of their severity and localized extent. Compared with motor vehicle accidents most of these fractures were isolated; the patients often had suffered only minimal trauma elsewhere. After reviewing the histories of these patients it was clear that alcohol, night driving, inadequate facial protection a

Topographic and anatomical variability of the facial artery: Structure and physiology.

A large number of complications associated with incorrect visualization of the facial artery or ignorance of the anatomy of the major facial vessel make this problem quite relevant. An in-depth understanding of anatomical variations, projections and topography, taking into account the depth of the facial artery, can help reduce the risk of adverse outcomes during cosmetic procedures. In addition, it assists plastic and maxillofacial surgeons in managing facial trauma and performing surgical procedures, including advanced endoscopic facelift techniques and endoscopic blepharoplasty, where the r

[Reoperation of tumors of the parotid gland, surgical approach and consequences for the 7th cranial nerve. Apropos of 42 cases].

Second or more surgical procedures on parotid are usually difficult and may induce injury on the facial nerve. The authors report their experience about 42 patients. The choice between a total parotidectomy or a surgical excision of a tumor depends on the number of surgical procedure, the type of initial procedure and the histological type of the tumor. Second surgical procedures for a wrong initial diagnosis (11 cases) were always a total parotidectomy with facial nerve preservation. The risk of a partial or total facial palsy is higher after several recurrences (3 of 8 cases). The facial ner

[Giant pleomorphic adenoma of the deep lobe of the parotid gland: role of MRI and nerve monitoring in facial nerve localization].

INTRODUCTION: Describe, from a clinical case, the usefulness of MRI and nerve monitoring in facial nerve and the interest of the MRI in detecting the position of the facial nerve. REPORT OF THE CASE: A 73 year-old woman has been supported for a huge tumour of the right parotid gland. Clinical examination and preoperative imaging were consistent with a pleomorphic adenoma located in the superficial lobe, with a facial nerve repressed deeply. Resection was performed using a facial nerve monitoring. The nerve was stretched and repressed in superficial position by the tumour. Parotid tissue was r

[Analysis of the relation between the operation time and functional recovery for the injured facial motor nerve].

OBJECTIVE: To analyse the relation between the operation time and functional recovery of the facial motor nerve. METHODS: 133 cases of facial motor nerve injury between 1983 and 1998 were involved in this survey. Most of them (130 cases) suffered from traumatic facial palsy. 73 cases were followed up for evaluating their functional recovery of mimic muscles according to the suggestion by the International Muscle Transplant Registry (1991). RESULTS: More than 80% cases of facial nerve injury were sharp cut. There were 28 cases undergone immediate or early repair of the facial nerve, 90 cases

Iatrogenic facial nerve exposure in cochlear implant surgery: incidence and clinical significance in the absence of intra-operative nerve monitoring.

Objectives: Iatrogenic facial nerve injury is one of the most feared complications of cochlear implantation. Intraoperative facial nerve monitoring is used as an adjunctive modality in a variety of neurotologic surgeries including cochlear implantation. With the lack of nerve monitoring, there is a theoretically higher risk of iatrogenic fallopian canal dehiscence with facial nerve exposure, particularly the mastoid portion, during cochlear implant surgery. The purpose of this study is to determine the incidence of iatrogenic exposure of the facial nerve and its relation to the incidence of po

Biomechanics of nasal septal trauma.

Fractures of the nasal complex are the most common facial fracture. This is due to the fact that the nose is the most anteriorly projecting facial structure and is a composite structure of bone, cartilage, and soft tissue. Furthermore, the forces required to cause significant injury are less than that causing other facial fractures. Because of the wide range of activities and mishaps that cause nasal trauma, the instruments of injury range from bicycle handles to elbows. The causes, clinical anatomy, and future considerations regarding nasal septal trauma are discussed in this article.

Qianzheng powder promotes facial nerve regeneration via BDNF/TrkB/CREB pathway activation.

INTRODUCTION: Facial nerve injury (FNI) is a common peripheral neuropathy that severely impairs facial function and quality of life. Qianzheng Powder (QZP) is a traditional Chinese herbal formula used to treat facial paralysis clinically, yet its neuroprotective mechanisms remain unclear. This study aims to evaluate the therapeutic effects of QZP on FNI and potential underlying mechanisms. METHODS: A FNI model was established in male C57BL/6 mice by performing facial nerve crush surgery. QZP (3.51 g/kg) was administered orally once daily for 14 days post-surgery. Facial function was assessed

[Evaluation and management of paediatric nasal fractures].

This is a review discussing evaluation and management of paediatric nasal fractures, which have been reported as one of the three most common facial bone fractures. Nasal fractures in children are usually treated with closed reduction after 3-5 days, while other injuries like septal haematoma must be treated at the initial presentation. It is important to pay careful attention to the nasal anatomy during the surgical management of paediatric nasal injuries, as disturbing the nasal growth zones can have an impact on future nasal development.

Facial nerve monitoring in otologic surgery: clinical indications and intraoperative technique.

While identification of the intratemporal portion of the facial nerve is mandatory in most otologic surgical procedures, inadvertent instrumentation, traction, or thermal injury may still result from inaccurate delineation, purposeful avoidance, or false protection of this critical structure. Improved functional preservation of the facial nerve has been achieved in acoustic neuroma surgery through the monitoring of evoked facial electromyographic activity. This technique may also be used during otologic procedures in which facial nerve manipulation is anticipated in the management of recurrent

[Changes of trkB and trkC mRNA in facial nucleus after axotomy and end-to-end anastomosis in the rat].

OBJECTIVE: To explore the changes of receptor tyrosine kinase B and C (trkB and trkC) mRNA in facial motoneurons following facial nerve injury and repair. METHODS: The time course of trkB and trkC mRNA in facial motoneurons following facial nerve transection and anastomosis was analyzed using in situ hybridization (ISH) and reverse transcriptase-polymerase chain reaction(RT-PCR) in adult rats. RESULTS: In the facial nerve simple transection group, the intensity of trkB mRNA signal began to increase to 3d post-operation and to its maximum at 7d. Then, the intensity started to decrease at 14d,

Craniofacial gunshot injury resulting in pseudoaneurysm of the left internal maxillary artery and Collet-Sicard syndrome.

Facial gunshot is a challenging medico-surgical emergency because of the complex neurovascular structure in the area. We present a patient who sustained a facial gunshot injury, with massive nasal and oral bleeding that resulted in shock. We review the management of gunshot injury to the internal maxillary artery, both in acute and chronic stages. In addition, we describe the clinical course of the rarely occurring Collet-Sicard syndrome, which involves injury to the 9th, 10th, 11th, and 12th cranial nerves associated with gunshot injury.

Hemifacial spasm and the facial nucleus.

Spontaneous and associated hyperkinetic facial movements and contracture which follow injury to the seventh cranial nerve (postparalytic hemifacial spasm) or arise without known previous injury (cryptogenic hemifacial spasm) are pathological motor phenomena not found in the distribution of other cranial or somatic motor nerves. The commonly expressed hypotheses of pathogenesis--aberrant regeneration and fiber excitation by false synapse formation (ephapses) at the site of injury--cannot account for all aspects of these phenomena or for the uniqueness of such movements to the distribution of th

A facial reconstruction and identification technique for seriously devastating head wounds.

Many authors have focused on facial identification techniques, and facial reconstructions for cases when skulls have been found are especially well known. However, a standardized facial identification technique for an unknown body with seriously devastating head injuries has not yet been developed. A reconstruction and identification technique was used in 7 cases of accidents involving trains striking pedestrians. This identification technique is based on the removal of skull bone fragments, subsequent fixation of soft tissue onto a universal commercial polystyrene head model, precise suture o
