STUDY DESIGN: Case report.
INTRODUCTION: This case report describes the use of the casting motion to mobilize stiffness (CMMS) technique in the management of a crush and degloving injury of the hand. The patient was unable to attend multiple hand therapy sessions due to geographic constraints. The CMMS technique involved the application of a nonremovable plaster of paris cast that selectively immobilizes proximal joints in an ideal position while constraining distal joints to direct desired motion over a long period. This uses active motion only. Traditional hand therapy techniques or modalit

Robotic piano playing is a challenging benchmark for biomimetic dexterous manipulation, requiring precise timing, coordinated multi-finger motion, and stable key contact. This study proposes a robotic piano-playing framework based on a two-stage reinforcement learning curriculum. Musical Instrument Digital Interface (MIDI) data are converted into target-key and fingering grids to provide future musical goals for policy learning in a parallel MJLab simulation environment. A Soft Actor-Critic (SAC) agent takes a 2106-dimensional observation vector, including joint states, previous actions, music

BACKGROUND: Functional reconstruction after a severe hand injury is best achieved by obtaining primary, stable soft tissue coverage to avoid delayed wound healing, prolonged immobilization, and joint stiffness. A wide range of procedures from spontaneous healing to local flaps has been proposed to treat soft tissue defects in appropriate circumstances. However, these pedicled flaps are insufficient in covering larger finger defects, so we used flaps from the free superficial palmar branch of the radial artery (SPBRA) for such cases.

Clinical assessment of the hand is important for diagnosing underlying hand disorders. Using a case study approach, the clinical assessment for three disorders of the hands is presented: trigger finger (stenosing tenosynovitis), carpal tunnel syndrome, and ulnar-sided wrist injury (styloid impingement). We assess the annular one pulley and finger range of motion for patients with trigger finger. To diagnose for carpal tunnel syndrome, assessment for Tinel's sign, Phalen's sign, abductor pollicis brevis muscle bulk, two-point discrimination, and obtaining a nerve conduction study are performed.

A 39-year-old tetraplegic patient had paralysis of elbow, thumb, and finger extension and thumb and finger flexion. We transferred axillary nerve branches to the triceps long and upper medial head motor branches, supinator motor branches to the posterior interosseous nerve, and brachioradialis tendon to the flexor pollicis longus and flexor superficialis of the index finger. Surgery was performed bilaterally 18 months after spinal cord injury. At 12 months after surgery, we performed bilateral distal radioulnar arthrodesis percutaneously. By 22 months postoperatively, we observed triceps stren

16 patients (75% women) were operated for a claw nail involving one of the three middle fingers. In 14 cases out of 16, a traumatic amputation, treated by directed healing in 60% of cases, was responsible for the deformity. The average loss of bone substance was 49% (range: 0-90%). The average shortening was 5 mm. The loss of substance was repaired by a local flap in 14 cases. Two early recurrences led to one failure and one good result after a second recession flap. One case of arthritis of the distal interphalangeal joint and one case of septic necrosis of the nail bed were also observed. El

We managed three patients who had both palmar fasciitis and polyarthritis coexisting with gynecological malignancies. The first patient was followed up for 3 years, during which she experienced mutilating osteolysis of the fingers and very severe inflammation as assessed by osteoarticular ultrasonography. The second patient had active synovitis by ultrasonography without structural joint damage. Finally, in the last patient, magnetic resonance imaging showed synovitis and extensor tenosynovitis of the fingers. The many differential diagnoses of this paraneoplastic syndrome are discussed.

Free transfers from the foot have good indications for finger reconstruction in young, healthy, and well-motivated patients. The toes provide a unique source to reconstruct amputated or partly destroyed fingers. If the range of motion of the toe joints is quite disappointing from the roll up needed on the ulnar side of the hand, it can provide strong pinch with good sensibility on the radial aspect. This article reviews the different types of transfers, their results, and indications, and provides a description of the authors' basic surgical technique.

Reliable quantification of finger and hand kinematic metrics is critical for evaluating recovery and rehabilitation outcomes in neurological diseases such as stroke. Traditional methods of kinematic tracking, such as Vicon, are effective but prohibitively expensive ($50-75k), creating a high entry barrier for academic research and rendering them impractical for non-human primate (NHP) studies. This work presents an automatic pipeline for markerless tracking that integrates DeepLabCut for 2D pose estimation and Anipose for 3D reconstruction. The pipeline was validated against Vicon (ground trut

INTRODUCTION: Kienböck's disease is a condition of osteonecrosis of the lunate bone in the hand, and most patients present with a painful and sometimes swollen wrist with a limited range of motion in the affected wrist. Vaughan-Jackson syndrome is characterized by the disruption of the digital extensor tendons, beginning on the ulnar side with the extensor digiti minimi and extensor digitorum communis tendon of the small finger. It is most commonly associated with rheumatoid arthritis. We describe a case of a patient with an unusual presentation of Kienböck's disease with symptoms similar to t

OBJECTIVES: The location of the fingertip entitles it to have significant cosmetic and functional values, but also places it at high risk for injury. During repair, finger length and function should be maintained, and stiffness and neuroma should be avoided. Various flaps have been described for reconstruction of distal finger defects with bone, tendon, or joint exposures, including reverse flow homodigital island flap. In this study, we present our experience of reverse flow homodigital island flap in terms of sensory recovery.
METHODS: Sixty-six patients (70 fingers) with fingertip amputati

INTRODUCTION: Trigger finger (TF) release is one of the most common procedures in hand surgery. It is not clear whether outcomes differ when multiple digits are released in the same operating session. The present study aimed to compare outcomes between patients undergoing single digit and multiple digit TF release.
METHODS: We conducted a retrospective cohort study using the TriNetX database (https://trinetx.com/) to identify patients undergoing trigger finger release between 2010 and 2025. Patients were categorized into single digit and multiple digit release groups. Postoperative complicati

BACKGROUND: Proximal interphalangeal (PIP) joint fracture-dislocations can be technically challenging injuries to treat, and no technique has proven to be superior nor lead to predictably good outcomes. We describe our experience of treating unstable dorsal fracture-dislocations of the PIP joint with extension-block pinning (EBP) at our institution over a 22-year period.
METHODS: In all, 23 patients with 24 unstable dorsal fracture-dislocations of the PIP joint treated with EBP between January 1998 and October 2020 were identified. All patients underwent closed reduction of the PIP joint and

Restorative measures in children's joints must be planned so that interference with the epiphyses, and consequent cessation of growth, can be avoided. Thus, tried and tested methods in adults, such as arthroplasty or arthrodesis, cannot be applied. The coriumarthroplasty in children has proved rewarding among our cases.
